3 introduction section 1 This directory provides a list of Alignment Health Plan s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage. You will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician/Provider (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care services from your PCP. Your PCP is a Primary Care Physician who meets state requirements and is trained to give you basic medical care. You will get routine or basic care from you PCP. Your PCP can also coordinate the rest of the covered services you need. In most cases, you must see your PCP to get a referral before you see any other health care provider. The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers. Other physicians/providers are available in our network. There may be times when you get a bill from a non-plan provider for the full cost of medical care you received. If this happens, you should send us the bill along with any documentation of any payment you have made for consideration. Town and Country Rd., Suite 300, CA If we decide that the medical care is not covered, or you did not follow all the rules, we will not pay for our share of the cost. Instead, we will send you a letter that explains the reasons why we are not sending the payment you have requested and your rights to appeal the decision. When you have a medical emergency, you believe that your health is in serious danger. A medical emergency can include severe pain, a bad injury, a sudden illness, or a medical condition that is quickly getting worse. Emergency care can always be obtained in or out of the service area from the nearest available provider. You must use plan providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor Alignment Health Plan will be responsible for the costs. If you use non-plan providers, you may have to pay more. III
