La hernia inguinal ocurre cerca del pliegue entre el abdomen y la pierna. Qué es una hernia? Una hernia es una protuberancia de tejido que sale por una abertura generalmente en el abdomen. La hernia inguinal ocurre muy cerca del pliegue entre el abdomen y la pierna. Aparece un bulto o inflamación por debajo de la piel en la ingle que, en los niños, puede extenderse hacia el escroto y en las niñas hacia los labios vaginales. Puede notar que la inflamación aparece y desaparece. La puede haber descubierto al cambiar los pañales o mientras baña a su niño. A diferencia de las hernias en los adultos, la hernia de su niño no se debe a una debilidad muscular. Otros niños se ponen fastidiosos o se quejan de dolor con la actividad física. Si la hernia se abulta repentinamente, su niño puede irritarse con facilidad, llorar de dolor o negarse a comer. Si el intestino queda atrapado en el saco, su niño probablemente sentirá dolor. Puede vomitar y que el bulto se ponga duro. Cómo se diagnostica la hernia inguinal? El cirujano examinará cuidadosamente el abdomen y la ingle de su niño para detectar una hernia. Será de los dos lados porque puede la hernia puede ser en los dos. Cómo se corrige la hernia inguinal? Usualmente, la hernia inguinal se corrige con cirugía ambulatoria. Su niño recibirá anestesia general lo que significa que le daremos medicinas para mantenerlo dormido y que no sienta nada durante la cirugía. Si es un bebé prematuro, puede quedarse internado y pasar la noche en el hospital para observación. La cirugía se hace a través de una pequeña incisión en la parte baja del abdomen. Si hay hernias en los dos lados, las incisiones serán dos, una de cada lado. La cirugía consiste en regresar el intestino o el ovario a su lugar dentro del abdomen y cerrar el saco y el abdomen. Las incisiones se cierran usando puntadas que se disuelven por dentro; no son visibles por fuera y no hay necesidad de quitarlos. En los adolescentes, el cirujano puede también usar puntadas externas que tampoco será necesario quitar. Como que las hernias en los niños no involucran músculos, el dolor que su niño puede sentir después de la cirugía será menor que para los adultos. Cuándo debe corregirse una hernia inguinal? El cirujano le informará cuál es el mejor momento para la cirugía. Generalmente, se puede planear a su conveniencia entre 1 y 4 semanas después de la consulta con el cirujano, siempre que su niño esté bien de salud. Qué cuidados necesitará mi niño? La incisión Mantenga la incisión seca por 48 horas (2 días). Una vez transcurrido ese tiempo, su niño se puede bañar normalmente. La incisión estará cubierta con pegamento quirúrgico transparente. Un poco de sangrado después de la cirugía es normal. Una de las enfermeras de Cirugía General le llamará en 5 días laborales después de la cirugía para saber cómo sigue el niño. No es necesario una cita, solamente la llamada telefónica pero usted puede pedir una cita si le gustaría que vean a su niño o si tiene alguna preocupación. Los puntos se disuelven solos y no es necesario quitarlos. Su cirujano le dirá si hay puntos externos que necesiten quitarse. Habrá inflamación alrededor de la ingle y se le puede formar un moretón leve alrededor de la herida o del escroto que puede tomar varias semanas en desaparecer completamente. 2 de 3

5 Patient and Family Education Inguinal Hernia Care Caring for your child after surgery A hernia is a bulging of tissue through an opening, usually in the belly area (abdomen). An inguinal hernia (pronounced ING-winul) occurs near the crease between the abdomen and leg. What is a hernia? A hernia is a bulging of tissue through an opening, usually in the belly area (abdomen). An inguinal hernia (pronounced ING-win-ul) occurs near the crease between the abdomen and leg. You can see it as a bulge or swelling under the skin in the groin area. The swelling can extend down into the scrotum in boys or the labia in girls. You may notice that the swelling comes and goes. You may have found the hernia while diapering or bathing your child. Unlike hernias in adults, your child s hernia is not due to a muscle weakness. In children, an inguinal hernia is formed by a sac of tissue in the groin that normally closes before birth. If the sac does not completely close off, a connection remains between the groin and the lower belly. Fluid, intestine, or even an ovary in girls can fill this sac and cause it to swell. What are the symptoms? An inguinal hernia causes a bulge or swelling that you can see under the skin in the groin area. Many children with a hernia seem to be comfortable and act like they usually do. Other children are fussy or complain of pain with exercise. If the bulge comes on suddenly, your child may become cranky, cry out in pain, or not want to eat. If the intestine is trapped in the sac, your child will probably have pain. They may vomit, and the bulge may feel hard. How is an inguinal hernia diagnosed? The surgeon will carefully examine your child s belly area and groin to feel if a hernia is present. They will check both sides because there may be a hernia on both sides. Why does the inguinal hernia need to be repaired? An inguinal hernia will not go away if left alone. Activities such as running, crying or straining for a bowel movement can make the hernia appear larger and be uncomfortable. Sometimes the hernia can become incarcerated or stuck. This means a portion of intestine, and in girls the ovary, may slip through the opening and become twisted or caught there. This may stop the blood supply to these tissues and lead to serious damage. You may notice your child seems to be in pain. You may see and feel a tender, hard lump in your child s groin. If this happens and your usual ways to comfort your child do not work, call your doctor right away. These may be signs of incarceration. This can happen at any time when your child has a hernia. That is why it is important get treatment for the hernia as soon as you find it. 1 of 3

6 Inguinal Hernia Care How is an inguinal hernia treated? An inguinal hernia is usually repaired as a same-day, outpatient surgery at the hospital. Your child will have general anesthesia for surgery. This means that we will give your child medicine to make them sleep without pain during the surgery. If your child is a premature infant, they may need to stay overnight in the hospital to be carefully watched. The surgery is done through a very small cut (incision) on the lower abdomen. If there are hernias on both sides, there will be two incisions, one on each side. The surgery involves gently pushing the intestine or ovary back into the abdomen, and closing up the sac and the lower belly. The incisions are closed using stitches that dissolve on the inside. These stitches cannot be seen from the outside, and they do not need to be removed. For teenagers, the surgeon may also use some outside stitches that do need to be removed a week after surgery. Since hernias in children do not involve the muscles, your child should have less pain after surgery than an adult. When should my child s inguinal hernia be repaired? You will talk about the timing of the surgery with your child s surgeon. Usually the surgery can be planned for a good time when your child is healthy, within 1 to 4 weeks of the visit with the surgeon. How do I care for my child at home? Incision Keep the incision dry for 48 hours (2 days) and then your child can return to bathing normally. The incision will be covered with clear surgical glue. A little blood from the surgery is normal. One of the nurses from General Surgery will call you in 5 business days after surgery to check on you. There is no routine follow up in clinic other than the phone call, but you are welcome to schedule a follow up clinic visit if you would like to be seen or if you have any concerns. The stitches will dissolve and do not need to be removed. Your surgeon will tell you if there are any outside stitches that need to be removed. There will be some swelling of the groin area and mild bruising around the incision or scrotum. This can take several weeks to go away completely. Pain and comfort We partner with you and your child to prevent and relieve pain as completely as possible. You know your child best. No matter the level of your child s pain, believe they are hurting and respond right away. After a surgery your child is likely to have some pain and discomfort. In addition to medicine prescribed for pain, we will work with you to create a plan that encourages coping activities to treat pain and provide support. Help your child get better, faster with good pain management. 2 of 3

7 Inguinal Hernia Care To Learn More General Surgery Hospital operator for after hours Your child s healthcare provider Diet and food Your child should drink plenty of liquids and eat light meals the evening of surgery. The next day, your child can eat what they would usually eat, if they feel well enough. Activity Younger children will limit their own activity and can be as active as they feel comfortable. Teens should avoid rough play, contact sports and bicycle riding for 2 weeks. Some school-aged children may need to stay home to rest 1 day after surgery. If they feel well enough, they can return to school after that. When should I call the doctor? Free Interpreter Services In the hospital, ask your child s nurse. From outside the hospital, call the tollfree Family Interpreting Line Tell the interpreter the name or extension you need. You will receive a follow-up phone call from the general surgery nurse 5 business days after your child s surgery. Please call the surgeon s office if your child has: Fever of F or higher for more than 24 hours Redness that spreads around the incision site New bleeding or drainage from the incision site Severe, constant pain at the surgery site Inability to urinate within 8 to 12 hours from surgery Vomiting If you have questions or concerns, please call the nurse in the General Surgery Clinic at After hours or on weekends call and ask for the on-call General Surgeon to be paged.
