This directory is for all the municipalities in Puerto Rico, including Vieques and Culebra. This directory is current as of August Some network providers and pharmacies may not be listed in this directory. Some network providers and pharmacies may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about MMM s network providers and pharmacies in your area, you can visit or call our Member Services Department at (Metro Area), (toll free) or TTY (hearing impaired), Monday through Sunday, from 8:00 a.m. to 8:00 p.m. Beneficiaries must use network pharmacies to access their prescription drug benefit. 12 Section 1 Introduction Providers This directory provides a list of MMM s network providers. To get detailed information about your healthcare coverage, please see your Evidence of Coverage. You will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician (PCP). The term PCP will be used throughout this directory. Generally, you must get your healthcare services from your PCP. A PCP is a physician with a specialty in internal, family, general practice or geriatric medicine to whom you will have direct access without a referral and who has a direct or indirect contractual arrangement with the plan. A PCP provides you with initial and primary care services, maintains the continuity of your medical care and initiates and manages referrals to other network providers as required by your medical needs or as established in your coverage. The network providers listed in this directory have agreed to provide you with your healthcare services. You may go to any of our network providers listed in this directory; however, some services may require a referral or preauthorization. If you have been visiting one network provider, you are not required to continue visiting that same provider. In some cases, you may get covered services from non-network providers. To receive services from a non-plan provider you will require prior authorization from the plan. What if I use non-plan providers to obtain my covered services? Non-plan providers are providers that are not part of MMM. If you use non-plan providers, you may have to pay more. Care or services you receive from non-plan providers will not be covered, neither Medicare nor MMM will be responsible for the costs, except for ambulance services, emergency care, including hospital care after you are stable (post stabilization), maintenance care, urgently needed care, renal dialysis (kidney), and any services which were ordered or covered through an appeals process or if you have a coverage with Point of Service (POS) option. Please note that under the MMM Excel coverage you have a Point-of-Service (POS) option. The POS option allows you to get care from non-plan providers and hospitals when you are outside the service area. You may receive all plan covered services, but you will need to pay higher cost sharing than the ones you pay when using network providers. This is not the same benefit as visiting out of network physicians and/or hospitals during an emergency or an urgent care situation. Please refer to Chapter 3 and Chapter 4 of your Evidence of Coverage for more information about your POS cost sharing, cap on the dollar amount and prior authorization requirements. What should I do if I receive a bill from a non-plan provider that I think should be paid by MMM? We cover certain healthcare services that you receive from non-plan providers. These include care for a medical emergency, urgently needed care, renal dialysis (kidney) that you receive when you are outside the plan s service area, healthcare services preauthorized by MMM and any services which were ordered or covered through an appeals process. If a non-plan provider requires you to pay for any covered service, please contact our Member Services Department at (Metro Area), (toll free) or

13 TTY (hearing impaired), Monday through Sunday, from 8:00 a.m. to 8:00 p.m. You may also write to us at the following address: MEDICARE Y MUCHO MÁS PO BOX SAN JUAN, PR You should never pay any non plan-provider more than what the plan is allowed by Medicare. The provider has a right to obtain their fees, but they do not have the right to obtain them from you. Ask the non-plan provider to bill us first. If you have already paid for the covered services, we will reimburse you for our share of the cost. If you receive a bill for the services, you can send the bill to us for payment. We will pay your non-plan provider for our share of the bill and will let you know if you need to pay any amount. Getting care when traveling or away from the plan s service area If you need care when you are outside the service area, your healthcare coverage is limited. The only services we cover when you are outside our service area are: care for a medical emergency, including maintenance care and hospital care after you are stable (known as post stabilization care), urgently needed care, renal dialysis (kidney), and care that MMM or a plan provider has approved in advance. If you have any questions regarding whether a service is covered when you travel, call our Member Services Department at (Metro Area), (toll free) or TTY (hearing impaired), Monday through Sunday, from 8:00 a.m. to 8:00 p.m. What should you do if you have a medical emergency or urgent need for care? In an emergency, you should get care immediately. You do not have to contact your PCP or get permission in an emergency. You can dial on your phone for immediate help, or go directly to the nearest emergency room, hospital, or urgent care center. The Evidence of Coverage indicates what steps to follow if you have a medical emergency or urgent need for care. You must use plan providers except in an emergency or during urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers, neither Medicare nor MMM will be responsible for the costs. What should you do if it is not a medical emergency? If you need to talk with your PCP or receive medical care when the PCP s office is closed, and it is not a medical emergency, call the PCP s telephone number located on your MMM membership card. You may also call the MMM nurse helpline, Haciendo Contacto, at or TTY (hearing impaired), 24 hours a day, 7 days a week, 365 days a year. These are toll free numbers. Healthcare professionals are available to help you decide what type of care is needed and provide information about available resources. See your Evidence of Coverage for more information about what to do if you have an urgent need for care. Keep in mind that if you have an urgent need for care while you are in the service area, we expect you to obtain this care from plan providers. In most cases, we will not pay for urgently needed care that you receive from a non-plan provider while you are in the plan s service area. (Our service area is listed below.) 13

14 You can obtain most services on your own, without the need for prior authorization As explained above, you will receive most of your routine or basic care from your PCP, and your PCP will coordinate the rest of the covered services you receive as a plan member. If you obtain services from any nonplan physician, hospital, or other healthcare provider without prior authorization (approval in advance), you may be required to pay for these services yourself. You can obtain most services on your own, without approval in advance from your PCP or MMM. These include: Routine women s healthcare, which includes breast exams, mammograms (breast X rays), Pap tests, and pelvic exams Flu shots and pneumonia vaccinations (as long as you obtain them from a plan provider) Emergency services, whether you receive these services from plan providers or non-plan providers Urgently needed care that you receive from non-plan providers when you are temporarily outside the plan s service area. Also, urgently needed care that you receive from non plan providers when you are in the service area but, because of unusual or extraordinary circumstances, the plan providers are temporarily unavailable or inaccessible. (Below we explain the plan s service area.) Renal dialysis services that you receive when you are temporarily outside the plan s service area. If possible, please let us know where you are going to be before you leave the service area so we can help arrange for you to have maintenance dialysis while outside the service area. 14

16 Pharmacies This booklet provides a list of MMM s network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and MMM s formulary. We call the pharmacies on this list our network pharmacies because we have made arrangements with them to provide prescription drugs to Plan members. In most cases, your prescriptions are covered under MMM only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will fill prescriptions at non-network pharmacies under certain circumstances as described in your Evidence of Coverage. All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. This means the pharmacies listed here may no longer be in our network, or there may be newer pharmacies in our network that are not listed. This list is current as of August For the most current list, please contact us. Our contact information appears on the front and back cover pages. Changes to our pharmacy network may occur during the benefit year. An updated Pharmacy Directory is located on our website at You may also call Member Services for updated provider information. You can identify the sections with the shade on the right border of the page as explained below: Black - this color refers to the Preferred Providers Network. This means that going to providers within this network represents that your cost share or copayment may be lower than with a provider from the General Network. White - this color refers to the General Providers and Pharmacies Network. Our General Providers and Pharmacies Network is organized by providers types including primary care physicians, facilities and hospitals and pharmacies where you can receive your plan services. By visiting providers within the General Network, your cost share or copayment may be higher than with a provider from the Preferred Network.
