1 Preparación preoperatoria para portadores de S. aureus o SARM Qué es el S. aureus? El S. aureus es el nombre abreviado para Staphylococcus aureus. El S. aureus es un tipo de bacteria (germen) que vive en la piel y en la nariz de ciertas personas. Produce problemas en raras ocasiones. Todos los humanos portan bacteria en su piel, la nariz, y dentro del tracto intestinal. En la mayoría de los casos, no nos hace daño y con frecuencia es muy útil. Pero si la bacteria llega a una parte de nuestro cuerpo donde no corresponde, puede causar problemas y producir infecciones. Qué es el SARM? El SARM (MRSA por sus siglas en ingles) es el nombre abreviado para el Staphylococcus aureus resistente a la meticilina. Es un tipo de germen de S. aureus que puede ser muy difícil de tratar con medicamento cuando se produce una infección. Algunas personas son portadoras de este tipo de S. aureus en su cuerpo o en la nariz sin tener ningún problema o infección. Pero si el SARM llega a una parte del cuerpo donde no corresponde, puede producir problemas y crear una infección que es difícil de tratar. El SARM y las precauciones de aislamiento Si se descubre que usted es portador del SARM, será necesario seguir medidas especiales de seguridad. Esto previene la propagación del SARM a otros pacientes. Estas medidas de seguridad se llaman precauciones de aislamiento de contacto. Para prevenir la propagación del SARM, sus visitantes y los trabajadores médicos seguirán las indicaciones indicadas en una señal colocada en su puerta mientras usted esté en el hospital.
2 Los trabajadores médicos y los visitantes: Se lavarán las manos y usarán guantes y una bata de protección cuando entren en su cuarto. Colocarán los guantes usados en la basura y la bata de protección en una cesta. Entonces se lavarán las manos de nuevo antes de salir de su cuarto. A usted le pediremos que: Hable con su enfermera sobre lo que necesita hacer para prevenir la propagación de gérmenes. Se tape la boca y la nariz cuando estornude o tosa. Si usa un pañuelo de papel o sus manos, lávese las manos cuando acabe. Se lave las manos antes de salir de su cuarto. Se ponga un bata de hospital limpia (puede usar un segundo traje como una bata) antes de salir de su cuarto. Mantenga la bata puesta mientras esté fuera de su cuarto. Permanezca en su cuarto excepto cuando necesite ir a una prueba o procedimiento. Evite el contacto con otros pacientes. Si se descubre que usted es portador del SARM, el Hospital y las Clínicas de la Universidad de Wisconsin (UWHC) requiere que usted esté en aislamiento de contacto durante su estancia completa en el hospital. Usted estará en aislamiento de contacto cada vez que venga como paciente al hospital o las clínicas del UWHC. Para cambiar este procedimiento, se deberán obtener tres nuevas series de muestras nasales y de la piel con hisopos. Todas las muestras deben mostrar que usted ya no tiene el SARM. Por favor, hable con su proveedor médico para saber más sobre estos hisopos para descartar el SARM. El tener SARM en la nariz o en la piel no debería constituir un problema en su vida cotidiana. La mejor forma de prevenir la propagación del SARM en casa y en lugares públicos es lavándose las manos con frecuencia. También debería lavar con frecuencia aquellos objetos que toca mucho. Si le preocupa ser portador del SARM, pregunte a su proveedor médico qué puede hacer para deshacerse de él. Hay medicamentos que pueden ayudar a reducir los gérmenes del SARM en su piel. En algunas personas, los medicamentos se deshacen de los gérmenes del SARM en su cupero. Otras personas portan el germen del SARM en su cuerpo durante muchos años.
3 Qué se puede hacer para prevenir una infección en mi área quirúrgica si se descubre que soy portador del S. aureus o el SARM? Si se descubre que usted es portador del S. aureus o el SARM o es probable que porte esta bacteria debido a su historial médico, le pedirán que siga las siguientes indicaciones. Reducirán su riesgo de contraer una infección de S. aureus o SARM en el área quirúrgica. 1. Aplique un ungüento con antibiótico en la nariz dos veces al día durante 5 días antes de su cirugía. 2. Lávese con un jabón antibacterial especial llamado Hibiclens todos los días durante 5 días antes de su cirugía.* El día antes de la cirugía, lávese con Hibiclens por la noche. También lávese con Hibiclens la mañana de la cirugía antes de salir de casa. 3. Le administrarán un antibiótico en el hospital que funciona contra el S. aureus o el SARM antes de la cirugía. 4 Hospital y Clínicas de la Universidad de Wisconsin Preparación de la piel antes de la cirugía S. aureus/sarm Nombre Fecha de la cirugía Cirugía Instrucciones 5 días antes de la cirugía Complete estos pasos diariamente durante 5 días hasta el día antes de la cirugía: 4. Lávese el cabello y la cara como lo hace normalmente. Enjuáguese el cabello y el cuerpo. Lávese cuidadosamente con Hibiclens durante 3 minutos de la barbilla a los dedos del pie. Enjuáguese bien. La mañana de la cirugía Lávese como se indica en el paso 1 arriba. Séquese la piel con una toalla limpia. No aplique lociones, cremas hidratantes, ni maquillaje. No aplique mupirocin el día de la cirugía. *Hibiclens debe usarse sólo en la piel. No lo use si tiene alergia al gluconato de clorexidina (CHG por sus siglas en inglés). No lo use en el área genital o en el recto. No lo use en la cabeza o la cara. No lo use en heridas profundas. El gluconato de clorexidina puede producir irritación de la piel, tal como picor o enrojecimiento. Esto ocurre con más frecuencia cuando se aplica sobre la piel sensible justo después de tomar una ducha o rasurarse. Si el picor o el enrojecimiento persisten, enjuague las áreas afectadas. Deje de usar Hibiclens. Contacte a la consulta de su cirujano.
5 Preoperative Preparation for S. aureus or MRSA Carriers What is S. aureus? S. aureus is the short name for Staphylococcus aureus. S. aureus is a type of bacteria (germ) that lives on the skin and in the nose of some people. It rarely causes any problems. All humans carry bacteria on our skin, in our nose, and inside our digestive tract. In most cases, they do not harm us and are often very helpful. But if bacteria get into a part of our body where they don t belong, they can cause problems and lead to infections. What is MRSA? MRSA is the short name for Methicillin Resistant Staphylococcus aureus. It is a type of S. aureus germ that can be very hard to treat with medicine when an infection occurs. Some people carry this type of S. aureus on their bodies or in their noses without any problems or infections. But if MRSA gets into a part of the body where it doesn t belong, it can cause problems and lead to an infection that is hard to treat. MRSA and Isolation Precautions If you are found to carry MRSA, special safety measures will need to be followed. This prevents the spread of MRSA to other patients. These safety measures are called contact isolation precautions. To prevent the spread of MRSA, your visitors and health care workers will follow directions outlined in a sign placed on your door while you are in the hospital. Health care workers and visitors will Clean hands and wear gloves and gown when going into your room. Put used gloves in trash and gowns in hamper. Then clean hands again before leaving your room.
6 You will be asked to Talk with your nurse about what you need to do to prevent the spread of germs. Cover your mouth and nose when sneezing or coughing. If you use a tissue or your hands, clean your hands when you are done. Clean your hands before leaving your room. Put on a clean robe (may use a second gown worn like a robe) before leaving your room. Keep the robe on while outside your room. Stay in your room except when you need to go for a test or procedure. Avoid contact with other patients. If you are found to carry MRSA, University of Wisconsin Hospital and Clinics (UWHC) requires you to be in contact isolation for your entire hospital stay. You will be in contact isolation every time you are a patient in a UWHC hospital or clinic at the hospital. To change this procedure, you will need to get three more sets of nasal and skin swabs. All swabs must show that you do not have MRSA anymore. Please talk with your health care provider to learn more about these MRSA clearance swabs. Having MRSA in your nose or on your skin should not be a problem in your daily life. The best way to prevent the spread of MRSA at home and in public is to wash your hands often. You should also wash objects often that you touch a lot. If you are concerned about carrying MRSA, ask your health care provider what can be done to help get rid of it. There are medicines that may help reduce MRSA germs on your skin. For some people, medicine gets rid of the MRSA germs from their bodies. Other people carry MRSA on their bodies for many years. What can be done to prevent an infection at my surgical site if I am found to carry S. aureus or MRSA? If you are found to carry S. aureus or MRSA or are likely to carry these bacteria due to your health history, you will be asked to follow the directions below. They will reduce your risk of a S. aureus or MRSA infection at your surgical site. 1. Apply an antibiotic ointment to your nose twice per day for the 5 days before your surgery. 2. Wash with a special antibacterial soap called Hibiclens daily for the 5 days before your surgery.* On the day before surgery, wash with Hibiclens in the evening. Also wash with Hibiclens on the morning of your surgery before you leave home. 3. An antibiotic will be given to you in the hospital before your surgery that works against S. aureus or MRSA. Special preoperative directions can be found on the next page. You must follow these. Contact your doctor if you have questions.
7 University of Wisconsin Hospital and Clinics Preparing Your Skin before Surgery S. aureus/mrsa Name Surgery Date Surgery Directions 5 Days before Surgery Complete these steps daily from the 5 days before surgery until the day before surgery: 1. Wash hair and face as usual. Rinse hair and body. Wash gently for 3 minutes with Hibiclens from chin to toe. Rinse thoroughly. Keep Hibiclens out of eyes, ears, and mouth. Do not use on the head, face, genitals, or rectal area. 2. Apply a generous amount of mupirocin with cotton swab to each nostril two times per day. Do this after washing in the morning and at bedtime. Shaving at the surgical site should be stopped at least 2 days before surgery. The Night before Surgery Wash as described in step 1 above. Dry skin with a clean towel. Do not apply lotions, moisturizers, or makeup. Apply mupirocin a second time as described in step 2 above. The Morning of Surgery Wash as described in step 1 above. Dry skin with a clean towel. Do not apply lotions, moisturizers, or makeup. Do not apply mupirocin on the day of surgery. *Hibiclens is for skin use only. Do not use if you are allergic to chlorhexidine gluconate (CHG). Do not use in the genital or rectal areas. Do not use on the head or face. Do not use on deep wounds. CHG may cause skin irritation such as itching or redness. This is more likely when it is applied to sensitive skin right after taking a shower or shaving. If itching or redness persists, rinse the affected areas. Stop using the Hibiclens. Contact your surgeon s office.
