1 Material educativo para el paciente y la familia Plasmaféresis Plasma Exchange / Spanish Qué es el plasma? El plasma es la parte líquida de la sangre. Transporta los glóbulos rojos y blancos a través del cuerpo. También, las vitaminas y otras sustancias que el cuerpo necesita. El plasma no solo transporta elementos que el cuerpo necesita sino también otros que causan enfermedades. Qué es plasmaféresis? Es un procedimiento en que se saca una pequeña cantidad de sangre a través de un catéter, un tubo plástico que se insertado en la piel llega hasta un vaso sanguíneo. Tiene dos tubos, uno para sacar la sangre del cuerpo y otro para regresar la sangre al cuerpo. Una vez que la sangre sale del cuerpo de su niño, circula por los tubos de una máquina llamada separador de células sanguíneas. El separador extrae el plasma junto con lo que causa la enfermedad de su niño, que será desechado. Las células de la sangre junto con un poco de líquidos intravenosos, regresan al cuerpo a través de una de los catéteres. El suero intravenoso evita que su niño se deshidrate (pérdida excesiva de agua o líquidos). La máquina sólo sacará de su niño una cantidad pequeña de sangre cada vez. Este proceso continuará hasta que toda la sangre se haya separado y devuelta al cuerpo de su niño. 2 Plasmaféresis Qué clase de líquidos recibirá a mi niño por vía intravenosa? El líquido más común que regresa al cuerpo de su niño durante el tratamiento de plasmaféresis es el plasma fresco y congelado y la albúmina. El doctor de su niño decidirá cuál de estos líquidos es el mejor. Plasma: de un donante sano que desea ayudar a personas enfermas. Fue analizado para tener la seguridad de que sea seguro. Albúmina: es el mismo tipo de proteína que se encuentra en la sangre de cualquier persona. Por qué necesita mi niño el tratamiento plasmaféresis? La sangre de su niño tiene algo que lo enferma y a veces ataca su cuerpo. Con qué frecuencia necesitará mi niño el tratamiento plasmaféresis? El doctor de su niño en Children s le indicará cuantas veces necesitará plasmaféresis. Puede mi niño tomar las medicinas usuales antes del tratamiento plasmaféresis? Menciónele al doctor todas las medicinas que su niño toma una hora o menos antes de la plasmaféresis o las que usted le da una vez al día. El doctor le puede pedir que espere hasta que finalice el procedimiento debido a que estas medicinas pueden permanecer en el plasma que será desechado. Cómo será el tratamiento de plasmaféresis? El proceso de plasmaféresis dura entre 2 y 4 horas. Su niño tendrá por lo menos 2 enfermeras: una del Puget Sound Blood Center (PSBC) y otra del Children s Hospital. La enfermera del hospital atenderá a su niño para se sienta cómodo y también para necesidad como comida o cambiarlo de posición. La enfermera de PSBC estará con su niño durante el procedimiento y se encargará de la máquina. Si lo desea puede acompañar a su niño. Antes de conectar a su niño a la máquina, los tubos deben contener una pequeña cantidad de sangre, el doctor de su niño puede decidir usar sangre de un donante sano para llenar los tubos si: Su niño pesa menos de 60 libras. Para su niño es un problema que le saquen un poco de sangre para llenar los tubos de la máquina. Durante el procedimiento su niño estará conectado a un monitor para observación. Sentirá algo mi niño durante la plasmaféresis? Su niño no debería sentir dolor pero puede sentirse incómodo ya que tiene que estar quieto. Es buena idea pensar en actividades tranquilas como ver una película o manualidades. A veces, durante la plasmaféresis, los niños se marean o sienten malestar estomacal debido a menos cantidad de sangre en el cuerpo. Si su médico lo aprueba, su niño puede comer y beber. 5 Patient and Family Education Plasma Exchange What is plasma? Plasma (plaz-mah) is the liquid part of the blood. It carries all of the red and white blood cells through the body. It also delivers vitamins and other things the body needs. Plasma does not just carry good things. It can also carry things that make your child sick. What is plasma exchange? It is a procedure that takes a small part of your child s blood out of the body through a catheter, a plastic tube that goes under your child s skin and into their blood vessels. The catheter has 2 tubes one for taking blood out of the body, and one for returning blood to the body. After the blood leaves your child s body it moves through the tubes of a machine called a blood cell separator. The blood cell separator removes the plasma from your child s blood cells. The things that make your child sick stay in the plasma and are thrown away. Your child s blood cells, along with some IV (intravenous) liquid, are sent back to your child s body through the catheter. The IV liquid keeps your child from becoming dehydrated (loss of too much water or fluids). The machine will only take out a small part of your child s blood at a time. It will keep going until all of the blood has been separated and sent back to your child s body. 6 Plasma Exchange What kind of IV liquid is put into my child? The most common liquids sent back to your child s body during plasma exchange are plasma or albumin. Your child s doctor will pick which of these new liquids is best for them: Plasma This has been donated by a healthy person who wants to help people who are sick. It is tested to make sure it is safe for your child. Albumin This is the same kind of protein that is found in everyone s blood. Why does my child need plasma exchange? Your child has things in the blood that make them sick. Sometimes these things attack your child s body. Other times they make your child s body create too much protein. Too much protein makes your child s blood move slowly, just like maple syrup moves slower than water. How often will my child need plasma exchange? Your child s doctor at Children s will tell you how many times they need plasma exchange. Can my child take their usual medicines before plasma exchange? Tell your child s doctor about any medicines that they would usually take an hour or less before plasma exchange or medicines that you give your child once a day. Your child s doctor may ask you to wait until after the procedure to give these medicines. This is because the medicines may stay in the plasma that is thrown away. What can I expect during plasma exchange? Plasma exchange lasts about 2 to 4 hours. Your child will have at least 2 nurses helping during plasma exchange: a Puget Sound Blood Center (PSBC) Nurse and a Children s hospital nurse. Your child s hospital nurse will help keep them comfortable. The hospital nurse will also be able to help with any needs like bringing food or changing your child s position. Your child s PSBC nurse will run the blood cell machine and stay with your child during the procedure. You are welcome to stay with your child. Before your child is connected to the machine, the tubing is filled with a small amount of blood. Your child s doctor may decide to use blood donated by a healthy person to fill the machine s tubing if: Your child weighs less than 60 pounds. Your child may not tolerate the removal of a small part of their blood to fill the tubing. 2 of 3
7 Plasma Exchange To Learn More Ask your child s healthcare provider Free Interpreter Services In the hospital, ask your child s nurse. From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. For Deaf and hard of hearing callers (TTY). During the procedure your child will be connected to a monitor to watch how their body does. The PSBC nurse will check your child s heart, breathing, temperature and blood pressure often. Will my child be given any special medicine before or during plasma exchange? Before plasma exchange your child may be given a medicine like Benadryl or Tylenol to keep your child from getting a temperature or rash, which happens sometimes. During plasma exchange your child will be given a medicine through the machine called an anticoagulant. This keeps the blood from sticking together inside the machine. The anticoagulant can cause the amount of calcium in your child s blood to become low. The PSBC nurse will test the blood often and give your child calcium to prevent the levels of calcium from becoming too low. Will my child feel anything during plasma exchange? Your child should not feel pain but may be uncomfortable sitting still. It is a good idea to plan calm things to do like watching a movie or working on an art project. Sometimes children will feel dizzy or sick to their stomach because there may be a little less blood in their body during plasma exchange. As long as it is OK with your doctor, your child may eat and drink during plasma exchange. Meals may have to wait until after the plasma exchange is done if space is limited or if your child cannot sit up during the treatment. Your child s nurse will provide a light blanket to prevent chills. If your child has a special blanket, you may bring it for the treatment. If the amount of calcium in your child s blood becomes low, your child may have one or more of these signs: Sour taste in the mouth Tingling feeling around the lips Sharp pain in the fingers or toes Please let your PSBC nurse know if your child has any of these signs.
