3 CONNT FOR TCHNOLOGY AND DIGITAL ROURC U INTRUCTION Parental permission is required in order for your student to access technology and digital resources at school. The Wake County Public chool ystem (WCP) uses a variety of technology and digital resources to enable and enhance instruction. With permission, students may use physical devices, including but not limited to, computers, tablets, ipads, and ipods (all of which allow some degree of Internet access.) tudents may also access web- based applications to create, review, store, share and potentially post their work on the Internet. xamples of these tools include, but are not limited to Google Apps for ducation (not Gmail), A Curriculum Pathways, and WCP student - Mail (K- 5 at the principal s request). In addition, student information and student work may be maintained by and stored on web- based instructional sites and applications. Not all tools are used at all grade levels. WCP has several processes in place to protect students while using technology and digital resources. tudents are educated every year about appropriate online behavior, including interacting with other individuals on social networking websites and cyber bullying awareness and response. The district also uses Internet filters to remove most harmful content. tudents Internet activity and e- mail communications may be monitored by school personnel as provided in Board Policy tudents are expected to use technology and digital resources under their teacher s direction for educational purposes only in accordance with Board Policy 3225 and related 3225 R&P referred to collectively as the Responsible Use Policy or RUP. You may grant permission for your student to access technology and digital resources. You should select this option if you want your student to use computers, tablets, etc. and be allowed to access web- based curriculum tools. Your permission grants WCP the right to create a WakeID necessary to access web- based instructional tools. The WakeID is visible in various applications to teachers and students across the school system. Parents may deny permission for their student to access technology and digital resources. 5 tudent Name: xhibit C-3 MINOR TUDNT ND UR PARTICIPATION AGRMNT This MINOR TUDNT ND UR PARTICIPATION AGRMNT (this Agreement ), effective as of the date executed below ( ffective Date ), is entered into by and between National Center for Research in Advanced Information and Digital Technologies d/b/a Digital Promise ( Digital Promise ) and (the parent or guardian ( Parent/Guardian ) of the minor student participating in the Digital Promise program (the tudent Participant ), who shall be referred to as the nd User or you or Your ) collectively referred to herein as the Parties. By your signature, you acknowledge and agree to the following: PURPO tudent Participant will receive a wireless device and data service at no charge in order to facilitate participation in the Digital Promise program. The wireless device and service have been donated by Verizon Wireless to Digital Promise and are provided to tudent Participant by Digital Promise under the terms and conditions of this Agreement. This Agreement may be terminated at any time by Verizon Wireless for any reason. The term of this Agreement shall be from the date of execution by Parent/Guardian until tudent Participant is no longer an active participant in the program. WIRL DVIC AND RVIC The wireless device is intended for tudent Participant s use alone, solely for purposes of the program, and it cannot be sold or transferred to any other person or entity. Verizon Wireless will provide voice and data service solely to facilitate participation in the program; however, if tudent Participant has excessive usage, makes international calls/text messages with the wireless device, stream games, or stream audio or video other than in connection with the program, the service may be limited, slowed or terminated without notice. At the end of tudent Participant s participation in the program, Verizon Wireless will discontinue wireless service to the device and you agree to return the device to the district program manager. TRATMNT OF INFORMATION AOCIATD WITH U OF TH DVIC AND RVIC Information about the tudent Participant s use of the wireless device and service, including, but not limited to, details of when he or she used data services or placed calls and to whom, is information of Digital Promise, as the customer of record for the device and service. Therefore, this information may be accessed by and shared with Digital Promise, and those parties to whom Digital Promise authorizes, including the Verizon Global Corporate Citizenship Organization ( VGCCO ). FAILUR AND/OR DIRUPTION OF RVIC Cellco Partnership d/b/a Verizon Wireless ( Verizon Wireless ) or its vendors and suppliers shall have no liability whatsoever for nd User s losses, claims or damages for any cause whatsoever, including direct, indirect, special, consequential, treble or punitive damages, or for limitations in service, including but not limited to, any failure or disruption of services provided hereunder, regardless of the form of action, whether in contract or in tort or otherwise. This limitation includes losses, damages, claims or expenses of any kind arising out of the use or attempted use of the services, or inability to access life support or monitoring systems or devices, 911 or 911 or other emergency call or service. You shall not be deemed a third-party beneficiary of any contract between Digital Promise and Verizon Wireless.

11 TUDNT NAM AND PHOTOGRAPH/VIDO PRIVACY RLA INTRUCTION This form explains potential uses of student photographs and video images by the Wake County Public chool ystem (WCP) and allows you to grant or deny permission to the WCP to release your child s image for display or publication. Yearbook and class photos are handled separately. If you do not want your child to be in the class photographs or yearbook, contact the school directly. This form also allows a parent or guardian the choice whether or not their student may be identified by name on the school or district s Internet websites. tudent names may be released unless a parent or guardian has expressly contacted the school and requested that their student s directory information not be shared. However, as a safeguard, the district does not directly publish student names to the Internet unless given permission by a parent or guardian. The WCP uses internal and external media to highlight the K-12 experience in a variety of ways, which may include the use of photographs and videos of students. For example, student images may be published or displayed in printed materials (such as brochures and newsletters), videos, school websites, and information about school events and activities provided to external organizations and media outlets. Parents have two options for granting or denying consent: Parents may deny permission for any display or publication of their student s image.
