2 HD Chambers Dear Parents and Community Members: When the Class of 04 graduates next June, our students will have had many experiences, both in and out of school, that will shape their lives forever. One of the most significant occurrences to impact each one of them during their school years will be that the world grew from 6. billion to 7. billion inhabitants. Considering the majority of this population growth took place out of our country, it underscores the importance of Alief ISD and public education, in general, to better prepare all students for an ever-increasing, competitive and global world. As with all school years, the 03-4 school year will bring more than its fair share of challenges. One positive challenge I welcome is the challenge of transitioning to the new test reduction and graduation flexibility plan passed by the Texas Legislature in May and signed by Governor Perry in June (House Bill 5). As I have mentioned before, I believe there should be some form of assessment and measurement used to determine how our students are growing and progressing, but the new testing requirements for high school students are much more meaningful and reasonable. I also strongly believe that today s students (not older generations like mine) need course and content flexibility as they go through high school. The new graduation flexibility plan will allow students to take meaningful courses within various content areas that are aligned with student interests, with preparation for post-secondary education (technical school/two or four year college), and the workforce. In other words, we need to be preparing student for a variety of possible futures instead of relying on the single assumption that every student will and must go to a four-year university. Students who want to pursue endorsements in science, technology, engineering, math (STEM), culinary arts, or even other areas of interest may do so as we transition over the next few years. If we (Alief, Texas, America) want to continue to outpace the world and the ever-growing population I mentioned earlier, we must adjust our education system to meet that challenge. Our main focus will continue to be college, career and workforce readiness; and our Alief staff has spent the entire summer working and preparing to help every student in our district be successful as we begin this transition. I also want to briefly mention that the Alief ISD Board and administration has been working since February to build a strong and supportive budget for As always, the Board is working hard to assure that each tax dollar collected is spent in support of our goals and priorities, and when possible, reduce the school district tax rate. As you may know, the Board reduced the school district tax rate for the past two years. I am honored to serve Alief ISD as superintendent and genuinely look forward to great things during the 03-4 school year. If you are a parent of a school-aged child, please be engaged in your child s education. We must never forget that the first and most important teachers to our children are their parents. If you do not have a school-aged child, please consider volunteering or mentoring at a local school. It is rewarding to know you can have a positive impact on a student and even more rewarding the impact the student might have on you. Parents enrolling children in Pre-kindergarten and qualifying on the basis of income must bring documentation of all sources of income for all members of the household. Acceptable forms of documentation include payroll check stubs for the most recent three months or the federal income tax return for 0. Where to register Pre-kindergarten through eighth grade students register at the school in the attendance zone where their home is located. High school students register at the Alief ISD Enrollment Office, located in the Alief ISD Support Facility, 405 Bellaire Boulevard (between Eldridge and Howell Sugar Land Rd). Following the draw assignment, students interested in attending Kerr High School should contact and students interested in Alief Early College High School should contact the campus at Exception: High school students who have left the district and are returning to re-enroll should go to the registrar s office at their former high school campus. Eligibility Kindergarten through th grade: Children who reside in Alief ISD and are at least five years old and under years of age by September may attend Alief schools. High school students at least years of age and under 6 years of age can request admissions for up to one year to complete the requirements for a high school diploma. Pre-kindergarten Enrollment: To enroll in Alief ISD s pre-kindergarten program, a child must be four years old on or before September, 03 and meet at least one of the following requirements: - is unable to speak or comprehend the English language (Testing will take place at registration) - is educationally disadvantaged - is homeless - is the child of an active duty member of the armed forces of the United States - is the child of a member of the armed forces injured or killed while on active duty - is or ever has been in the conservatorship of the Department of Family and Protective Services following an adversary hearing - has a household income that is at or is below the subsistence level as defined by the State Board of Education (Documentation of all sources of income to all members of the household must be submitted at the time of registration. Acceptable documentation includes payroll check stubs for the most recent three months or a federal income tax return for 0.) Approval into pre-kindergarten is dependent on supporting documentation of requirements listed above. Bilingual program registration Parents interested in enrolling their children in the district s Spanish-English Bilingual Program, available for qualifying students in prekindergarten through sixth grade, must first register at their home school (the school in their attendance zone) before going to the assigned bilingual campus. 3 RIGHTS AND PROCEDURES RELATED TO SECTION 504 THE REHABILITATION ACT OF 973 Students who are eligible for services under 504 are not necessarily also covered under the provisions of Individuals with Disabilities Education Act (IDEA) which calls for the provision of special education services. Section 504 procedures may not be used to resolve disagreements with admission, review and dismissal committee decisions. Eligible students with disabilities have the right to participate in and benefit from public education programs without discrimination on the basis of a disability. They have the right to equal opportunity to participate in school programs and extracurricular activities. The law states that an eligible person with a disability is one who has, has a record of having, or is regarded as having, a physical or mental impairment which substantially limits a major life activity such as learning, self-care, walking, seeing, hearing, speaking, breathing, working, major bodily functions (immune system, normal cell growth), sleeping, standing, lifting, bending, reading, concentrating, thinking, communicating and working. In the school setting, a substantial limitation on learning may include evidence on success with school work, serious behavior problems or any other physical or mental impairment that interferes with learning. The principal of each building has designated a Campus Section 504 Coordinator who will be the person responsible for implementing eligibility procedures and the provision of educational services. For more information, contact the After School Program Office at , extension 5890, or Texas Minimum State Vaccine Requirements for Students Grades K- This chart summarizes the vaccine requirements incorporated in the Texas Administrative Code (TAC), Title 5 Health Services, Sections 97.6 to This chart is not intended as a substitute for consulting the TAC, which has other provisions and details. The Department of State Health Services (DSHS) is granted authority to set immunization requirements by the Texas Education Code, Chapter 38, Health & Safety, Subchapter A, General Provisions. IMMUNIZATION REQUIREMENTS A student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas. For 7th grade: dose of Tdap is required if at least 5 years have passed since the last dose of tetanuscontaining vaccine. For 8th-th grade: dose of Tdap is required when 0 years have passed since the last dose of tetanuscontaining vaccine. Td is acceptable in place of Tdap if a medical contraindication to pertussis exists. 4 doses of polio; one dose must be received on or after the 4th birthday. However, 3 doses meet the requirement if the 3rd dose was received on or after the 4th birthday. The first dose of MMR must be received on or after the st birthday. For K-4th grade, doses of MMR are required. For 5th-th grade, doses of a measles-containing vaccine, and dose each of rubella and mumps vaccine is required. For students aged -5 years, doses meet the requirement if adult hepatitis B vaccine (Recombivax) was received. Dosage and type of vaccine must be clearly documented. (Two 0 mcg/.0 ml of Recombivax). The first dose of varicella must be received on or after the first birthday. For grades K-4th and 7th-0th, doses are required. dose is required for all other grade levels. For any student who receives the first dose on or after 3 years of age, doses are required. The first dose of hepatitis A must be received on or after the first birthday. Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirement. Serologic confirmation of immunity to measles, mumps, rubella, hepatitis B, hepatitis A, or varicella or serologic evidence of infection is acceptable in place of vaccine. 3 Previous illness may be documented with a written statement from a physician, school nurse, or the child s parent or guardian containing wording such as: This is to verify that (name of student) had varicella disease (chickenpox) on or about (date) and does not need varicella vaccine. This written statement will be acceptable in place of any and all varicella vaccine doses required. When you are notified that an immunization is needed, promptly make arrangements with your doctor or an area clinic. You will need to provide the school with proof of the immunization. 4 School Meals Program Well-balanced breakfast and lunch, which meet USDA guidelines are available daily. School meals are designed to meet the requirements of growing children while taking into consideration their food preferences and the need to decrease fat, sugar & sodium intake. In addition to the regular menu entrée, a meatless entrée is available daily. Milk, juice and a limited variety of snacks can be purchased. Breakfast is free for all students. The price for lunch is $0.80 for paying students and $0.0 for students who qualify for reducedpriced lunches. (Meals application must be completed by parents & processed by the Alief Meals office to determine if a student qualifies for free or reduced-priced lunches. Parents may go to to pay for their child s meals in advance. Menu calendars are sent home with each elementary student monthly. General Texas Department of Agriculture Food Guidelines The Alief ISD Nutrition Services Department is regulated by the Texas Department of Agriculture (TDA) and the City/County Health Departments. In order to comply with these agencies, the District has the following procedures in place in regards to food being provided to students: Parents cannot bring food or drinks to school for anyone other than their own children. However, TDA does make an exception to this rule for birthdays. Parents may bring birthday cake/cupcakes for the class to celebrate with the birthday child. However, the Health Department requires that these cakes must come from an approved source a bakery or grocery store inspected by the Health Department. To ensure this safety issue, cake must be brought in the original box in which purchased. Cakes prepared in a private home cannot be brought to share with students. Approved birthday cakes will be served after the celebrating class has finished lunch. Check with the campus principal for an approved location which must be outside of the cafeteria. National School Breakfast and Lunch Programs Under the authority of USDA, Alief ISD provides free or reduced priced meals for children who meet eligibility requirements. Parents must apply for these benefits yearly. Qualification is determined by income or food stamp eligibility. Applications are available in the school office or online at meals.aliefisd.net. See link for applications on the Alief ISD home page. Complete only one application per family. Do not complete an application for each child because it increases the time it takes to determine eligibility. Exception: NO application is required for students whose names are on the State Direct Certification Food Stamp List which is sent to the Alief ISD Meals Office monthly. Students on this list automatically receive free meals. During the first week of school, the Meals Office will send a letter to parents of students on this list to advise them that no application should be completed and that their student is now qualified for free meals. (As new students are added to this monthly updated list, notification letters will be sent home to parents.) For more information regarding the National School Breakfast and Lunch Programs, call , or visit the Alief ISD Web site at Special Diets (Refer to Student Handbook) Per USDA regulations and physician s order, the Alief Nutrition Services Department will make food substitutions or modifications for students with disabilities. As defined under ADA (Americans with Disabilities Act), a person with a disability means any person with physical or mental impairment that substantially limits one or more major life activities. Substitutions will be made for children whose disabilities restrict their diets as supported by a statement signed by a licensed physician. The physician s statement is required and must include: the child s disability, an explanation of why the disability restricts the child s diet, the major life activity affected by the disability, and the food or foods to be omitted from the diet and allowable substitutions. Procedure. Parents are to request a Physician s Statement form from the school s nurse. This form must be taken to the student s physician to complete.. Student and parent are to make an appointment with the school s nurse and bring completed physician s statement. Also, bring all special dietary information (if any) provided by the Registered Dietitian who has worked with the student. 3. The nurse will review the physician s statement, ensuring that all required information is included. 4. Parents will be responsible for teaching their child about his or her diet. 5. VERY IMPORTANT: If, at a later date, the physician makes changes to a student s diet, these changes must be shown on an updated Physician s Statement. No verbal request can be accepted. 6. When going through the serving lines, the student must be responsible for choosing correct foods. 7. Please encourage your child not to eat food from another student s tray. These foods may not be allowed on your child s diet. Food Allergy Generally, children with food allergies or intolerances do not have a disability as defined under either Section 504 of the Rehabilitation Act or Part B of IDEA (Individuals with Disabilities Education Act). In most cases, the child s dietary needs can be met in the school s cafeteria where a variety of nutritious foods is available for individual choice. (Menus may be viewed on the district s website). However, when the physician s statement includes that the specified food allergy may result in severe, life-threatening (anaphylactic) reactions, the child s condition would meet the definition of disability. The parent will present a completed Physician s Statement to the school s nurse and appropriate food substitutions will be made available for the student. These forms are in the nurse s office.) Vending Machines The district has adopted policies and implemented procedures to comply with federal & state regulations for restricting student access to vending machines. In accordance with Federal law and U.S. Department of Agriculture policy, this institution is prohibited from discriminating on the basis of race, color, national origin, sex, age, or disability. Notifying the Campus Clinic of Food Allergies The district requests notification when a student has been diagnosed with a food allergy, especially those allergies that could result in dangerous or lifethreatening reactions. It is important to contact the school nurse and to disclose the food to which the student is allergic, as well as the nature of the reaction. Additionally, upon enrollment, an allergy form is provided on which you can disclose this information. If the specified food allergy may result in severe, life-threatening (anaphylactic) reactions, the child s condition would meet the definition of disability. The nurse will provide a Physician s Statement that must be completed by a licensed physician. When this completed Physician s Statement is presented to the school nurse, appropriate food substitutions will be made available for the student. Review of Student Code of Conduct, Student/Parent Handbook During the first week of school, every student will receive a handbook that includes the District Code of Student Conduct. To ensure that instruction takes place in a safe and positive environment, Alief ISD requests the support of students, staff, and parents in upholding the code. Parents are asked to review and discuss code provisions with their children, sign, and return the Parent Acknowledgment/Consent Form promptly to the school principal. Failure to do so does not exempt students from complying with the Code of Conduct. Updates will be posted online and can be viewed at

Public Notification of Nondiscrimination in Career & Technical Education Programs Alief ISD offers career and technical education programs in 4 of the 6 Career Clusters. 6 Mensaje del Superintendente ANTES DEL INICIO DE LAS CLASES HD Chambers Estimados padres y miembros de la comunidad: Para cuando la Clase de 04 reciban sus diplomas en junio próximo, estos estudiantes ya habrán vivido muchas experiencias, tanto en la escuela como fuera de ella. No obstante, el hecho de que la población del mundo aumentó de 6. billones a 7. billones de habitantes es el suceso de mayor significado y el que tendrá mayor impacto sobre la vida estudiantil de cada uno de ellos. Considerando que la mayor parte de este aumento se dio fuera de nuestro país, es imperativo hacer resaltar la importancia que tiene, tanto para Alief ISD como para la educación pública en general, el preparar mejor a todos los estudiantes para que puedan desempeñarse en un mundo cada vez más competitivo y globalizado. Al igual que todos los años escolares, el traerá consigo muchos desafíos. Uno de los desafíos positivos al que doy la bienvenida es el de la transición hacia la reducción de los exámenes y el plan flexible de graduación. Ambos fueron aprobados por la Cámara de Representantes de Texas (Texas Legislature) en mayo y firmados por el gobernador Perry en junio (House Bill 5). Como ya mencioné antes, si bien sostengo que debe haber algún tipo de prueba y evaluación para medir cómo están progresando y creciendo académicamente nuestros alumnos, los nuevos requisitos para los estudiantes de preparatoria son mucho más significativos y razonables. Además, creo firmemente que los estudiantes de hoy no los de las generaciones más antiguas como la mía necesitan de flexibilidad en sus cursos y en el contenido de las materias académicas, a medida que van pasando por la preparatoria. El nuevo plan flexible de graduación permitirá a los estudiantes tomar cursos de mayor trascendencia dentro de las diversas materias académicas alineadas con sus intereses y así, prepararse para su educación post-secundaria (escuela técnica/college de dos o cuatro años) y su ingreso a la fuerza laboral. Dicho de otro modo, necesitamos preparar a los estudiantes para una variedad de futuros posibles en vez de basarnos en la única idea de que todo estudiante debe ir a algún programa de cuatro años en la universidad. Además, aquel estudiante que desee adquirir algún aval en ciencias, tecnología, ingeniería y matemáticas (STEM por sus siglas en inglés) o tal vez, en ingeniería, artes culinarias u otras áreas de interés, podrá hacerlo a medida que vayamos haciendo la transición en los próximos años. Si nosotros Alief, Texas y los Estados Unidos deseamos continuar siendo líderes en el mundo y de la creciente población que mencioné al principio, debemos ajustar nuestro sistema de educación para enfrentar ese desafío. Seguiremos enfocándonos principalmente en el college, las carreras y en alistar al estudiante para ingresar en la fuerza laboral. Nuestro plantel de Alief se ha pasado todo el verano trabajando y preparándose para ayudar a cada estudiante del Distrito a alcanzar el éxito al iniciar esta transición. También, me gustaría mencionar brevemente que la Junta Directiva de Alief ISD y la Administración han venido trabajando desde febrero para lograr un presupuesto que sea fuerte y que nos sostenga. Como siempre, la Junta está trabajando arduamente para cerciorarse de que cada dólar recaudado con los impuestos se gaste en pro de los objetivos y las prioridades del Distrito. Además, la Junta trata de reducir el índice del impuesto escolar en el Distrito, cada vez que le sea posible. Como usted sabrá, la Junta Directiva ya redujo los índices del impuesto escolar correspondientes a los últimos dos años. Tengo el honor de servir a Alief ISD como su superintendente. De todo corazón, les digo que espero con ansias los grandes logros que alcanzaremos en este año escolar que se inicia. Si usted es padre de algún niño en edad escolar, haga el favor de participar en la educación de su hijo. 7 Derechos y procedimientos relacionados a la Sección 504 de la Ley de Rehabilitación El objetivo de la presente es brindarles información acerca de la Sección 504 de la Ley de Rehabilitación (Rehabilitation Act) de 973 y de los derechos y salvaguardas a los que usted tiene derecho de acuerdo a las disposiciones de dicha ley. No obstante, el hecho que algún estudiante cualifique para recibir los servicios en virtud de la Sección 504 no significa que necesariamente también cumpla con los requisitos para recibir los servicios de Educación Especial en virtud de la Ley de Educación de Personas con Discapacidades (Individuals with Disabilities Education Act IDEA por sus siglas en inglés). No se pueden usar los procedimientos de la Sección 504 para resolver los desacuerdos con respecto a las decisiones de un comité de admisión, revisión y retiro. Todo estudiante con discapacidad, que cualifique, tiene derecho a participar en los programas de educación pública y a beneficiarse de ellos sin sufrir discriminación alguna en base a la discapacidad. También tiene derecho a gozar de las mismas oportunidades de participar en programas escolares y actividades extracurriculares. La ley establece que una persona con discapacidad «que cualifica» es aquélla que padece, tiene un historial de estar padeciendo o se considera que padece de alguna discapacidad física o mental que limite sustancialmente cualquiera de sus actividades de rutina importantes, como aprender, cuidarse a sí misma, caminar, ver, escuchar, hablar, respirar, dormir, pararse, levantar algo, agacharse, leer, concentrarse, pensar, comunicarse y trabajar; o alguna función clave del cuerpo (sistema inmunológico, crecimiento normal de las células). En el ambiente escolar, las evidencias que indican tal limitación sustancial en el aprendizaje son, entre otras: el nivel de desempeño académico, los problemas serios de conducta o cualquier otro impedimento físico o mental que interfiera con el aprendizaje. El director de cada escuela designa a un Coordinador de Sección 504 de la escuela, quien tiene a su cargo implementar los procedimientos para la cualificación y para la provisión de los servicios correspondientes. Se requieren dosis para todos los estudiantes que reciban la primera dosis en o después de los 3 años de edad. La primera dosis de la vacuna contra la hepatitis A debe recibirse en o después del. er cumpleaños. Recibir la dosis hasta (e inclusive) 4 días antes del cumpleaños satisfará el requisito de inmunización para entrar a la escuela. La confirmación serológica de la inmunidad al sarampión, las paperas, la rubéola, la hepatitis B, la hepatitis A o la varicela o la evidencia serológica de infección son aceptables en lugar de la vacuna. 3 La enfermedad previa puede documentarse con una declaración escrita de un médico, una enfermera escolar o el padre o tutor del niño que diga algo como: Esto es para verificar que (nombre del estudiante) tuvo varicela el (fecha) o por esa fecha y no necesita la vacuna contra la varicela. Dicha declaración escrita será aceptable en lugar de todas las dosis requeridas de la vacuna contra la varicela. Cuando a usted se le notifique que a su hijo le hace falta alguna inmunización, haga el favor de hacer los arreglos correspondientes con su médico o con la clínica de su área, lo antes posible. 8 Programa de Alimentos Escolares Diariamente la escuela brinda un desayuno y un almuerzo bien balanceados que cumplen con las normas de El Departamento de Agricultura de los Estados Unidos (U.S. Department of Agriculture-- USDA por sus siglas en inglés). Estas comidas de la escuela están diseñadas para satisfacer los requisitos de los niños en crecimiento, pero tomando en cuenta sus preferencias alimenticias y la necesidad de reducir el consumo de azúcar, grasas y de sodio. Todos los días, además del plato principal regular, se dispone de una opción sin carne. También se puede comprar leche, jugos y una variedad de aperitivos ligeros (snacks). El desayuno es gratuito para todos los estudiantes. El almuerzo cuesta $.80 para aquellos alumnos que paguen el precio regular y $.0 para aquellos que cualifiquen para pagar un precio reducido. Para determinar si un estudiante cualifica para obtener comidas gratuitas o pagarlas a precio reducido, los padres deben completar la solicitud correspondiente y esta debe ser tramitada por la oficina de servicio de comidas de Alief (Alief Meals). Los padres pueden pagar por adelantado por las comidas, en el sitio en Internet www. parentsonline.net. En la primaria, se envía mensualmente a la casa el calendario con el menú del mes, por intermedio de los estudiantes. Normas generales del Departamento de Agricultura de Texas sobre los alimentos El Departamento de Servicios de Nutrición de Alief ISD (Alief ISD Nutrition Services Departament) está regulado por el Departamento de Agricultura de Texas (Texas Department of Agriculture TDA por sus siglas en inglés) y los departamentos de salud correspondientes del condado y de la ciudad. Por tanto, el distrito establece los siguientes procedimientos para cumplir con lo estipulado por estas agencias: Ningún padre puede traer o enviar alimentos ni bebidas a la escuela, excepto para sus propios hijos. Sin embargo, TDA hace una excepción a esta regla tratándose de los cumpleaños. Los padres pueden traer a la clase un pastel de cumpleaños o «cupcakes» para celebrar el cumpleaños de su hijo con los compañeros de clase. Pero, el Departamento de Salud exige que este pastel o los «cupcakes» vengan de alguna fuente «aprobada»: una pastelería o tienda de alimentos que cuente con la inspección del Departamento de Salud. Como precaución sanitaria, se exige que se traigan el pastel o los «cupcakes» en su embalaje original: la misma caja en la que se compraron. También se pueden comprar los «cupcakes» de la Cafetería, pidiéndolos con anticipación. No se pueden traer a la escuela pasteles preparados en alguna casa particular. El pastel de cumpleaños autorizado se sirve después de que haya terminado de almorzar la clase que celebre el cumpleaños. Consulte con el director de la escuela para que le indique el lugar apropiado. Este debe estar fuera de la Cafetería. Programas Nacionales de Desayunos y Almuerzos Escolares Con la autorización del USDA, Alief ISD provee comida gratis o a precio reducido para aquellos niños que cumplan con los requisitos que los habiliten para ello. Los padres deben solicitar anualmente este beneficio. La cualificación se determina en base a los ingresos familiares anuales o a la habilitación de la familia para recibir cupones de alimentos (food stamps). La solicitud está disponible en la oficina de la escuela o en Internet en net. Siga el enlace correspondiente en la página en Internet de Alief ISD. Complete sólo una solicitud por familia. Presentar una solicitud por cada niño sólo retrasará más la gestión. EXCEPCIÓN: la solicitud no es un requisito para aquellos estudiantes cuyos nombres figuren en la lista estatal para la certificación directa de los cupones de alimentos (State Direct Certification Food Stamp List) que se envía mensualmente a la oficina de comidas de Alief ISD (Alief ISD Meals Office). Los estudiantes en esta lista reciben comidas gratuitas en forma automática. Durante la primera semana de la escuela, la oficina de comidas enviará una carta a los padres de los estudiantes en la lista para aconsejarles no llenar ninguna solicitud de comidas gratuitas: basta con que los nombres de sus hijos estén en la lista estatal para habilitarlos a recibir las comidas gratis. A medida que se vayan agregando más nombres a la lista, el Distrito seguirá enviando a la casa las notificaciones escritas para los padres. Para mayores informes sobre los Programas Nacionales de Desayunos y Almuerzos Escolares, llame al o consulte el sitio en Internet de Alief ISD: Dietas Especiales (ver el «Manual del estudiante y de los padres») De acuerdo con el reglamento de Departamento de Agricultura de Estados Unidos (United States Department of Agriculture--USDA, por sus siglas en inglés), el Departamento de Servicios de Nutrición de Alief ISD hará sustituciones o modificacioes a los alimentos para los estudiantes con discapacidades. ADA define a una persona con discapacidad como cualquiera que padezca de un impedimento físico o mental que limite en forma sustancial una o más de sus actividades importantes para la vida.. Las substituciones se harán para los niños cuyas discapacidades restrinjan su dieta y en base a un certificado firmado por un médico con licencia para ejercer la medicina. El certificado médico debe incluir:. la discapacidad del niño. una explicación de por qué dicha discapacidad limita la dieta del niño 3. la actividad importante para la vida que se ve afectada por dicha discapacidad 4. los alimentos que deben ser omitidos de la dieta y las sustituciones permitidas 8 Procedimiento. Los padres deben solicitar a la enfermera de la escuela un formulario de Certificación Médica. Este formulario se debe llevar al médico del estudiante para que lo llene..
