PILAR VIGIL, BMED, MD, PHD MEDICAL DIRECTOR OF THE RHRI studied medicine at the Pontifical Catholic University of Chile from 1975 to She specialized in obstetrics and gynaecology and went on to complete a PhD in Biological Sciences (Physiology) in She undertook postgraduate training at Texas Institute for Reproductive Medicine, Houston, Texas, USA; and Royal Women s Hospital, Melbourne, Australia. Professor Vigil has seen her work published in more than 100 different international scientific publications (articles, books and book chapters), alongside numerous publications and interviews in journals dedicated to areas of general interest and education. She has written three books on fertility and has organized and/or participated in more than 200 congresses or seminars. She has been invited to give lectures in over 30 countries worldwide, and awarded repeatedly as women leader by the press in Chile. Her current professional appointments include Associate Professor at the Pontifical Catholic University of Chile. She has worked since 1978 in reproductive medicine including fertility, infertility and natural family planning. Together with Professor J. B. Brown she has developed the concept of the ovarian continuum. She has received numerous awards for her work in medicine and is a Member of the Chilean Fertility Society and Member of the Chilean Reproduction Society and is a member of the Latin-American Society for research on human reproduction. Dr. Vigil is also president of the San Lucas Academy [Chilean Catholic Medical doctors] and currently is President of Teen STAR international. She is also a member of the Chilean Obstetricians and Gynaecologists Society and a member of the Chilean Natural Family Planning society. Pilar is married to Miguel and they have 4 children.

2 S A R E RHRI MEDICAL PROTOCOLS RHRI develops standardized medical protocols for FEMM-trained doctors and nurses. These protocols reflect specialized research in clinical reproductive endocrinology. Over the last 60 years, research in this branch of medicine has given rise to the oral contraceptive pill and in vitro fertilization (IVF), but has not equipped doctors with the critical information to diagnose and treat the underlying hormonal imbalances that lead to conditions such as dysmenorrhea (severe menstrual pain), amenorrhea (cessation of menstruation), polycystic ovarian syndrome (PCOS), and infertility. Using the same body of endocrinological research, RHRI medical researchers, led by Dr. Pilar Vigil, MD, PhD, have defined standards for endocrinological health. Their work enables doctors to diagnose and treat a wide range of abnormalities. Dr. Vigil specialized in obstetrics and gynecology before completing a PhD in physiology and postgraduate training at the University of Texas, Houston, in Medicine, and Melbourne, Australia. She worked closely with Dr. James Brown, whose methods for testing hormonal levels and longitudinal studies of the ovarian continuum established him as a leading specialist in women s reproductive endocrinology. She is the founding Medical Director of the RHRI. The work of the RHRI is focused on understanding and recognizing healthy endocrine activity. Having collected over thirty years of data on the natural continuum of women s hormonal levels, she has demonstrated that hormonal imbalances express themselves in a number of different ways, such as: cycle irregularities, pain, fatigue, depression, acne, hirsutism,

3 and infertility. While the symptoms of hormonal imbalance may at times appear to be very similar as is the case with Addison s disease and adrenal insufficiency the underlying disorders are distinct and necessitate specific kinds of treatment in order to recover the lost standard of hormonal health. Without understanding how hormonal imbalance disrupts women s health, doctors are unable to diagnose and treat these types of symptoms effectively. Doctors frequently prescribe the pill to patients suffering from a wide range of gynecological disorders. Though the Pill can alleviate some patients symptoms of acne, irregular periods, dysmenorrhea, amenorrhea, and PCOS, it does not treat the underlying health problem. Similarly, doctors recommend IVF treatment for those who suffer from infertility. While IVF can enable a couple to have a child, it does not resolve the underlying health concerns. T P Ongoing research at RHRI is developing medical protocols to diagnose and treat hormonal imbalance. The protocols take into account data from the charting of a woman s cycle and from tests of up to nine different hormones in her blood and urine. Of course, a woman s age, lifestyle, and, clinical history are also considered. FEMM The protocols lay out the specific conditions, and sequence of tests to diagnose a wide range of endocrinological disorders. They also outline the treatment necessary for women to recover healthy hormonal function. Depending on the measured deficiencies, treatment can range from immunological support, including removal of stress factors, to targeted pharmacological interventions. Women s reproductive health needs attention: infertility is an estimated $2 billion industry in the United States alone, and a reported 30% of women who use the pill do so for treatment of gynecological problems. RHRI medical protocols provide a welcome contribution to women s healthcare. As doctors learn to use these protocols, women will gain access to more effective treatment for their health problems.
