3 A Guide to HIV & AIDS Services in North Texas September 2014 The 2014 HIV services handbook is designed as a guide to help find information about organizations and services for people living with HIV and AIDS in Dallas County and surrounding areas. It is not a directory of every service available and inclusion in this directory does not constitute an endorsement by the Dallas Area Ryan White Planning Council or Parkland Health & Hospital System, HIV Department. The scope of this 16th edition of the handbook is primarily agencies providing services to residents of the Dallas eligible metropolitan area and health service delivery area. Agencies providing services in other areas of Texas and surrounding counties have been included as a courtesy for all users of this informational handbook if they furnished information for publication. We have relied on the agencies listed to supply us with current and accurate information about their programs and services. If an agency did not respond to our request for information, it may not be included in the handbook. The handbook is designed to provide a starting point for accessing services in the Dallas area. In many cases it is only a partial list of the kinds of programs, services, and agencies available to someone living with HIV or AIDS in the Dallas area. We have made every effort to be accurate, and have included Spanish translations wherever possible. Risk Reduction Message The Human Immunodeficiency Virus (HIV) is not spread easily. You can only get HIV if you get infected blood or sexual fluids into your system. You can t get it from mosquito bites, coughing or sneezing, sharing household items, or swimming in the same pool as someone with HIV. Some people talk about shared body fluids being risky for HIV, but no documented cases of HIV have been caused by sweat, saliva or tears. To decrease the risk of spreading HIV: Use condoms during sexual activity Quit using injection drugs or do not share drug injection equipment If you are HIV-infected and pregnant, talk with your health care provider about taking HIV medications If you are an HIV-infected woman, don t breastfeed any baby Protect cuts, open sores, and your eyes and mouth from contact with blood If you think you have been exposed to HIV, get tested. 6 Section I-a: Tips and Instructions Each agency and/or service will have specific requirements in order for you to qualify for assistance. Different requirements might include things like your health status, which county you live in, or how much money you make. Read through the Getting Started section of this guide to get a better understanding of the help that is available and how to access services. Call the agency and ask what you ll need to bring when you go for your first appointment or intake. Case managers, are people who work with you to help you find the services you need. Not all agencies have case managers. Once you begin working with a case manager he/she may be the only case manager to assist you with referrals to other services and for general assistance in accessing HIV/AIDS services in the Dallas County and surrounding areas. When choosing the agency that will handle your case management, please take into consideration that you may be required to visit your case manager in person so you will want to ensure that the agency s location is easy for you to access. Be aware that information changes frequently. For 2014, several agencies either added or stopped offering services previously available. Always call the agency first to verify information. 8 Section II-a: Getting Started-Care Connections Resources & Support in North Texas Reasons to Enter Primary Medical Care Receiving an HIV diagnosis is often an overwhelming and frightening experience. Where do you go? Who do you call? What should you do next? There are choices to consider at every stage of the disease. There should be someone who understands how all parts of your care fit together in keeping you whole. That person is usually your primary care provider (PCP). PCPs are responsible for providing health maintenance care and coordinating care. The PCP may be a doctor (MD or DO), nurse practitioner (NP), or physician s assistant (PA). It s best if you and your health care provider work together as a team. That makes it easier to choose and stick to your treatment plan. Care provider means a doctor, a physician s assistant, or a nurse practitioner. Reasons to Remain in Primary Medical Care HIV medical and dental care is very complicated and changes quickly. This makes it important to find an HIV care provider who works with HIV/AIDS patients. Your relationship with an HIV provider will be better if you are comfortable with each other s personal style and approach to dealing with health issues in general, and HIV in particular. We can help. The North Texas Care Coordination Team is a group of nonprofit organizations committed to helping you connect to a comprehensive system of health care and resources. Whether you are newly diagnosed or have lived with the disease for many years, we are here to support your efforts to manage the challenges of living with HIV and AIDS. Who Do I Call First? To connect to the HIV/AIDS care system, simply call one of the following organizations and ask for an intake session. If you are a veteran, contact the Veteran s Affairs office listed within this handbook. 10 What is an Intake Session? Your intake session is your first introduction to what is available in the community to help you take care of your health and begin to plan for the future. Intake sessions are scheduled within one to two weeks of your call, although appointments for urgent situations can be scheduled sooner. Every effort will be made to locate an intake site convenient to you. Home visits are available from most intake sites when illness or transportation problems exist. There are many services available that, based on certain eligibility criteria such as income, could be free of charge to you. These services are funded by the Ryan White HIV/AIDS Treatment Extension Act of 2009 and other funding sources. They include medical and dental care, case management, medication assistance, insurance assistance, HIV education, transportation, mental health counseling, food, legal services, child care, housing, substance abuse treatment and home health services. Completing the intake process is the first and most important step towards accessing these services and finding out if and how ongoing case management can continue to help you. All HIV+ individuals who seek care, support and resources funded by the Ryan White HIV/AIDS Treatment Extension Act must complete an intake session. Intake sessions involve providing important information about yourself so needs/eligibility assessments may be determined. There will be forms to fill out, information and documents to provide to an intake specialist, and an in-depth discussion about how help can be provided to deal effectively with HIV/AIDS now and in the future. What Information Do I Bring to the Intake Session to Establish Services? The information you must bring to your intake session includes: Proof of HIV and/ or AIDS diagnosis (one of the following): A lab report of detectable HIV viral load that includes the name of the client and testing facility; A signed statement from a physician, physician s assistant, an advanced practice nurse or a registered nurse (RN) attesting to the HIV positive status of the person; A hospital discharge summary that documents HIV positive status and includes name of the client; A confirmatory HIV+ laboratory result (Western Blot, IFA, NAAT, Multispot HIV-1/HIV-2 Rapid Test (Bio-Rad) or a detectable HIV RNA) that includes the name of the client and testing facility. Proof of Dallas area or Sherman/Denison area residency (one of the following): A valid Texas driver s license or Texas state identification card, with an address within a specified service delivery area; A current Voter Registration Card; Mortgage or lease agreement with a Texas address within a specified service delivery area, as it relates to the standard Texas Apartment Association (TAA) lease. The first and last page of an executed lease is acceptable, provided all required information is included within these two pages. As for non-taa leases, the lease must be reviewed thoroughly to determine if the first and last pages have sufficient information to satisfy eligibility requirements; 8

11 A receipt or documentation from the landlord, dated within the last 30 days, indicating residence or month to month lease; One household bill with the client s name and address (including residential or cell phone bill) delivered within the last 30 days; A signed statement, dated within the last 30 days, from the client indicating homelessness or non-traditional habitation outside the boundaries of a physical address, institution or homeless shelter; Release paperwork from a correctional facility documenting a local address within the Dallas and/or Sherman/Denison area; One article of personal mail with the client s name and address postmarked within the last 30 days; One article of business or bulk mail with the client s name and address, delivered in the last 30 days; a postmarked envelope is not required if the correspondence includes the business name, date sent and the clients name and address; Paystubs dated within the last 30 days; A signed statement from a person in the household, dated within the last 30 days verifying that the client lives at a specific address. Proof of Income (one of the following): Award letter (including but not limited to: SSI, RSDI, VA and pension) granting benefits for the current calendar year; Payroll check stubs to verify last 30 days of income; Most recent bank statement that shows deposit and source within last 30 days; this may include electronic bank statements obtained online from the banking institution; No Income Certification form signed by the client within 30 days of intake; Financial support within the last 30 days including cash payment and assistance from family must be documented with the benefactor, verbally or in writing; Letter or verbal communication within the last 30 days from an employer verifying frequency of payment and amount of wages or salary; Child support statements; Alimony statements; Signed statement from the client indicating that they receive cash payments for labor performed; Most recent W-2 form or U.S. tax return. Picture ID Proof of insurance, if available (Medicaid, Medicare, private) Mother s maiden name Medical history such as significant illnesses, hospitalizations, current medications and medical provider, and emergency contact information There are times when individuals do not have all of the information listed. If this is your situation, call and talk to an intake specialist. They often can offer helpful advice on how to collect needed information or offer alternatives not listed in this brochure. 9

12 Once you arrive for your intake session, the intake specialist will fill out a Common Intake Form (CIF) and several other forms that ask many questions. These questions are not meant to intrude on your privacy! They are meant to provide critical information on how best to serve your needs and ensure you understand your diagnosis and the HIV care system. It is good to know that many service organizations require additional paperwork from you beyond the Common Intake Form when you arrive for your first appointment, such as consent to receive services forms. Additionally, eligibility recertification will occur every six months after the initial intake session. The Benefits of Case Management Your intake specialist will discuss the benefits of case management and how it can assist your long-term efforts to manage your health, offer support as you and your loved ones adjust to your diagnosis, and help link you to future services you may need, such as legal and counseling services, Social Security, Medicaid, Medicare, Medicare Part D and many other community resources. Case managers are specially trained to respond to the needs of HIV+ people and help plan for long term financial, emotional and physical needs. They can be your advocate and guide to accessing important services, support, education and health care. Case management is free to any HIV+ person regardless of income. What Happens Next? Once the intake specialist completes the required paperwork and assessment for eligibility and needs, he or she will provide you with immediate referrals to needed medical care and supportive services. They will inform you on how to make an appointment and what additional paperwork you may need that is unique to each service organization. They will also provide you a referral form for all referred services you need at that time and educate you on how case management can help you access additional services you may need in the future. Although intake specialists can provide initial referrals at the time of your intake, only a case manager can provide referrals after the intake process is completed. It is therefore very important to consider case management as a potential service you may need in the future. Once your intake is complete and you receive your intake packet and referral form, you simply call the referred organizations for a first appointment. Be sure to ask if there is additional information you need to bring beyond the intake packet provided to you. Your intake specialist can help you make these appointments if you choose. How Long Does an Intake Session Take? You should make plans to spend from two to four hours at your intake session to allow the intake specialist the time needed to complete required paperwork, explain the HIV care system, offer information about HIV, the importance of ongoing health care and provide referrals to needed care and resources. Can I Bring Someone With Me? Absolutely! It is important that your loved ones be involved in your care and support. We welcome them at the intake session. We can help them better understand HIV and become a stronger support system for you. There may also be services we can offer them as affected family members such as counseling or child care for your children. In addition, we have access to many other non- 10

13 HIV related services in the community that may be very helpful for caregivers, partners and family members. What Happens to My Information? Information provided at the intake session is collected and entered into a Texas Department of State Health Services database called ARIES. ARIES information is secure and confidential in order to protect your privacy. It is used to track the HIV epidemic, the needs of the people impacted by the disease and the services utilized by them. All Ryan White funded organizations are required to enter this information and maintain strict confidentiality of the information provided to us. Only you can provide permission for us to share that information with others. You will be provided a copy of all forms completed during intake. It is important that you take this packet with you to your first appointment at other referral service organizations. We will also, with your permission, share needed information with these referral service organizations to help expedite the referral process so you can begin needed services as quickly as possible. How do I Connect to Case Management? All intake sites are also case management sites, so it s easy to get linked to this service. The intake specialist will assess your needs and provide an opinion about your need for case management; however, case management is available to any HIV+ person upon request. Once you request the service, the intake specialist will help you choose the best site to meet your unique needs. You will be referred immediately and a case manager will call you for a first appointment. They will provide additional assessment and begin a comprehensive care plan that focuses on reaching optimal quality within your medical, financial and emotional life. Ongoing referrals will be provided to ensure you remain linked to medical care. HIV education, treatment adherence planning, home visits, family/partner education and testing, and support groups are just some of the additional support available to you in addition to the wide range of services throughout the North Texas area. Home visits are available at several of the case management sites and bilingual services are available at most for Spanish, French and American Sign Language. You should ask about the availability and the criteria for case management home visits when you contact one of the listed sites. What if I Don t Want Case Management? Not a problem. Not everyone needs ongoing case management and some people only need intermittent assistance. If you do not want or need future referrals beyond the intake session, you are not required to accept case management. If you should need future referrals, you will require a case manager to assist you. Simply call your original intake site and let them know your needs. They will link you with a case manager as soon as possible in an area that s most convenient to you. We are here to help! If you are HIV+ and need assistance, please call us. 14 How Do I Get Housing Assistance? Housing Opportunities for People with AIDS (HOPWA) is grant funding for the provision of rental, mortgage and utility payments. HOPWA ensures that individuals and families affected by HIV and AIDS do not fall into homelessness. The HOPWA program offers two types of housing assistance, which is based on the needs of the individual or family. Short-term assistance is time-limited assistance designed to prevent homelessness and increase housing stability. Tenant-based rental assistance enables individuals to pay rent and utilities until they are able to secure more affordable and stable housing. Tenant-based assistance is not time-limited. HOPWA and housing search assistance is provided by Homebase for Housing, offering one-on-one help to individuals with HIV/AIDS; assistance is available for clients in jeopardy of losing housing with referrals for rent and utility payments. A resource room with access to phone and computers is available for clients. Homebase for Housing is listed under Legacy Counseling Center is a free, easy-to-remember phone number connecting callers with health and human services in their community. When you dial 2-1-1, you will talk to a trained resource specialist who has access to comprehensive data base listings of nearly all health and human services that exists in Texas. This includes but is not limited to: rent and utility assistance, food, emergency shelters, where to get employment help, medical and mental health assistance, help with transportation, and trained suicide intervention counseling. You can also get information on affordable child care, information about caring for an aging relative, or help recovering from a disaster. Information and referral is available 24 hours a day, 7 days a week throughout the year. Information can be provided in over 90 different languages. 12

15 Sección II-b: Para Comenzar Contactos de Asistencia Recursos y Apoyo en el Norte de Texas Razones para Inscribirse en la Asistencia Médica Primaria Recibir un diagnóstico de VIH es, con frecuencia, una experiencia abrumante y aterradora. A dónde irá? A quién llamará? Qué debe hacer ahora? Estas son opciones a considerar en cada una de las etapas de la enfermedad. Debería haber alguien que comprenda cómo se conjugan todas las partes de su asistencia médica para mantenerlo sano. Esa persona es, por lo general, su médico de cabecera (primary care provider o PCP, por sus siglas en inglés). Los médicos de cabecera son responsables por proporcionarle asistencia para el mantenimiento de la salud y por coordinar la asistencia correspondiente. Es mejor si usted y su médico de cabecera trabajan juntos como equipo. Eso facilita la selección y cumplimiento con su plan de tratamiento. El término proveedor de asistencia o médico de cabecera se refiere a un doctor, médico asistente o enfermero especializado. Razones para Permanecer bajo la Asistencia Médica Primaria La asistencia médica y dental para el VIH es muy complicada y cambia rápidamente. Esto hace que sea importante encontrar un proveedor médico especializado en VIH que trabaje con pacientes que padezcan de VIH o SIDA. Su relación con su especialista en VIH será mejor si ambos se sienten cómodos con el estilo personal de cada cual y se concentran en tratar con los problemas médicos en general y el VIH en particular. 17 En qué consiste la Sesión Inicial? Su sesión inicial constituye su introducción a los servicios comunitarios disponibles para ayudarlo a cuidar de su salud y a hacer planes para el futuro. Las sesiones iniciales se programan una o dos semanas después de su llamada aunque las citas médicas para situaciones urgentes pueden programarse para una fecha más cercana. Existen muchos servicios disponibles que, basados en criterios de elegibilidad, tales como el ingreso, podrían ser gratis para usted. Estos servicios los patrocina la Ley de Extensión del Tratamiento del VIH o SIDA (conocida en inglés como Ryan White HIV/AIDS Treatment Extension Act of 2009 y demás fuentes de financiamiento, incluyendo la asistencia médica y dental, coordinación de casos (case management), asistencia para la obtención de medicamentos, asistencia para el seguro médico, información sobre el VIH, transporte, consejería para la salud mental, alimentos, servicios legales, servicios de guardería (child care), vivienda, tratamiento para la drogadicción y servicios médicos a domicilio. Completar el proceso inicial es el primer y más importante paso para tener acceso a estos servicios y descubrir si y cómo puede ayudarlo la coordinación continua de su caso. Todas las personas con VIH que buscan asistencia, apoyo y recursos patrocinados por la Ley de Extensión del Tratamiento del VIH o SIDA deben acudir a la visita inicial. Estas visitas incluyen la proporción de información importante sobre usted de tal manera que puedan realizarse evaluaciones a fin de determinar sus necesidades y elegibilidad. Habrá formularios que usted deberá llenar así como información y documentos que deberá proporcionarle al especialista y una conversación detallada sobre cómo podemos ayudarlo a hacerle frente con eficacia al VIH o SIDA ahora y en el futuro. Qué Información Debo Traer a la Sesión Inicial para Solicitar Servicios? Entre la información que debe traer a su sesión inicial se incluye: Prueba del diagnóstico de VIH o SIDA. (Presente uno de los siguientes): Informe del laboratorio de la carga viral detectable del VIH que incluya el nombre del cliente y el del laboratorio; Declaración firmada proporcionada por un médico, médico asistente, enfermero especializado avanzado o enfermero registrado (registered nurse o RN, por sus siglas en inglés), la cual de fe de que el resultado del análisis para detectar el VIH ha sido positivo para la persona; Un resumen del alta del hospital que documente el resultado del análisis para detectar el VIH e incluya el nombre del cliente; El resultado de un análisis de laboratorio que confirme que usted padece del VIH (tales como, Western Blot, IFA, NAAT, Multispot HIV-1/HIV-2 Rapid Test [Bio-Rad] o un análisis del ácido ribonucleico del VIH [HIV RNA]) que incluya el nombre del cliente y el del laboratorio. 15

18 Prueba de domicilio que demuestre que vive en el área de Dallas o Sherman/ Denison. (Presente uno de los siguientes): Licencia de conducir válida o tarjeta de identificación emitida por el estado de Texas que muestre su domicilio dentro del área especificada de servicio; Tarjeta actualizada de Registro para Votantes (Voter Registration Card); Contrato de compraventa (mortgage) o alquiler que muestre su domicilio en Texas, dentro del área específica de servicios, según se relacione con el contrato de alquiler regular de The Texas Apartment Association o TAA. La primera y última página del contrato de alquiler firmado se consideran como aceptables, siempre y cuando incluyan toda la información requerida en estas dos páginas. Para las personas con contrato de alquiler no relacionado con la TAA, deberá revisarse cuidadosamente el contrato para determinar si la primera y última páginas del mismo contienen suficiente información para satisfacer los requisitos de elegibilidad; Recibo o documentación emitida por el propietario de la vivienda (landlord), en los últimos 30 días, certificando que existe un contrato de alquiler mensual o a largo plazo; Cuenta de algún servicio que muestre el nombre y domicilio del cliente (incluyendo cuentas tales como la del pago de la casa o teléfono celular) con fecha de emisión en los últimos 30 días; Declaración firmada del cliente, con fecha de emisión en los últimos 30 días, indicando que se encuentra usted indigente (homeless) o que habita en una residencia no tradicional fuera de los límites de un domicilio físico, institución o albergue para indigentes; Documentos de liberación emitidos por una institución correccional con domicilio ubicado dentro del área de Dallas o Sherman/Denison; Correspondencia con el nombre y domicilio del cliente que muestre un matasellos con fecha de envío en los últimos 30 días; Correspondencia comercial o en serie (bulk) con el nombre y domicilio del cliente, emitida en los últimos 30 días. No se requiere el sobre con matasellos si la correspondencia incluye el nombre del negocio, fecha en que se envió la correspondencia así como el nombre y domicilio del cliente; Talones de cheque con fecha de emisión dentro de los últimos 30 días; Declaración firmada de alguna persona que habite en la vivienda, con fecha de emisión dentro de los últimos 30 días, la cual verifique que el cliente sí reside en un domicilio específico; Prueba de Ingreso. (Presente uno de los siguientes): Carta de notificación (incluyendo, aunque sin limitarse a ello, SSI, RSDI, VA y la pensión de jubilación), otorgándole beneficios para el presente año natural; Talones de cheque de nómina para verificar el ingreso correspondiente a los últimos 30 días; El estado de cuenta bancario más reciente que muestre un depósito y la fuente de dicho ingreso en los últimos 30 días. Esto puede incluir estados de cuenta electrónicos obtenidos en la Internet a través de la institución bancaria; Formulario para certificar la falta de ingresos del cliente ( No Income Certification form) firmado en un lapso de 30 días posterior a la sesión inicial; La asistencia financiera recibida en los últimos 30 días, incluyendo pagos y asistencia en efectivo brindada por sus familiares, a documentarse con el benefactor verbalmente o por escrito; 16

19 Carta o comunicación verbal, sostenida en los últimos 30 días, con la persona o empresa para la cual trabaja, verificando la frecuencia y cantidad de ingresos pagados por concepto de sueldo o salario; Declaraciones de pago del beneficio de manutención del menor (child support statements); Declaraciones de pago del beneficio de manutención del ex cónyuge (alimony statements); Declaración firmada por el cliente, indicando que recibe pagos en efectivo por trabajos realizados; Más reciente formulario W-2 o declaración del impuesto sobre la renta de los Estados Unidos (U.S. tax return). Tarjeta de Identificación con foto Prueba de seguro, si la hubiere (para beneficios de Medicaid, Medicare y seguro privado) Nombre de soltera de la madre Historial médico, incluyendo información sobre enfermedades significativas, hospitalizaciones, medicamentos que esté tomando actualmente y el nombre de su médico de cabecera así como información de contacto en casos de emergencia En ocasiones, las personas no cuentan con toda la información aquí mencionada. Si este es su caso, llame y hable con uno de los especialistas de la sesión inicial (intake specialist). Con frecuencia, ellos pueden ofrecerle consejo útil sobre cómo recaudar la información necesaria u ofrecer alternativas no mencionadas en este folleto. Una vez que haya usted llegado a su sesión inicial, el especialista llenará un formulario conocido como Common Intake Form o CIF y varios formularios, los cuales podrían contener muchas preguntas. Tales preguntas no pretenden invadir su privacidad! Antes bien, pretenden proporcionar información crítica sobre cómo podemos servirle mejor y asegurarnos de que usted comprenda su diagnóstico y el sistema de asistencia médica para el VIH. Es bueno que sepa que muchas organizaciones que prestan servicios requieren que usted presente documentos adicionales, además del formulario utilizado en la sesión inicial, cuando llegue usted a su primera cita, tales como formularios de consentimiento para recibir servicios. Por otra parte, la recertificación de elegibilidad se llevará a cabo cada seis meses después de la sesión inicial. Beneficios de la Coordinación del Caso Su especialista de la sesión inicial conversará con usted sobre los beneficios de la coordinación del caso y cómo puede esta ayudarlo en sus esfuerzos por controlar su salud a largo plazo, brindarle apoyo a medida que usted y sus seres queridos se adaptan al diagnóstico y un enlace de ayuda para futuros servicios comunitarios que pudiere usted necesitar, tales como los servicios de consejería médica y legal y recursos tales como los prestados por el Seguro Social (Social Security), Medicaid, Medicare, Medicare Parte D y muchos más. Los coordinadores del caso (case managers) están especialmente capacitados para responder a las necesidades de las personas que padecen del virus VIH y para ayudarlos a planificar sus necesidades financieras, emocionales y físicas a largo plazo. Ellos pueden abogar por sus derechos y ofrecerle orientación sobre cómo 17

20 tener acceso a servicios, apoyo, información y asistencia médica de importancia. La coordinación del caso se ofrece gratis a las personas que padecen del VIH, independientemente del sueldo que perciban. Qué Sucederá Después? Una vez que el especialista de la sesión inicial haya llenado los documentos requeridos así como la evaluación de su elegibilidad y necesidades, le proporcionará remisión inmediata para la asistencia médica y servicios de apoyo que usted necesite. Le informarán cómo puede usted hacer citas y qué otros documentos podría necesitar, los cuales sean exclusivos para cada organización de servicios. Le proporcionarán además remisión médica para los servicios mencionados que necesite y le brindarán información sobre cómo la coordinación del caso puede ayudarlo a obtener acceso a servicios adicionales que podría usted necesitar en el futuro. Aun cuando estos especialistas pueden proporcionarle remisión inicial al momento de llevar a cabo su primera visita, sólo el coordinador del caso puede proporcionarle las remisiones necesarias posteriormente. Por tal motivo, es importante que considere la coordinación del caso como un servicio que podría usted necesitar en el futuro. Una vez que se haya realizado la sesión inicial y que haya recibido usted su paquete y formulario de remisión, sólo tendrá que llamar a las organizaciones mencionadas para programar su primera cita. Asegúrese de preguntar si existe información adicional que deba traer consigo además del paquete de la sesión inicial que le hayan proporcionado. Si usted gusta, el especialista de la sesión inicial lo ayudará a programar estas citas. Cuánto dura la Sesión Inicial? Usted debe hacer planes para pasar de dos a cuatro horas en su sesión inicial a fin de darle al especialista el tiempo suficiente para llenar los documentos requeridos, explicarle el sistema de asistencia médica para pacientes que padecen del VIH, ofrecerle información sobre la enfermedad, explicarle la importancia de la asistencia médica continua y remitirlo para que reciba usted la asistencia médica y recursos que necesite. Puedo Traer a un Acompañante? Claro que sí! Es importante que sus seres queridos se involucren en la asistencia médica y apoyo que usted reciba. Ellos son bienvenidos a la sesión inicial. Podemos ayudarlos a comprender mejor el VIH y a convertirse en un sólido sistema de apoyo para usted. Podría haber además servicios que podamos ofrecerles como familiares afectados por la situación, tales como servicios de consejería o guardería para el cuidado de sus niños. Por otra parte, tenemos acceso a muchos otros servicios comunitarios no relacionados con el VIH, los cuales podrían ser muy útiles para las personas encargadas del cuidar al paciente (caregivers), compañeros y familares. Qué Sucederá con Mi Información? La información proporcionada durante la sesión inicial se recauda e ingresa una base de datos del Texas Department of State Health Services database, conocida como ARIES. La información de ARIES se considera como segura y confidencial a fin de proteger la privacidad del paciente. Se utiliza para hacerle seguimiento a la epidemia del VIH, las necesidades de las personas afectadas por la enfermedad y los servicios utilizados por ellos.
