2 The NYS Department of Labor Direct Payment Card, available through Chase Bank, is safer and more convenient than paper checks. With the Direct Payment Card, you can access your cash 24 hours a day, 7 days a week through Automated Teller Machines (ATMs) and retail Point-of-Sale (POS) terminals anywhere. IMPORTANT: Before you can use the Direct Payment Card, you must call Chase Customer Service at to activate your Card by selecting your access code and your Personal Identification Number (PIN). You should also sign the back of your Card right away. YOU MUST KEEP THIS CARD FOR YOUR CURRENT CLAIM AND FUTURE USE.

3 Using Your Card at ATMs You can use your Card to withdraw cash in local currency at over one million ATMs anywhere, with no-cost access at Chase, Bank One and Allpoint ATMs in the U.S. Just look for ATMs that display the logos on the back of your Card and: 1. Insert or swipe your Card. 2. Enter your 4-digit PIN. 3. Press Withdrawal and then Checking. Follow the instructions until the transaction is completed. 4. Take your cash, your Card and your receipt. Using Your Card at Retail Locations You can use your Card anywhere that accepts MasterCard debit cards, including grocery stores, gas stations, restaurants and more! To use your Direct Payment Card at a retail location, just insert your Card into the Point-of-Sale (POS) terminal and choose debit or credit. If you choose Debit: 1. Enter your 4-digit PIN. 2. Tell the clerk if you would like cash back with your purchase. (Some stores do not allow this option. Please ask the cashier about the store s policy.) 3. The clerk will enter the purchase amount and the amount of cash you requested (if applicable). You will receive a receipt along with your Card, your purchases and your cash (if applicable). If you choose Credit: You cannot get cash back and will be asked to sign a receipt. You will receive a receipt with your Card and your purchase. All purchases and cash back amounts will come directly out of your account.

The Direct Payment Card has no daily limit for ATM withdrawals. You may withdraw up to the balance available in your Direct Payment Card account or the maximum amount allowed by the ATM. Will I have to pay ATM surcharges? It depends on which ATM you use. The Direct Payment Card is part of the Chase, Bank One and Allpoint ATM networks, which means you have access to over 38,000 no-fee ATMs nationwide. Please note that at Allpoint ATMs you must press YES to accept the surcharge and proceed with your withdrawal. You will not be charged for the surcharge, nor will the surcharge be deducted from your account. You can verify this by checking your ATM transaction receipt. For the location of the nearest Allpoint ATM, visit To locate the nearest Chase or Bank One ATM, visit Many other ATM owners do charge a fee to use their machines. If you use an ATM with a surcharge, that fee will be charged to your account. What if I enter the wrong PIN? If you are having trouble remembering your PIN, DO NOT try to guess your PIN when entering it on a POS terminal or ATM. When you enter the wrong PIN, you have two more chances to enter the correct number. If the correct PIN is not entered by the third try, you must wait until after midnight to try again or call Chase Customer Service for assistance. What is the difference between a PIN and an access code? Your PIN is a four-digit number that you enter on the keypad at ATMs and retail locations. Your access code

5 is a six-digit number that you enter when you call Customer Service. You must select both numbers when you call to activate your Card. What if my Card doesn t work? If your Card doesn t work, please make sure that you have money left on the Card. If an ATM doesn t accept your Card, simply try another ATM. The first ATM may be out of service or may not be on the network that accepts the Card. What if I forget my PIN or access code? If you forget your PIN or access code, call Chase Customer Service to select a new one. You should choose numbers that are easy for you to remember, but hard for someone else to figure out. Can I go to a bank teller and withdraw money or inquire about my Direct Payment Card? You may withdraw money from a teller at a Chase or Bank One location, or any bank that displays the MasterCard logo. If you have questions, call Chase Customer Service at Are there any fees for using my Card? Please see your Direct Payment Card Disclosure Statement and User Agreement for details. What should I do if I lose my Card? If your Direct Payment Card is lost, stolen or damaged, call Chase Customer Service toll-free immediately to receive a new Card. What is the best way to avoid paying fees to access my money? To avoid paying fees, it is important to keep track of your account balance. You can do this at Chase s cardholder website ( at any Chase, Bank One or Allpoint ATM, or by visiting a teller at any bank that displays the MasterCard logo. You can also get cash without paying a fee at these locations. You can also use your Direct Payment Card to make purchases at millions of retail locations that accept MasterCard debit cards.

La Tarjeta de Pago Directo no tiene límite diario para los retiros en cajeros automáticos. Usted puede retirar todo el saldo disponible en su cuenta de Tarjeta de Pago Directo o la cantidad máxima permitida por el cajero automático. Tengo que pagar recargos del cajero automático? Cuando ingresa el PIN incorrecto, tiene otras dos posibilidades de ingresar el número correcto. Si no ingresa el PIN correcto en el tercer intento, debe esperar hasta después de la medianoche para intentarlo nuevamente o llamar al Servicio al Cliente de Chase para solicitar asistencia. Cuál es la diferencia entre un PIN y un código de acceso? Su PIN es un número de cuatro dígitos que usted ingresa en el teclado de los cajeros automáticos y establecimientos minoristas. Su código de acceso es un número de seis dígitos que usted ingresa cuando llama al Servicio al Cliente. Debe elegir ambos números cuando llama para activar su Tarjeta.

10 Cómo usar su tarjeta en cajeros automáticos... Usted puede usar su tarjeta para retirar dinero en moneda local en más de un millón de cajeros automáticos, en cualquier lugar, sin costo por acceso en los cajeros automáticos de Chase, Bank One y Allpoint en los EE. UU. Simplemente busque cajeros automáticos que exhiban los logotipos que figuran en el reverso de su Tarjeta y haga lo siguiente: 1. Inserte o deslice su Tarjeta. 2. Ingrese su PIN de 4 dígitos 3.
