1 The Home Language Survey (HLS) and Identification of Students The Home Language Survey (HLS) is the document used to determine a student that speaks a language other than English. Identification of a language other than English on a HLS prompts a student for initial assessment to determine their English proficiency. The State assessment used to determine English proficiency is the Language Assessment System (LAS) Links Placement Test. The HLS is a document that should only be completed once during a student s educational career. Therefore, identification of a student must be based off of the student s original HLS that they completed when they first enrolled in school. The table below should first be used to identify each student who attends your school and which category they fit into. Flow charts have been created in order for a school to follow the correct protocol for administering the HLS, based on the category that student was placed into. The last column in the table ( Flow Chart ) identifies which chart should be followed for HLS and identification purposes based on the category that each student was placed into. The flow charts can be found in the subsequent pages of this document. Category Description Flow Chart to Use Category 1 Category 2 Students who have transferred to your school from another Indiana school corporation Students who have only attended your school since initial enrollment (i.e. kindergarten or first grade) Transfer-In Students (At Any Point in Their Education) Students Who Have Attended Since Initial Enrollment in School *Every student who attends your school should be placed into either category 1 or 2. **For students that have transferred to your school from another state, place them in category 2. The following definitions have been provided for interpretation of flow charts: Approaching Proficiency (AP): Refers to the score a student receives on the LAS Links Placement Test. If a student receives a score of AP, they are considered LEP. Individual Learning Plan (ILP): The plan created for any student that has been identified as LEP. This plan documents a student s accommodations or strategies that they are able to receive both in the classroom and on state assessments. (Please see Appendix C in the ISTEP+ Program Manual for allowable accommodations or strategies for state assessments). Limited English Proficient (LEP): A student who has been identified as on the LAS Links Placement Test as someone who is not fluent in English (also referred to as an English Learner). Not Proficient (NP): Refers to the score a student receives on the LAS Links Placement Test. If a student receives a score of NP, they are considered LEP. Original HLS: Refers to the document that was completed when the student first enrolled in school (typically kindergarten or first grade). Proficient (P): Refers to the score a student receives on the LAS Links Placement Test. If a student receives a score of P, they are considered fluent and would not have an ILP created for them. Transfer Student: Refers to a student who did not begin their initial education at your school (i.e. transferred to you from a different Indiana school corporation).

2 Transfer-In Students (At Any Point in Their Education) 1. Check the for the original HLS. OR 2. If the original HLS cannot be found, call the students previous school to obtain the original HLS¹ Original HLS Indicates a Language Other Than English The student should be administered the LAS Links Placement Test to determine English proficiency * A copy of the original HLS should be placed in Original HLS Indicates All English The student should not be given the LAS Links Placement Assessment and nothing further should be done. *A copy of the original HLS should be placed in the Student Scores NP or AP on LAS Links Placement Test If student scores NP or AP on the LAS Links Placement Test, the student is considered Limited English Proficient (LEP). Student Scores P on LAS Links Placement Test If student scores P on the LAS Links Placement Test, nothing further should be done. The student is not considered Limited English Proficient (LEP). Student Identified as LEP An ILP should be created for any student that has been identified as LEP (by scoring NP or AP on the LAS Links Test). The ILP should document accommodations or strategies that the student is able to use both in the classroom and on state assessments. 1. If original HLS cannot be obtained from student s previous school, administer a new HLS to the student. Documentation must be kept indicating the dates and times calls were made to the student s previous school in order to obtain the original HLS.

3 Students Who Have Attended Since Initial Enrollment in School A HLS should be provided to parents for completion, ensuring that the purpose of the HLS is clearly explained. HLS Indicates a Language Other Than English The student should be administered the LAS Links Placement Test to determine English proficiency * The HLS should be placed in student s cumulative folder HLS Indicates All English The student should not be given the LAS Links Placement Assessment and nothing further should be done. *A copy of the original HLS should be placed in the Student Scores NP or AP on LAS Links Placement Test If student scores NP or AP on the LAS Links Placement Test, the student is considered Limited English Proficient (LEP). Student Scores Proficient on LAS Links Placement Test If student scores Proficient on the LAS Links Placement Test, nothing further should be done. The student is not considered Limited English Proficient (LEP). Student Identified as LEP An ILP should be created for any student that has been identified as LEP (by scoring NP or AP on the LAS Links Test). The ILP should document accommodations or strategies that the student is able to use both in the classroom and on state assessments.

4 Home Language Survey (HLS) The Civil Rights Act of 1964, Title VI, Language Minority Compliance Procedures, requires school districts and charter schools to determine the language(s) spoken in each student s home in order to identify their specific language needs. This information is essential in order for schools to provide meaningful instruction for all students as outlined Plyler v. Doe, 457 U.S. 202 (1982). The purpose of this survey is to determine the primary or home language of the student. The HLS must be given to all students enrolled in the school district / charter school. The HLS is administered one time, upon initial enrollment, and remains in the student's cumulative file. Please note that the answers to the survey below are student-specific. If a language other than English is recorded for ANY of the survey questions below, the LAS Links placement test will be administered to determine whether or not the student will qualify for additional English language development support. Please answer the following questions regarding the language spoken by the student: 1. What is the native language of the student? 2. What language(s) is spoken most often by the student? 3. What language(s) is spoken by the student in the home? You understand that if a language other than English has been identified, your student will be tested to determine if they qualify for English language development services, to help them become fluent in English. If entered into the English language development program, your student will be entitled to services as an English learner and will be tested annually to determine their English language proficiency.
