2 Instructions Dr. Garcia s Office Receptionist Green Patient FIRST VISIT OF GREEN PATIENT Have patient sign in. Ask them how they will pay? Insurance, cash or does someone else pay? You want them to give you their insurance card. Look at it and return it to them. Give them a green intake form and ask them to return the form when it is completed. When they bring you the completed form, place it in a folder and ask them to be seated and you will call their name when the Doctor is ready. Keep the folders in the order that they were turned in. Call names of patients in folders in order as the doctor becomes available. SECOND VISIT OF GREEN PATIENT When someone comes back for their second visit, they will have a green card from the Lab. Have them sign in and pull their original intake form out and place form and card in a folder and place at the bottom of the pile of folders to be seen by the doctor. Hand the patient the green English questionnaire and have them take a seat until their name is called.

Is your throat sore? (2 weeks) (No) **GIVE THEM THE REFERRAL CARD TO THE X-RAY LABORATORY AND TELL THEM THAT THEY NEED TO GO TO THE X-RAY LABORATORY. SECOND VISIT OF GREEN PATIENT Look at the X-Ray. Patient: The x-ray shows that the patient has bronchitis. Need to take prescription Kefl ex milligrams 4 pills per day for 1 week Take with food ** GIVE THEM THE REFERRAL CARD TO THE PHARMACY.

Ask them to turn to the side and put their hands on their head. Shine the light again. **GIVE THEM THE GREEN X-RAY AND A REFERRAL/RESULTS CARD. TELL THEM THAT THEY NEED TO GO BACK TO DR. GARCIA FOR THEIR RESULTS.
