Asthma is a disease in which the air passages of the lungs are inflamed. This causes the air passages to over-react to a variety of things. The muscles around the air passages tighten. Both the tightening and the inflammation cause the air passages to get narrower for a time. This can make it hard to breathe. Also, thick mucus is created in the air passages. This blocks them even more. Asthma is called "mild persistent" if your child needs to use a rescue inhaler more than twice a week, but not every day. It is also considered "persistent if your child wakes up at night with wheezing or coughing twice a month or more, (or if under 4 years old once per month or more). What is the cause? The tendency to have asthma is often inherited. It may not start until there is another problem such as infection in the lungs. The inflammation of asthma causes your child's lungs to be more sensitive to certain irritations. We call these "triggers." They can cause an asthma attack. Triggers may include year-round or seasonal allergens. Allergens are anything you are allergic to. Year-round triggers include dust mites, pets, cockroaches, and molds. Seasonal triggers include grass pollens, weed pollens, and tree pollens. Cigarette smoke is a major trigger for most people. Other triggers may include exercise, laughing, breathing cold air, and air pollution. Catching a cold is a common asthma trigger. Even emotional upset can make asthma worse. Foods rarely cause just asthma symptoms. What are the symptoms? Your child may have a tight feeling in the chest, shortness of breath, or fast breathing. Your child may wheeze. This is a raspy or whistling sound made when breathing. Sometimes your child may only have a cough without any obvious breathing difficulty. Your child may occasionally wake up at night or in the morning with a cough or wheeze. How is it treated? Treatment goal: is to minimize asthma symptoms and long term damage through reduction of exposure to specific causes, appropriate use of medication and prompt response to increasing symptoms, while avoiding side effects from the treatment itself. Parents must understand asthma to help their children do well. You can learn to know when your child is getting worse. With our help you can learn how to use your child's asthma medicines. Ask your child's provider to give you a written "Asthma Plan" and keep it handy to help you to use these medicines. Most important, you can learn how to help your child avoid the things that make asthma worse. For instance, your child may need to avoid pollen, dust, or animal dander. Animal dander comes from animal skin, fur, or feathers. Cigarette and other smoke, perfume, and paint fumes must be avoided. Do not allow any smoking in your home, your car, or around your child. In summer, watch the local weather report for air quality warnings. Your child may need to stay indoors on days when air pollution is bad or pollen counts are high. The main problem with asthma is inflammation of the airways. Because of this, we may need to give your child a daily medicine to treat and prevent more inflammation, we call this control medicine. We may only give your child medicine to treat episodes of cough and wheezing. We call this rescue medicine since it is only used when your child has symptoms. Your child may need to use this medicine before exercise or other known triggers. It can help to

2 prevent an attack. We want your child to be able to play, exercise, and attend school like any other child. Make sure you know the difference between the rescue medicine and daily or control medicine. Ask us if you aren t sure. We may also teach your child to use a spacer. This is a device that makes it easier to use inhalers and allow more effective delivery of the medicines. Sometimes younger children with asthma need to be given medicine using a nebulizer machine at home. This allows you to give an inhaled medicine to a child who is too young or too ill to use an inhaler well. Be sure that your child's school, teachers, and daycare workers know about your child's condition and what to watch for. We may recommend an annual flu shot for your child and your family. The flu can bring on a severe asthma episode. By getting flu shots every year, we may prevent this from happening. What can I expect? Children with mild asthma will lead a normal life with good care by you and by us. It is important that we work together as a team. With good consistent treatment, we may be able to decrease the strength or amounts of medicine that your child needs. This will take time. Your child may have asthma symptoms a few times a week but not daily. Your child's worst attacks may happen after catching a cold. Learn to monitor your child's asthma carefully if there is a runny nose, sore throat or cough. Asthma often is a life-long disease. Your child's asthma may get better or worse over time. To treat children properly, you need to monitor their asthma and we need to see them regularly. We may need to change their medicines from time to time. We may do breathing tests to help measure your child's asthma. This may help guide treatment. Many children seem to grow out of asthma. Asthma returns in about a third of these children when they are adults or if they start smoking. What to watch for. Seek immediate medical help if your child has extreme shortness of breath that does not improve right away with the use of an inhaler or nebulizer. Call us if your child has severe cough, wheeze, or is breathing fast. Let us know if your child wakes at night with cough or wheeze. We want to know if the medicines do not control your child's asthma. Let us know if your child's asthma symptoms get more frequent or severe. Qué es? Con nuestra ayuda, puede aprender a adaptar los medicamentos. Lo que es más importante, usted puede aprender cómo ayudar a su hijo a evitar las cosas que le empeoran el asma. Por ejemplo, su hijo necesitará evitar el polen, el polvo y la caspa de los animales. La caspa de los animales proviene de la piel, el pelo o las plumas de los animales. 4 Medications: Beta Adrenergics:: Ventolin HFA 108 (90 Base) MCG/ACT Inhalation Aerosol Solution Inhale 2 to 4 puffs every 4-6 hours as needed for asthma symptoms Qty: 1 GM Albuterol Sulfate (2.5 MG/3ML) 0.083% Inhalation Nebulization Solution Inhale 1 unit dose 3-4 times daily by nebulization over 5 minutes as needed. Qty: 1 ML Elixophyllin 80 MG/15ML Oral Elixir TAKE 1 TEASPOONFUL 4 TIMES DAILY. Qty: 60 ML Proventil HFA 108 (90 Base) MCG/ACT Inhalation Aerosol Solution Inhale 1 puff 4 times daily. Qty: 1 GM Maxair Autohaler 200 MCG/INH Inhalation Aerosol Breath Activated INHALE 2 PUFFS EVERY 4-6 HOURS AS NEEDED. Qty: 1 GM Xopenex 0.31 MG/3ML Inhalation Nebulization Solution Inhale 1 unit dose 3 times daily, 6-8 hours apart, by nebulization. Qty: 1 ML Steroid Inhalants:: Asmanex 120 Metered Doses 220 MCG/INH Inhalation Aerosol Powder Breath Activated USE 1 UNIT DOSE DAILY. Qty: 1 GM Flovent HFA 44 MCG/ACT Inhalation Aerosol Inhale 2 puffs 2 times daily. Qty: 1 GM Pulmicort 0.25 MG/2ML Inhalation Suspension Inhale 1 respule via nebulizer 1-2 times daily as directed. Qty: 1 ML Qvar 40 MCG/ACT Inhalation Aerosol Solution Inhale 2 puffs 4 times daily. Qty: 1 GM Leukotriene Receptor Antagonists:: Accolate 10 MG Oral Tablet Take 1 twice daily. Use your peak flow meter twice a day or whenever you feel your breathing is getting worse. A spacer can be used with your metered dose inhaler medication. We recommend a flexible bronchoscopy. Your child has difficulty breathing. Your child's cry is quieter and shorter. Your child is short of breath. Your baby is too short of breath to suck or feed. Your child is using the rescue inhaler more than 2 times a day. The medicine effects seem to be wearing off too soon. Peak flow is still less than 200 after 2 days Peak flow is in the yellow zone for more than 2 days. The symptoms seem worse. Wheezing causes night waking more than once a week.
