1 Dear Third and Fourth Grade City of Chicago Teacher: The Chicago Symphony Orchestra is seeking 10 outstanding current third and fourth grade City of Chicago students to participate in an exciting full scholarship program. This program, which began in the fall of 1995, includes weekly classes and private lessons with a member of the Chicago Symphony free of charge. The success of this program, which is sponsored by the Negaunee Music Institute at the Chicago Symphony Orchestra, depends on the strength of the recruiting process. We need your help in finding 10 of the most talented, hardest-working students in Chicago. Most importantly, we need students who work well with others and have a great sense of responsibility and commitment. Equally important, for students to succeed in the program they must have extremely supportive, dependable parents and a strong sense of self-discipline. An application is enclosed for distribution to your best students. You can also download an electronic copy from If you have more than one student whom you feel is well qualified, you may photocopy the enclosed application. The student will need to do the following three steps to be considered for this program: 1. Fill out Part I (student questionnaire). 2. Have you, their teacher; fill out part III (Teacher Questionnaire). 3. Have the student s parent or guardian do the following: Read the letter addressed to Dear Parent or Guardian. Fill out part II of the application (parent questionnaire) Provide three (3) self-addressed business-size envelopes with the student s home address. Staple all 3 parts of the application (teacher, student, & parent) together and mail (no FAXs) the completed forms and envelopes to: Patricia Dash (The Percussion Scholarship Program) 812 Pinto Lane Northbrook, IL Applications must be postmarked no later than Monday, May 4, Faxes cannot be accepted. After this deadline, qualified students & parents or guardians will be invited to attend the Percussion Scholarship Group s spring concert at 7:00pm on Sunday, June 7, 2015 at Symphony Center and schedule an interview. (Please note that attendance at this concert is a mandatory component of the application process.) A neat, well thought-out application is important in our decision process. We suggest that you work out your application on a blank sheet of paper so you can check your spelling and sentence structure before preparing your final draft.
