RESUMEN Introducción: la ablación de la fibrilación auricular es una opción terapéutica en pacientes seleccionados. Sin embargo, se ha estudiado poco acerca de la eficiencia de este tratamiento. Material y métodos: Los pacientes presentaban fibrilación auricular paroxística o persistente, sintomáticos, con 3 o más episodios de fibrilación auricular al año y resistente a 3 o más fármacos antiarrítmicos. Se han comparado los costes generados tras la ablación (visitas al servicio de urgencias, ingresos hospitalarios, pruebas diagnósticas y tratamiento) con el mismo grupo de pacientes antes de la ablación y con un período de seguimiento similar. Resultados: Se incluyeron un total de 61 pacientes en el estudio, 52 varones con una edad media de 47 +/ 13 años. El seguimiento medio tras la ablación fue de 22+/ 14 meses. Tras la ablación, el 90% de los pacientes presentaron mejoría clínica, si bien el 42,60% presentó recurrencia de la fibrilación auricular, aunque con menos frecuencia e intensidad. En cuanto al tratamiento farmacológico antiarrítmico, existe una clara menor necesidad de estos tras la ablación, reduciéndose su toma hasta en un 50%. En cuanto a la necesidad de asistencia médica, existe un aumento significativo en los pacientes antes de la ablación (3,6 ± 1,7 visitas a la urgencia tras la ablación versus 26,1 ± 5,3 en el grupo control, p = 0,001; 0,24 ingresos al año en el grupo tratado versus 1,21 en el grupo control, p = 0,006 y con una duración mayor de los ingresos). Si analizamos los costes generados en cada grupo y el coste adicional inicial que supone la ablación de la fibrilación auricular, el procedimiento se amortiza a los 21,1 meses. Conclusiones: La ablación de venas pulmonares en el tratamiento de la fibrilación auricular es una técnica que una vez superada la curva de aprendizaje presenta escasas complicaciones, evitando la recurrencia en más de la mitad de los pacientes. Mejora la sintomatología en el 90% de los pacientes, disminuyendo los ingresos hospitalarios. Este tratamiento supone una alternativa más eficiente al tratamiento convencional. Madrid ABSTRACT Introduction: The ablation of atrial fibrillation constitutes a therapeutic option in selected patients. However, the efficiency of this treatment has been little studied to date. Material and methods: The patients included in the study presented symptomatic paroxysmal or persistent atrial fibrillation, comprising 3 or more episodes of atrial fibrillation a year, with resistance to 3 or more antiarrhythmic drugs. A comparison was made of the costs generated following ablation (visits to the Emergency Room, hospital admissions, diagnostic tests and treatment) versus the costs generated by the same group of patients before ablation (control group) and involving a similar follow-up period. Results: A total of 61 patients were included in the study (including 52 males; mean age 47 ± 13 years). The mean follow-up after ablation was 22 ± 14 months. After ablation, 90% of the patients showed clinical improvement, though 42.6% suffered recurrence of atrial fibrillation, of comparatively lesser frequency and intensity. As to pharmacological antiarrhythmic treatment, a clearly lesser need for medication was observed after ablation with reductions of up to 50%. In relation to medical care, significantly greater needs were recorded among the patients before ablation: 3.6 ± 1.7 visits to the Emergency Room following ablation versus 26.1 ± 5.3 in the control group, p = 0.001; 0.24 admissions a year in the treated group versus 1.21 in the control group, p = 0.006, and with a longer duration of hospital stay. On analyzing the costs generated in each group and the initial additional cost implied by the ablation of atrial fibrillation, the latter is seen to be amortized within 21.1 months. Conclusions: The ablation of pulmonary veins in the treatment of atrial fibrillation poses few complications once the technique is mastered, and avoids recurrence in over half of all patients. Symptoms improvement is observed in 90% of cases, with a reduction in the number of hospital admissions. The treatment is more efficient than conventional management. Los pacientes con esta arritmia tienen el doble de mortalidad y un riesgo aumentado de ictus. Es un problema médico significativo que afecta un elevado porcentaje de la población española, y como resultado de esto, produce una pérdida en la calidad de vida y grandes costes económicos. Puede presentarse de múltiples formas: paroxística, persistente, permanente, como un primer episodio o recurrente y cursar de forma sintomática o asintomática (1-9). En la pasada década los arritmólogos pusieron todo su empeño en conocer la etiología y la fisiopatología de esta enfermedad. Como resultado de estas investigaciones surgieron nuevos conceptos sobre el inicio, perpetuación y el tratamiento de esta arritmia. Sin embargo, hasta ahora, la mayoría de los pacientes eran subsidiarios del tratamiento tradicional: fármacos antiarrítmicos y/o cardioversión eléctrica. La cardioversión eléctrica de la fibrilación auricular persistente es un tratamiento muy efectivo para la restauración del ritmo sinusal, aunque presenta un porcentaje considerable de recurrencias. Muchas de estas recurrencias son la consecuencia clínica del remodelado eléctrico del tejido auricular con acortamiento del período refractario del músculo auricular (10-17). Uno de los problemas importantes a los que nos debemos enfrentar cuando hablamos de fibrilación auricular es que debemos intentar clasificarlo, ya que el pronóstico y el tratamiento no depende sólo del diagnóstico de fibrilación auricular, sino si ésta es paroxística (generalmente autolimitada con episodios de duración inferior a 7 días), persistente (no autolimitada, duración superior a 7 día), o permanente, en el que no se realizó cardioversión o se optó por mantener la frecuencia cardiaca permaneciendo el paciente en fibrilación auricular de forma crónica. Es bien sabido que cuando el episodio de fibrilación auricular persistente tiene una duración larga, superior al año, las posibilidades de mantener posteriormente el ritmo sinusal disminuyen progresivamente (16-22). Recientes estudios han mostrado que la estrategia basada en el control del ritmo no ofrece ninguna ventaja en cuanto a la supervivencia sobre el grupo del control de la frecuencia cardíaca. (13-15). Sin embargo, estos resultados deben interpretarse con cautela, pues los pacientes inclui- Atrial fibrillation (AF) is the most frequent arrhythmia in clinical practice. Patients with atrial fibrillation present a two-fold increase in mortality and an increased risk of stroke. This arrhythmia constitutes a significant medical problem that affects a large proportion of the Spanish population, resulting in important economic costs and impaired patient quality of life. Atrial fibrillation can present in different forms paroxysmal, persistent, permanent, as a first episode, or recurrent and may be either symptomatic or asymptomatic (1-9). In the past decade, specialists in arrhythmia focused effort on defining the etiology and physiopathology of the disease. As a result of these investigations, new concepts have emerged regarding the origin, perpetuation and treatment of this arrhythmia. However, to date, most patients have been subjected to traditional treatment in the form of antiarrhythmic drugs and/or electric cardioversion. The electric cardioversion of persistent atrial fibrillation is very effective for restoring sinus rhythm, though recurrences are quite frequent. Many of these recurrences are the clinical consequence of electric remodeling of the atrial tissue, with a shortening of the refractory period of the atrial myocardium (10-17). One of the major problems encountered in dealing with atrial fibrillation is that the disorder requires classification, since patient prognosis and treatment depend not only on the actual diagnosis of atrial fibrillation but also on whether the latter is paroxysmal (generally self-limited with episodes lasting less than 7 days), persistent (not self-limited, with a duration of over 7 days), or permanent (where no cardioversion is performed, or the decision is taken to maintain the heart rate keeping the patient in chronic atrial fibrillation). It is well known that when persistent atrial fibrillation episodes are prolonged (for over one year), the possibilities of subsequently maintaining sinus rhythm gradually decrease (16-22). Recent studies have shown that the strategy based on cardiac rhythm control affords no advantage in terms of survival versus the heart rate control group (13-15). 3 Ablación de la Fibrilación Auricular Ablation of atrial fibrillation dos no representan a todo el espectro de enfermedad (pacientes con un primer episodio de fibrilación auricular, pacientes jóvenes, entre otros), diferente tratamiento entre los grupos. Por otra parte, la posible ventaja de mantener a los pacientes en ritmo sinusal probablemente se vea encubierta por los efectos adversos que presentan los fármacos antiarrítmicos utilizados en estos estudios (en el AFFIRM era aceptable de acuerdo con el protocolo el empleo de amiodarona, disopiramida, flecainida, moricizina, procainamida, propafenona, quinidina sotalol y combinaciones de estos fármacos). Hace más de una década, Coplen et al (23), publicaron los resultados de un metaanálisis con Quinidina. Resultaba evidente que la Quinidina era efectiva en la prevención de las recurrencias de la fibrilación auricular, pero con un incremento en la tasa de mortalidad (odds ratio 2.98, p < 0.05). Pocos fármacos antiarrítmicos tras el estudio CAST (24) han demostrado ser neutrales en cuanto a la supervivencia, incluyendo las situaciones de inestabilidad eléctrica como el infarto de miocardio, como la Amiodarona (EMIAT (25) y CAMIAT (26)). La lucha contra la fibrilación auricular ha comenzado recientemente con el uso de nuevos agentes cardioprotectores capaces de prevenir el remodelado o modificar los canales (27-34). Parece que el calcio intracelular juega un papel importante en el remodelado eléctrico (35), aunque el tratamiento con antagonistas del calcio no siempre ha sido efectivo (36). Diversos estudios con ARA IIs y/o IECAs han demostrado un efecto positivo en la prevención de los episodios de fibrilación auricular tanto en humanos como en animales. Pedersen et al (28) investigaron el efecto del Trandolapril en la incidencia de fibrilación auricular en pacientes con disfunción ventricular izquierda. El Trandolapril redujo el riesgo de desarrollar fibrilación auricular en un 55%. Los inhibidores de la enzima convertidora de angiotensina podrían tener un efecto beneficioso debido a su efecto sobre la fibrosis en el aparato cardiovascular y la apoptosis (29). Por otra parte, la disponibilidad de nuevas terapias que pueden permitir su curación, como la ablación con radiofrecuencia y el aislamiento de venas pulmonares, pueden permitir un cambio en el manejo de grupos de estos pacientes, para lo que es necesario una evaluación apropiada, así como una disminución futura en la morbimortalidad de esta enfermedad. On the other hand, the possible advantage of keeping patients under sinus rhythm is probably diminished by the adverse effects of the antiarrhythmic drugs used in these studies (in accordance with the trial protocol, the AFFIRM study accepted the use of amiodarone, dispyramide, flecainide, moricizine, procainamide, propafenone, quinidine sotalol and combinations of these drugs). Over a decade ago, Coplen et al. (23) published the results of a meta-analysis with quinidine. While this drug proved clearly effective in preventing recurrences of atrial fibrillation, it also led to an increase in mortality (odds ratio (OR) 2.98, p < 0.05). Following the CAST study (24), few antiarrhythmic drugs have been found to be neutral as regards patient survival, and conditions have been reported including electric instability such as myocardial infarction, associated with amiodarone for example (EMIAT (25) and CAMIAT studies (26)). Efforts to combat atrial fibrillation have recently included the use of new cardioprotective agents capable of preventing remodeling or modify ion channels (27-34). In this context, intracellular calcium seems to play an important role in electric remodeling (35), though treatment with calcium antagonists has not always proven to be effective (36). Different studies with angiotensin II receptor antagonists (ARA IIs) and/or angiotensin-converting enzyme inhibitors (ACEIs) have shown positive effects in the prevention of atrial fibrillation in both humans and animals. Pedersen et al. (28) investigated the effect of trandolapril upon the incidence of atrial fibrillation in patients with left ventricle dysfunction. Trandolapril reduced the risk of developing atrial fibrillation by 55%. ACEIs in turn could exert beneficial effects as a result of their action upon cardiovascular fibrosis and cell apoptosis (29). On the other hand, the availability of new healing therapies such as radiofrequency ablation and isolation of the pulmonary veins, could allow for changes in the management of adequately selected patients, with future reductions in the morbidity-mortality associated with atrial fibrillation. Los pacientes con fibrilación auricular pueden ser tratados mediante la ablación de los substratos de la taquicardia supraventricular y de los focos en las venas pulmonares. El paciente con episodios de fibrilación auricular paroxística recurrente es un candidato idóneo para la ablación por catéter (12, 38, 39). La ablación percutánea de la fibrilación auricular tiene buenas perspectivas actuales, como se reconoce en una editorial y artículo recientes de la Revista Española de Cardiología (35, 36), y puede ser considerada como una opción terapéutica de elección en determinados pacientes con fibrilación auricular paroxística sintomática y recidivante, a pesar del tratamiento médico. Sin embargo, este tratamiento supone un coste no despreciable, que debe ser evaluado para obtener la mayor eficiencia en el tratamiento de la fibrilación auricular. MATERIAL Y MÉTODOS Se incluyeron en el estudio a los pacientes remitidos a nuestro servicio por fibrilación auricular paroxística o persistente, sintomáticos, con 3 o más episodios de fibrilación auricular al año y que sea recurrente a pesar de la toma de 3 o más fármacos antiarrítmicos. Finalmente se incluyeron 61 pacientes y se analizaron los gastos generados en este grupo tras la ablación. Como grupo control se utilizó el mismo grupo con un seguimiento similar previo a la ablación. Análisis de costes Se analizaron los costes directos generados por los pacientes a lo largo del seguimiento, desde el momento de la inclusión hasta el final del mismo. Los costes se dividieron en varios grupos: costes generados por complicaciones, gastos generados del tratamiento farmacológico, costes debidos a pruebas diagnósticos y finalmente, los gastos generados por el equipo facultativo. Los costes generados por las complicaciones incluyeron gastos por hospitalización en planta de Cardiología y/o de Medicina Interna por descompensación debidos a la fibrilación auricular y estancias en el Servicio de Urgencias. Para el cálculo de los gastos generados por las hospitalizaciones se multiplicó cada día de ingreso (en been shown to be effective in the management of a number of arrhythmias. Patients with atrial fibrillation can be treated via ablation of the substrates for supraventricular tachycardia and arrhythmogenic foci in the pulmonary veins. Individuals with episodes of recurrent paroxysmal atrial fibrillation are ideal candidates for catheter ablation (12, 38, 39). The percutaneous ablation of atrial fibrillation offers interesting perspectives, as has been commented in a recent editorial and article published in the Revista Española de Cardiología (35, 36). The procedure may be considered the management of choice in certain patients with symptomatic and recurrent paroxysmal atrial fibrillation despite medical treatment. However, this novel therapeutic approach entails considerable costs that must be assessed in order to ensure increased efficiency in the treatment of atrial fibrillation. MATERIAL AND METHODS The present study comprised the patients referred to our Department with symptomatic paroxysmal or persistent atrial fibrillation, comprising 3 or more episodes of atrial fibrillation a year, with resistance to 3 or more antiarrhythmic drugs. A final total of 61 subjects were included, with conduction of an analysis of the costs generated in this group following ablation. As control group we used the same patients prior to ablation and involving a similar follow-up period. Cost analysis An analysis was made of the direct costs generated by the patients from the time of inclusion to the end of follow-up. The costs were divided into a number of groups: costs due to complications, costs generated by pharmacological treatment, costs due to diagnostic tests, and lastly costs associated to the attending medical team. The costs generated by complications in turn included expenses related to hospitalization in the Cardiology and/or Internal Medicine ward because of decompensations attributed to atrial fibrillation, and stays in the Emergency Room. 5 Ablación de la Fibrilación Auricular Ablation of atrial fibrillation planta o urgencias) por el coste medio de cada indicador ajustándolo por el tiempo de seguimiento. Para el análisis del coste generados por el tratamiento farmacológico, se valoró el tratamiento farmacológico al inicio del tratamiento, así como las modificaciones a lo largo del mismo de cada paciente, multiplicando el coste medio de cada principio activo por el número de días de toma, dividiéndolo por el total de pacientes en cada grupo. En cuanto a la valoración del gasto generado por las distintas pruebas diagnósticas, se incluyeron todas aquellas pruebas realizadas para alcanzar el diagnóstico de fibrilación auricular, así como las que se realizaron a lo largo del seguimiento (electrocardiograma, ecocardiograma, analítica básica, ergometría, holter ECG, costes cardioversión eléctrica y/o farmacológica, estudio electrofisiológico y ablación con catéter). El coste se calculó multiplicando el coste de cada prueba por paciente realizada y dividido por el total en cada grupo, ajustándolo por el seguimiento. Finalmente, se incluyeron costes los sueldos del equipo facultativo como costes directo. Como costes indirectos se analizaron los días de trabajo y productividad perdidos por fibrilación auricular. También se analizó la tasa de mortalidad asociada a cada proceso. Técnica de ablación de la fibrilación auricular Se siguieron las indicaciones de anticoagulación estrictas en estos pacientes y se recomienda realizar un eco transesofágico previamente al procedimiento de ablación con catéter, ya que probablemente un porcentaje no inferior al 1% presentan trombos en orejuela izquierda a pesar de estar anticoagulados. La técnica de aislamiento completo de las venas pulmonares también es posible realizarla de múltiples formas (12). Quizás la más popular consiste en emplear un catéter circular decapolar para mapear el ostium de las venas y guiar la ablación empleando un catéter convencional de 4 mm o irrigado. La mayor contribución del catéter circular es que permite la identificación de las conexiones eléctricas entre la aurícula izquierda y venas pulmonares. Estas áreas muestran el retrasos más corto entre el potencial de vena pulmonar y la aurícula izquierda, bien en ritmo sinusal o con estimulación en seno coronario. In turn, the costs generated by the different diagnostic tests performed included all those tests made to establish the diagnosis of atrial fibrillation, together with the tests carried out in the course of follow-up (electrocardiogram, echocardiogram, basic laboratory tests, exercise testing, Holter ECG recording, electric and/or pharmacological cardioversion, electrophysiological study and catheter ablation). The costs were calculated by multiplying the cost of each test per patient divided by the total in each group, and adjusting for follow-up. Finally, the salary expenses associated with the attending medical team were also defined as direct costs. As indirect costs, we analyzed the number of working days and productivity lost because of atrial fibrillation. The mortality rate associated to each process was also analyzed. Atrial fibrillation ablation technique Strict anticoagulation measures are observed in these patients, and a transesophageal echocardiographic study is recommended prior to catheter ablation, since probably no fewer than 1% present left atrial appendage thrombi despite anticoagulation. The complete pulmonary vein isolation technique can be done in several ways (12). The most popular approach may be the use of a decapolar circular catheter to map the ostium of the veins and guide ablation with a conventional 4-mm or irrigated catheter. The principal advantage of the circular catheter is that it allows identification of the electric connections between the left atrium and the pulmonary veins. These areas show the shortest delays between the pulmonary vein potential and left atrium in either sinus rhythm or upon pacing at the coronary sinus. También proporciona un end-point de la ablación muy claro, todos los potenciales de venas pulmonares deben desaparecer o estar disociados tras la ablación. Con esta técnica, la primera lesión se realiza proximal al catéter circular enfrente del bipolo que muestra el retrasos entre potenciales más corto. Posteriormente se continúan con aplicaciones hasta el aislamiento, o bien la anulación de esta conexión y que aparezca o no otro conexión en un lugar distinto. La aproximación electrofisiológica más correcta sería realizar ablación de solamente las venas pulmonares arritmogénicas. Sin embargo, la combinación de la evidencia empírica y prágmática sugiere que la ablación sistemática de las 4 venas pulmonares debería reducir el número de procedimientos requeridos para conseguir una curación completa. De nuevo, esto es aceptable si el riesgo de estenosis de vena pulmonar es muy bajo, como en centros con más experiencia. Con esta técnica, el 99% de las venas pulmonares se pueden desconectar satisfactoriamente. La tasa de recurrencia no es baja, requiriendo en ocasiones un segundo procedimiento, bien porque se recupere la conexión entre la vena y la aurícula, o bien por la aparición de nuevos focos. Sin embargo, la tasa de éxito es razonable. Más del 70% de los pacientes en los que se realiza ablación con catéter obtienen mejoría, muchos sin fármacos antiarrítmicos o con fármacos que previamente eran inefectivos. Otra aproximación, consiste en la descrita por Pappone (39), que realiza lesiones lineales largas de forma circular alrededor de los ostium de las venas pulmonares guiado por sistema de mapeo no fluoroscópico CARTO. Con esta técnica se han descrito porcentajes de éxito similares a la anterior. Análisis estadístico Los datos se expresan como media y desviación estándar para variables contínuas y como frecuencias para variables categóricas. Para el análisis estadístico se utilizó la chi-cuadrado para las variables cualitativas, la t de student para comparación de medias en aquellas variables con una distribución normal y la U de Mann-Whitney cuando no seguían la distribución normal. This is acceptable provided the risk of pulmonary vein stenosis is very low (as in more experienced centers). With this technique, 99% of all pulmonary veins can be satisfactorily disconnected. The recurrence rate is not low, and a second procedure is sometimes needed either because the connection between vein and atrium is restored, or due to the appearance of new arrhythmogenic foci. Nevertheless, the success rate is reasonable, and over 70% of patients subjected to catheter ablation improve many without antiarrhythmic drugs or with drugs that were previously ineffective Another approach has been proposed by Pappone (39), who performed long linear lesions in circular form around the ostium of the pulmonary veins, guided by a nonfluoroscopic mapping system (CARTO). This technique has been found to offer a percentage success similar to that of the previously described procedure. Statistical analysis The data are expressed as the mean and standard deviation in the case of continuous variables, and as frequencies for categorical variables. The chi-square test was applied for the comparison of qualitative parameters, with the Student t-test for contrasting means of variables exhibiting a normal distribution. 7 Ablación de la Fibrilación Auricular Ablation of atrial fibrillation RESULTADOS Se incluyeron un total de 61 pacientes en el estudio, 52 varones con una edad media de 47 +/ 13 años. El seguimiento medio tras la ablación fue de 22+/ 14 meses. Como grupo control se utilizó el mismo grupo de sujetos antes de la ablación, con un seguimiento de tiempo similar (21+/ 9 meses). Las características basales de los pacientes se resumen en la tabla 1. Destaca que únicamente 10 pacientes presentaban cardiopatía estructural (hipertensiva). El resto de los pacientes no presentaban cardiopatía. Los pacientes incluidos en el estudio presentaban fibrilación auricular paroxística o recurrente muy sintomática, resistente al menos a 3 tratamientos antiarrítmicos y con más de 3 episodios de fibrilación auricular al año. Tras la ablación, el 90% de los pacientes presentaron mejoría clínica (Figura 1), si bien el 42,60% presentó recurrencia de la fibrilación auricular, aunque con menos frecuencia e intensidad. En cuanto al tratamiento farmacológico antiarrítmico (tabla 2), existe una clara menor necesidad de estos tras la ablación, reduciéndose su toma hasta en un 50%. En nuestra serie, no hubo complicaciones mayores del procedimiento, no produciéndose ningún caso de muerte ni de estenosis de venas pulmonares. Un 9% de los pacientes presentaron complicaciones menores durante el procedimiento que se resolvieron todas sin secuelas (hematoma inguinal, entre otras) En cuanto a la necesidad de asistencia médica, existe un aumento significativo en los pacientes antes de la ablación (3,6 ± 1,7 visitas a la urgencia tras la ablación versus 26,1 ± 5,3 en el grupo control, p = 0,001; 0,24 ingresos al año en el grupo tratado versus 1,21 en el grupo control, p = 0,006 y con una duración mayor de los ingresos) statistical package was used throughout. The rest of patients showed no cardiopathy. The patients included in the study presented highly symptomatic paroxysmal or recurrent atrial fibrillation resistant to at least three antiarrhythmic drugs, and with over three atrial fibrillation episodes a year. After ablation, 90% of the patients showed clinical improvement (Figure 1), though 42.6% suffered recurrence of atrial fibrillation, of comparatively lesser frequency and intensity. As to pharmacological antiarrhythmic treatment (Table 2), a clearly lesser need for medication was observed after ablation with reductions of up to 50%. In our series there were no major complications associated with the procedure, and no cases of death or pulmonary vein stenosis were recorded. Nine percent of the patients suffered minor complications during the procedure that were all resolved without sequelae (inguinal hematoma, among other problems). Como consecuencia de esta mayor necesidad de asistencia hospitalaria y de fármacos antiarrítmicos el coste que se genera en este grupo de pacientes es mayor que tras la ablación. Como se muestra en la Figura 3, si analizamos los costes generados en cada grupo y el coste adicional inicial que supone la ablación de la fibrilación auricular, el procedimiento se amortiza a los 21,1 meses. As can be seen in Figure 3, on analyzing the costs generated in each group and the initial additional cost implied by the ablation of atrial fibrillation, the latter is seen to be amortized within 21.1 months. Nuestro trabajo demuestra que los pacientes sometidos a ablación tienen una menor necesidad de terapia antiarrítmica, lo que sin duda contribuye a la mejor calidad de vida de estos pacientes y a un menor coste económico. Aunque el 42,60% de los pacientes incluidos en nuestra serie presentaron recurrencia de fibrilación auricular, ésta era menos e intensa y frecuente, como indica el hecho de que únicamente el 10% no presentó mejoría clínica. Suzuki S et al (40) ha señalado recientemente la influencia tan negativa que tienen los episodios de fibrilación auricular en la calidad de vida de estos pacientes, no sólo por la sintomatología en el momento de la arritmia, sino por la ansiedad tan importante que genera el hecho de que ésta pueda comenzar. Por otra parte, Suzuki señala cómo un mejor control de esta arritmia, hace que disminuya la ansiedad y mejore la calidad de vida. Engelmann M et al (41) publicó recientemente que la calidad de los pacientes con fibrilación auricular se relacionaba muy estrechamente con los efectos secundarios asociados a la terapia antiarrítmica. Hagens V et al (43) señalaron recientemente, que el control de la frecuencia cardíaca supone una alternativa más eficiente que el control del ritmo en paciente con fibrilación auricular persistente. Sin embargo, la población analizada en este estudio difiere de la de nuestro trabajo, pues en este estudio los pacientes eran mayores que nuestra población (69 años versus 47) y presentaban mayor cardiopatía estructural (31% cardiopatía isquémica, 19% enfermedad valvular, 13% miocardiopatía y un 40% presentaban hipertensión arterial versus un 10% de cardiopatía hipertensiva). Sin duda estas diferencias en cuanto a las características de la población expliquen estas diferencias. Por otra parte, Ten MP (44) señala que para una mayor eficiencia, el manejo del paciente con fibrilación auricular debe ser el resultado de un análisis individualizado de las circunstancias de cada caso particular, valorando los antecedentes del enfermo, la necesidad de anticoagulación, terapia antiarrítmia y sintomatología. En el estudio COCAF (45) analiza los costes generados por los pacientes con fibrilación auricular, señalando, al igual que ocurre en nuestra serie, que el mayor coste generado por estos pacientes proviene de las hospitalizaciones y de las rhythm control was found to be due to the side effects of antiarrhythmic drugs (among other factors). Our findings indicate that patients subjected to ablation have a lesser need for antiarrhythmic therapy a fact that undoubtedly contributes to improve quality of life and reduce the economic costs. Although 42.6% of the patients included in our series suffered recurrence of atrial fibrillation, the latter proved less frequent and intense as reflected by the fact that only 10% failed to show clinical improvement. Suzuki et al. (40) have recently emphasized the strong negative impact of atrial fibrillation episodes upon patient quality of life not only due to the symptoms at the time of arrhythmia, but also because of the anxiety caused by knowing that such episodes may manifest. On the other hand, Suzuki et al. pointed out that improved control of the arrhythmia causes anxiety to decrease, and patient quality of life therefore improves. Engelmann et al. (41) recently reported that the quality of life of patients with atrial fibrillation is closely related to the side effects of antiarrhythmic therapy. Hagens et al. (43) likewise recently published that heart rate control is more efficient than rhythm control in patients with persistent atrial fibrillation. However, the population analyzed in this study was older than our own (69 years on average, versus 47), and the prevalence of structural heart disease was greater (ischemic heart disease in 31%, valve disease in 19%, myocardiopathy in 13%, and hypertension in 40% of subjects versus only 10% in our series). Undoubtedly, these population differences account for the observed discrepancies. On the other hand, Ten (44) commented that in order to ensure increased efficiency, the management of patients with atrial fibrillation must be based on the individualized analysis of each patient assessing disease antecedents, the need for anticoagulation, antiarrhythmic therapy and symptomatology. The COCAF study (45) analyzed the costs generated by patients with atrial fibrillation, and in coincidence with our own observations found the largest proportion of expenditure to correspond to hospital admissions and Emergency Room visits. Study limitations In this study the control group consisted of the same patients that were posteriorly subjected to pulmonary vein ablation, and involving a similar duration of follow-up. Although this implies that the two groups had the same baseline characteristics, the fact that they did not start therapy at the same time (concurrent groups) may have modified the results obtained. CONCLUSIONES La ablación de venas pulmonares en el tratamiento de la fibrilación auricular es una técnica que una vez superada la curva de aprendizaje presenta escasas complicaciones, evitando la recurrencia en más de la mitad de los pacientes. La ablación de la fibrilación auricular mejora la sintomatología en el 90% de los pacientes, disminuyendo los ingresos hospitalarios. Este tratamiento supone una alternativa más eficiente al tratamiento convencional. CONCLUSIONS The ablation of pulmonary veins in the treatment of atrial fibrillation poses few complications once the technique is mastered, and avoids recurrence in over half of all patients.
