Qué es la estenosis pilórica? La estenosis pilórica es un engrosamiento del músculo situado en la parte inferior del estómago que puede bloquear o dificultar el pasaje de comida del estómago al intestino delgado. Este engrosamiento del músculo puede hacer que el bebé vomite, a menudo de manera forzada, lo que se llama vómito en escopeta. Es un problema común que afecta a bebés de entre 2 semanas a 3 meses de edad. Cómo se diagnostica? Su doctor examinará el estómago de su bebé para determinar el estado del músculo. Es posible que se haga un ultrasonido o un estudio con contraste para comprobar el engrosamiento del músculo del estómago o para identificar otras posibles causas de los vómitos. Cómo se trata? Su bebé necesitara una cirugía para abrir el musculo y la comunicación entre el estomago y el intestino. Antes de la cirugía le haremos análisis de sangre a su bebé y también necesitará suero por vía intravenosa (IV) para reponer el líquido perdido en los vómitos y la falta de comida mientras espera la cirugía. Su doctor le explicará los detalles de la cirugía y cuál es el mejor momento para llevarla a cabo. En el momento de la cirugía su bebé recibirá anestesia general. Esto significa que para la cirugía su niño recibirá un medicamento que lo mantendrá completamente dormido y no sentirá ningún dolor. Se hace una pequeña incisión encima del estómago. El cirujano abrirá el músculo engrosado del estómago para que el alimento pueda salir del estómago con más facilidad. Por lo general, la cirugía toma menos de una hora y después su bebé estará en la sala de recuperación aproximadamente por una hora más. Es posible que se quede en el hospital entre 1 y 2 días. Tendrá mi bebé algún dolor? Trabajamos junto a usted y su hijo para prevenir y aliviar el dolor todo lo posible. Usted conoce a su niño como nadie. Participe activamente en su recuperación hablando con su proveedor de atención médica acerca de la manera en que su bebé manifiesta dolor. Su bebé puede tener dolor y molestias después de la cirugía. Su doctor le recetará los medicamentos necesarios para calmar el dolor. Cuando salga del hospital puede continuar con Tylenol (acetaminofeno). Siempre consulte con su médico antes de darle cualquier medicina a su bebé. Y la comida después de la cirugía? Su bebé comenzará a comer en unas 4 horas después de la cirugía una vez que esté completamente despierto, seguirá un plan de alimentación. Será alimentado con fórmula o leche materna en un biberón de acuerdo a lo que necesite. Si vomita, tendrá que esperar 2 horas y comenzar de nuevo. Tan pronto como pueda tomar fórmula regular o la leche materna por varias horas, podrá regresar a casa. Y si mi bebé toma pecho? Tal vez haya notado que sus pechos están más blandos y que su leche ha disminuido. Es una respuesta normal del cuerpo si su bebé no ha estado comiendo bien por varios días. Es importante que utilice una bomba sacaleche del hospital mientras su bebé no esté comiendo para aumentar la producción. Si su producción de leche es buena mientras su bebé no esté amamantando, es importante que use la bomba cada 2 o 3 horas o tan a menudo como su bebé normalmente come. Esto ayudará a mantener su producción de leche hasta que su bebé esté listo para tomar pecho otra vez. Contamos con espacio para que guarde la leche de la bomba. Podrá volver a dar el pecho una vez que estemos seguros que su bebé toma algunos biberones sin problema. Si necesita ayuda para mantener o aumentar su producción de leche, le puede pedir a la enfermera que llame a los especialistas en lactancia. Qué cuidados necesita mi niño en el hogar? Dieta y alimentación: su bebé necesitará alimentarse más lentamente y eructar con más frecuencia por algunos días. Sostenga a su bebé en posición vertical para darle de comer y también después de que haya comido. Colocando a su bebé del lado derecho cuando esté acostado, le ayudará a vaciar el estómago. Después de 3 ó 4 días, puede regresar a su rutina de alimentación normal. Incisión: la incisión puede estar cubrierta con alguna venda o apósito a prueba de agua. Algunos cirujanos cubren la herida con cintitas Steri-Strips que se saldrán solas. Puede bañar a su bebé con una esponja 2 días después de la cirugía, pero no debe sentarlo en la tina con agua o en una piscina. Actividad: su bebé no necesita limitar su nivel de actividad. Puede darle los cuidados normales. Citas: una enfermera de Cirugía General le llamará entre 4 y 7 días después de la cirugía para ver cómo sigue su bebé. Si está bien, no necesitará más 2 de 3

5 Patient and Family Education Pyloric Stenosis Care Caring for your child after surgery What is pyloric stenosis? Pyloric stenosis is a thickening of the muscle at the end of the stomach that can block or slow down the passage of food from the stomach to the small intestine. This thickening of the muscle can cause a baby to vomit, often in a forceful way, called projectile vomiting. It is a common problem which affects babies usually 2 weeks to 3 months of age. How is it diagnosed? Your doctor will examine your baby s stomach to feel for the thickened muscle. Your baby may have an ultrasound or contrast study to check for a thickened stomach muscle or to find other possible causes of vomiting. How is it treated? Your baby will need surgery to open the thickened muscle and the passage from the stomach to the intestine. Before surgery, your baby will have blood tests. Also, your baby will need fluids through their veins, called IV (intravenous), to replace fluid lost from vomiting and not eating while waiting for surgery. Your doctor will discuss the timing and the details of the surgery with you. At the time of the surgery, your baby will have general anesthesia. This means we will give your child medicine to make them sleep without pain during the surgery. A small incision is made over the stomach. The surgeon will open the thickened stomach muscle so that food can pass out of the stomach more easily. The surgery usually takes less than 1 hour and your baby will be in the recovery room for about another hour. You can expect your baby to stay in the hospital for 1 to 2 days after the surgery. Will my baby be in pain? We partner with you to prevent and relieve your baby s pain as completely as possible. You know your baby best. We encourage you to take an active part in your baby s recovery by talking with your care team about how your baby expresses pain. After a surgery your baby is likely to have some pain and discomfort. No matter the level of your child s pain, we join you to assess and respond right away. We will give medicines to keep your baby comfortable while in the hospital. Your child s doctor will prescribe pain medicine. When you go home, you can continue giving your child acetaminophen (Tylenol) following your doctor s instructions. Check with your doctor first before giving any type of medicine to your baby. 1 of 3

6 Pyloric Stenosis: Care After Surgery What about feeding after surgery? Your baby will start to feed about 4 hours after surgery. When your baby is fully awake, you will begin a feeding plan. Your baby will be bottle fed on demand with formula or breast milk. If your baby vomits, he or she will need to wait for 2 hours and start with ondemand feeds again. As soon as your baby is able to eat regular full-strength formula or breast milk for several hours, your baby can go home. What if my baby breastfeeds? You may have noticed that your breasts are softer and that your milk supply has decreased. This is a normal response if your baby has not been eating well for several days. It is important that you use a hospital-grade electric breast pump while your baby is not eating so that you are able to make more milk. If you have a full milk supply, while your baby is not breast feeding, it is important that you pump every 2 to 3 hours or as often as your baby would normally breast feed. This will help to maintain your supply until your baby is ready to breast feed again. There is a place for you to store the milk you pump. You will be able to breastfeed once we know your baby is able to successfully take a few bottle feedings. You can ask your nurse to contact the lactation support team if you need help increasing your milk supply. How do I care for my baby at home? Diet and Feeding - Your baby will need slower feedings with more frequent burping for a few days. Hold your baby upright to eat as well as after the feeding. Placing your baby on the right side when lying down will help to empty the stomach. After 3 to 4 days, you can resume your usual routine for feeding your baby. Incision - The incision may be covered with some form of waterproof skin glue dressing. Some surgeons will cover the wound with Steri-Strips and these will fall off on their own. Your baby can have a sponge bath 2 days after surgery, but should not soak in the tub or swim. Activity - Your baby does not need to limit any activities. You can take care of your baby like normal. Follow-up A nurse from General Surgery will call you 4 to 7 days after the surgery to see how your baby is doing. If your baby is doing well, you will not need to come in for a clinic visit. Your child s regular doctor will provide routine care. Vaccines may continue on schedule. 2 of 3
