1 Caring for 2 nd and 3 rd Degree Burns Pautas para el Cuidado de Quemaduras de Segundo y Tercer Grado Recuerde que nunca debe colocar hielo sobre una quemadura. Reduce el flujo sanguíneo y previene la cicatrización. Las Quemaduras de Segundo Grado están abiertas, tienen un aspecto brillante y húmedo, o tienen ampollas. Estas quemaduras son dolorosas y sensibles al tacto. Pueden ser tratadas en casa, la clínica, o el hospital. Con frecuencia, las quemaduras de segundo grado tardan de 1 a 3 semanas para cicatrizar. Las Quemaduras de Tercer Grado tienen una apariencia seca o correosa (como cuero), blanca, café, marrón o negra. Estas quemaduras no son sensibles al dolor. Con frecuencia, las quemaduras de tercer grado tardan más de 3 semanas en cicatrizar o requieren injertos de piel. Estas quemaduras sólo se tratan en casa si son muy pequeñas. Siga los siguientes tratamientos para el cuidado de las quemaduras marcados con una X. Quite el vendaje. No lo sumerja para quitarlo. Al quitar el vendaje en seco se remueve el tejido muerto y los residuos. Lave las quemaduras suavemente una vez al día con jabón Dial. Limpie el área para quitar la crema de antibióticos, las ampollas y la piel suelta. Enjuáguela bien. Es de esperar que la herida sangre un poco. Las quemaduras faciales deben lavarse dos veces al día. Saque la crema/ungüento antimicrobiano del recipiente con una gasa o una paleta para prevenir que el resto del recipiente se contamine. Aplique crema o ungüento antimicrobiano sobre la herida dos veces al día (aproximadamente con 10 a 12 horas de diferencia). Para las quemaduras faciales, aplique ungüento de antibiótico doble dos veces al día, y con más frecuencia si la cara se reseca. No necesita usar gasa en la cara. Para quemaduras en las orejas, aplique crema antimicrobiana dos veces al día. No deje que entre crema en el conducto auditivo, ya que puede acumularse y tapar los oídos. Coloque gasa en las orejas. Aplique gasa no-adherente tal como Cuticerin. Envuelva todas las quemaduras, excepto las de la cara y las orejas, con una gasa no elástica de rollo. Es posible que necesite más capas de gasa si la herida supura mucho. Deje de fumar. El fumar disminuye el flujo sanguíneo y el oxígeno que llega a las quemaduras nuevas y curadas. La quemadura se siente dura (rígida) o como si se estuviese estirando a medida que cicatriza. Ejercicios Haga ejercicios para mantener las articulaciones (coyunturas) móviles y para estirar la piel nueva. Dieta Beba bastante líquido (de 8 a 10 vasos de 8 onzas cada uno en 24 horas) para evitar la deshidratación. Consuma una dieta bien equilibrada, alta en carbohidratos y proteínas para ayudar a que la(s) herida(s) sane(n). Piel cicatrizada Use cremas hidratantes/humectantes para prevenir que se reseque y parta. La piel cicatrizada tendrá un color rojo opaco o rosado. Aplique cremas hidratantes, tales como Elta Lite, Aquaphor, Eucerin, o Nivea tanto como sea necesario para mantener la piel hidratada y suave. Evite usar cremas que contengan alcohol o agentes entumecedores.

4 Burn Care Guidelines 2nd & 3rd Degree Burns Remember, never use ice on a burn. It decreases blood flow and prevents healing. Second Degree Burns appear open, shiny and moist, or blistered. These burns are painful and sensitive to touch. These burns may be treated at home, in the clinic, or in the hospital. Second degree burns often take 1-3 weeks to heal. Third Degree Burns appear dry or leathery, white, brown, maroon or black. These burns are not sensitive to pain. Third degree burns often take greater than 3 weeks to heal or need skin grafting. These burns are treated at home only if they are quite small. Follow the burn care treatments below marked with an X. Remove the dressing. Do not soak to remove it. Dry dressing removal cleans away dead tissue and debris. Wash burns gently once a day with Dial soap. Wash off antibiotic cream, blisters, and loose skin. Rinse well. A small amount of bleeding may be expected. Facial burns should be washed twice daily. Remove antimicrobial cream/ointment from jar with a tongue blade or gauze to prevent contamination of the jar. Apply antimicrobial cream or ointment to the burn two times per day (about hours apart). For facial burns apply double antibiotic ointment twice a day, and more often if face becomes dry. No gauze is needed on the face. For ear burns apply antimicrobial cream twice a day. Do not get cream in the ear canal. It may build up and plug the ears. Apply gauze to ears. Apply non-stick gauze such as cuticerin Wrap all burns except the face and ears with non-stretch roller gauze. Extra layers of gauze may be needed if the wound is weepy. Stop Smoking. Smoking decreases blood flow and oxygen to new and healed burns. It slows down the healing process. How to Apply an Ace Bandage Apply ace bandages to feet and legs over burn dressings to prevent swelling or bleeding into the burn. See Health Facts for You #4406, Leg Ace Bandaging. How to Prevent Swelling Face or head burns rest head on 2 or more pillows. Hand or arm burns rest hand or arm on pillows above the level of the heart as much as you can. Foot and leg burns keep feet and legs up on pillows when sitting or in bed.
5 Watch for Signs and Symptoms of Infection Redness (about 1 inch in width) and swelling around the burn. Foul smelling drainage or pus from the wound. Flu-like symptoms (temperature greater than 100.4º by mouth for two readings 4 hours apart, chills, nausea, vomiting or muscle aches). Increasing burn pain. Pain Take pain pills 1 hour before dressing changes. Between dressing changes use Tylenol (acetaminophen or extra strength acetaminophen) to treat pain. You will have less pain as the burn heals. The burn feels stiff or like it is being pulled as it heals. Exercises Exercise to keep joints moving and to stretch new skin. Diet Drink plenty of fluids, 8-10 (8 oz.) glasses in 24 hours, to prevent dehydration. Eat a well balanced diet high in carbohydrates and protein to help the wound(s) heal. Healed Skin Use moisture creams to prevent drying and cracking. Healed skin appears dull red or pink. Apply moisture creams such as Elta lite, Aquaphor, Eucerin or Nivea as often as needed to keep the skin moist and soft. Avoid creams with alcohol or numbing agents.
