2 Welcome to your NURSE ADVICE LINE What should I do? Emergency Room? Urgent Care Center? You re not alone! A few minutes is all it takes to get help. Talk to a nurse who will help you decide what to do next... 2

3 NURSE ADVICE LINE This is a no-cost benefit for OHP fee-for-service clients. Talk to a nurse any time you have a question. Help! When you need it! Day or night, ask a nurse: How do I treat a fever? What do I do if my health changes? I forgot to take a pill, what should I do? If you are sick, hurt, or want to talk to a nurse about: Vomiting, cramps or other pains Falls, balance problems, scrapes, and cuts Understanding what a doctor told you Or help you decide when to: Care for yourself at home Call a doctor Call or go to the emergency room Call if you think the situation might be life or limb threatening. 3

4 Oregon Health Plan Care Coordination Client Rights and Responsibilities As an Oregon Health Plan fee-for-service client you are enrolled in a no-cost program offered by the Oregon Department of Human Services called Oregon Health Plan Care Coordination (OHPCC). This program will help you with your healthcare needs and support you in having the best health possible. A team of healthcare providers, including a nurse health coach, will work with you to achieve these goals. It is important for you to know your rights while participating in this program. I have the right to: 1. Get information about OHP Care Coordination services and programs. 2. Talk to a nurse day or night, about my healthcare. 3. Get the names and contact information of my nurse or health coach. I can also ask to talk with their boss. 4. Have my information remain private. OHP Care Coordination will only use my records for providing health services. OHP Care Coordination can only release my information according to State and Federal laws. 5. Be treated with respect as an individual. 4

5 6. Get these services regardless of my: Race Sexual preference Ethnicity Age National origin Health Status Religion Need for health Gender care services Marital status Disability 7. Be involved in making decisions about my health. As allowed by law, a family member or guardian may represent me. 8. Speak with OHP Care Coordination staff in my own language. If needed, OHP Care Coordination will get me a translator for free. 9. Be told of the benefits for participating in the OHP Care Coordination Program. 10. Be educated on how people are chosen for the program. 11. Refuse any service offered and retain all your other benefits. 12. Have an honest talk about all health services that might help me and information on available options and alternatives. 13. To receive information in a way I can understand. 14. Be informed of new health benefits and programs from OHP Care Coordination. 15. Know if this program changes or ends. 5

6 16. Talk openly about the OHPCC Program, and my rights. I also have the right to state my opinion without fear of punishment or discrimination. 17. Continue to receive all other DHS and OHP benefits whether or not I participate in this program. 18. Exercise all my rights without fear of being treated differently by the OHP Care Coordination Program. The Nurse Advice Line helps you get the help you need. It is not medical treatment. You may call your doctor with any questions you have about your treatment or care. If you think you may have a life or limb threatening emergency, call your doctor or 911 immediately. 6

11 LÍNEA DE ASESORAMIENTO DE ENFERMEROS Este es un beneficio gratuito para los clientes que pagan por el servicio de Oregon Health Plan (OHP). Si tiene alguna pregunta, hable con un enfermero en cualquier momento. Ayuda! Cuando la necesita! De día o de noche, pregunte a un enfermero: Cómo trato la fiebre? Qué hago si cambia mi estado de salud? Olvidé tomar una píldora, qué debo hacer? Si está enfermo, lesionado o desea hablar con un enfermero acerca de: Vómitos, calambres u otros dolores Caídas, problemas de equilibrio, raspones y cortes Comprender lo que le dijo su médico Necesita que lo ayuden a decidir cuándo: Cuidar de usted mismo en su hogar Llamar a un médico Llamar al o ir a la sala de emergencias Llame al si piensa que la situación podría poner en peligro su vida o sus extremidades. 11

12 Programa de coordinación de atención de Oregon Health Plan derechos y responsabilidades del cliente Como cliente que paga por el servicio de Oregon Health Plan, usted está inscrito en un programa gratuito que le ofrece el Departamento de Servicios Humanos (Department of Human Services, DHS) de Oregón denominado Programa de coordinación de atención de Oregon Health Plan (Oregon Health Plan Care Coordination, OHPCC). Este programa lo ayudará con sus necesidades de atención de la salud y le brindará apoyo para que tenga la mejor salud posible. Un equipo de proveedores de atención de la salud, incluido un enfermero asesor de salud, trabajará con usted para alcanzar estos objetivos. Es importante que conozca sus derechos mientras participa en este programa. Tengo derecho a: 1. Obtener información sobre los servicios y programas de coordinación de atención de OHP. 2.
