PAY NO LATER THAN. DO NOT MAIL. BRING THIS NOTICE. PAY IN OFFICE. Dear Customer: Our records indicate that your account is still unpaid and overdue. If you do not pay the amount OWED by the date stated above as "PAY NO LATER THAN", the Company will discontinue its natural gas service to you. If there is a bona fide dispute as to a portion of the amount stated to be due, you may obtain a review of that dispute by contacting the head bookkeeper of Raton Natural Gas Company at 1115 South Second Street, Raton, New Mexico 87740, telephone number In the event of such bona fide dispute, the discontinuance proposed by the Company will be held in abeyance if you pay that portion of the bill which is not in dispute, during the period of the review of the dispute. The working hours of the review personnel by telephone are 9:00 a.m. to 12 p.m. and 1 p.m. to 4:00 p.m. Monday through Friday, excepting State holidays. The Lobby Office hours of the review personnel are 10:00 a.m. to 12 p.m. and 1:00 p.m. to 3:00 p.m. If you desire to make arrangements for a Settlement Agreement, you are entitled to do so by coming into the Company office and inquiring about the Settlement Agreement procedures. You also have the right to file a Complaint with the New Mexico Public Regulation Commission in accordance with NMPRC Rule 410, NMAC (Residential Service) or NMPRC Rule 650, NMAC (Commercial Service), if you disagree with the Company's determination of discontinuance of service. If your gas service is cut off, reconnection will be made in accordance with our work order schedule, and only after you have paid the bill in full, a reconnect charge of $20.00, and a security deposit, for reconnection of Gas Service.

4 Notice of Your Rights and Responsibilities Dear Residential Customer: This is to inform you that your Raton Natural Gas payment is past due. As a service to you, this form outlines ways that you might be able to avoid disconnection under NMPRC Rule NMAC. Please read to determine if you qualify. Your service will be disconnected after the date printed on the enclosed bill if payment is not made. Our policies on delinquent payments and service disconnection are described below. If you believe that an error has occurred, contact RNG immediately for a review of your account by calling a RNG Customer Service representative at between 9:00 a.m to 12:00 p.m. and 1:00 p.m. to 4:00 p.m. Monday through Friday. The utility Office Lobby is open for walk-in customers 10:00 a.m. to 12:00 p.m. and 1:00 p.m. to 3:00 p.m. Monday through Friday. Holiday hours may vary. Payment of the undisputed amount of your bill will postpone disconnection until the dispute is resolved. If you are financially unable to pay the full amount ofthis bill, contact RNG immediately. If you qualify, installment agreements and payment plans are available. RNG will not discontinue service to any residence where a seriously chronically ill person resides if, at least two days prior to the disconnection date indicated in the notice you have entered into a written settlement agreement or deferred payment plan with RNG AND BOTH of the following (forms attached): (1) RNG receives an original of the included medical certification form. The form must be completed and signed by a licensed physician, physician's assistant, osteopathic physician, osteopathic physician's assistant, or certified nurse practitioner, stating that service disconnection might endanger the person's health or life, AND (2) RNG receives an original of the included financial certification form stating that the customer qualifies for financial assistance as determined by the New Mexico Human Services Department or a charitable organization, or RNG receives the form proving that the customer is currently eligible for financial assistance from the Medicaid program. If service has been disconnected, RNG will restore service within 12 hours after you have satisfied the requirement above. Medical certificates are valid for 30 days and financial certificates are valid for 30 days. Your obligation to pay your bill is not relieved if utility service is continued or reestablished because of the receipt of the medical and financial certificates. TO RESTORE SERVICE THAT HAS BEEN DISCONNECTED, A RECONNECT FEE MAYBE CHARGED. IF YOU HAVE DIFFICULTY PAYING THIS BILL, AND FEEL YOU MAY QUALIFY FOR ASSISTANCE IN PAYING YOUR UTILITY BILL FROM THE LOW INCOME HOME ENERGY ASSISTANCE PROGRAM OR ANOTHER ASSISTANCE PROGRAM IN YOUR COMMUNITY, CONTACT THE COMMUNITY ASSISTANCE SECTION OF THE HUMAN SERVICES DEPARTMENT AT OR CONTACT THE CUSTOMER SERVICE REPRESENTATIVE AT THIS UTILITY. APPLICATION FORMS FOR THE LOW INCOME UTILITY HOME ENERGY ASSISTANCE PROGRAM ARE AVAILABLE AT THE BILLING OFFICES OF THIS UTILITY AND AT THE HUMAN SERVICES DEPARTMENT. RNG no va a desconectar el servicio de una residencia donde vive una persona que tiene una enfermedad grave 0 cronica con tal que usted haya ilegado a un acuerdo escrito para aplazar el pago de la cuenta u otro arreglo sobre los pagos con RNG, por los menos dos dias antes de la fecha fijada para la desconexion que aparece en el aviso. ADEMAS, debe cumplir con los DOS requerimientos siguientes; (1) RNG debe recibir Ia planilla original de la certificacion medica adjunta la planilla debe ser llenada y firmada por uno de los siguientes profesionales certificados; un medico, un asistente medico, un medico osteopatico, un asistente medico osteopatico, 0 por una enfermera certificada como professional medico, y debe declarar que la desconexion del servicio puede arriesgar la salud 0 la vida de la persona, y (2) RNG debe recibir la planilla original de la certificacion economica adjunta que declara que el c1einte califica para asistencia economica segun la determinacion del Departamento de Servicios Humanos de Nuevo Mexico, 0 de una organizacion, caritativa, 0 RNG recive la planilla que comprueba que el c1einte as elegible actualmente para recivir ayda economica del programa de "Medicaid". Si el servicio ha sido desconectado, RNG 10 restaurara dentro de las 12 horas despues de que se haya cumplido con los requerimientos estipulados en los requisitos anteriores. Los certificados medicos son validos por 30 dias y los certificados economicos son validos por 90 dias. Su obligacion de pagar su cuenta se mantiene aunque el servicio de energia electrica y de gas se continue 0 se vuelva a conectar debido al recibo de los certificados medicos y economicos.
