2 Dr. Louis Stoerner Dear Parents and Community Members: EMERGENCY CLOSINGS In the event of emergency school closings, local radio and television stations will be notified so the information can be announced to the public. Superintendent s Message The beginning of a new school year is always an exciting time for our teachers, parents, and especially our students. As our summer draws to an end, the Alief Independent School District anxiously awaits the school year. Our focus remains on preparing our students for tomorrow while caring for them today. The upcoming year will bring new challenges, including beginning the implementation of the state s new accountability system, which is a result of the 81st Texas Legislative Session and is intended to be easier for our public to understand. We also begin the year celebrating the continuation of improvements in our students academic achievement over the last few years. Although our final campus accountability ratings from last year will not be officially released until the end of summer, preliminary reports predict that we will have seven campuses rated exemplary and 17 schools rated as recognized. Unfortunately, although Hastings, Elsik, and Taylor made significant achievement gains and met the academic requirements of being recognized campuses, our completion rates fell just short of the requirement. Completion and/or dropout rates continue to be a concern and focus of most districts across the state. Alief has seen improvements in these figures over the last two years as we have reached out to dropouts and re-enrolled several hundred students into new and innovative programs that better meet their personal and instructional needs. Our Night High School, which allows students who have fulltime employment to return to school, or our S.O.A.R. program, which enables students who have fallen behind to catch up by accelerating the number of classes a student can take during a semester, have been well received and appreciated by high school students. We also think that this year s new Early College High School will be another innovative program that will better meet the needs of our students in becoming college or career ready. The Early College High School, which is a partnership with HCC, will give incoming freshmen the opportunity to gain college credit while meeting their high school graduation requirements. The best part is that it is tuition free. Another exciting new program is our Dual-Language Immersion pilot program, which will begin this fall at Holmquist Elementary. English and Spanish-speaking students will be in the same classroom, receiving 50% instruction in Spanish and 50% in English. One of the goals is for our students to become bilingual and biliterate. These programs are great additions, but we all realize that the most important factor in determining our success is in the learning that takes place in each and every classroom at every campus and every grade level, along with the caring relationship our teachers have with their students. We are fortunate to have some of the best teachers in the state. I feel certain that their efforts, in conjunction with their students hard work and our continued partnerships with our community, will make another exceptional year in Alief. I hope that you have enjoyed the summer so far, and that you have had the opportunity to enjoy time with your friends and family. Students who are eligible for services under 504 are not necessarily also covered under the provisions of Individuals with Disabilities Education Act (IDEA) which calls for the provision of special education services. Section 504 procedures may not be used to resolve disagreements with admission, review, and dismissal committee decisions. Eligible students with disabilities have the right to participate in and benefit from public education programs without discrimination on the basis of a disability. They have the right to equal opportunity to participate in school programs and extracurricular activities. The law states that an eligible person with a disability is one who has, has a record of having, or is regarded as having, a physical or mental impairment which substantially limits a major life activity such as learning, self-care, walking, seeing, hearing, speaking, breathing, working, major bodily functions (immune system, normal cell growth), sleeping, standing, lifting, bending, reading, concentrating, thinking, communicating, and working. In the school setting a substantial limitation on learning must be demonstrated by an education need. This may be evidenced by an inability to be successful with school work, serious behavior problems, or any other physical or mental impairment which interferes with learning. The principal of each building has designated a Campus Section 504 Coordinator who will be the person responsible for implementing eligibility procedures and the provision of educational services. Patrons will be accommodated for registration Monday through Friday and appointments are not necessary. Patrons who wish to register at hours other than the posted times are to be accommodated if they have a sufficient reason (taken time off from work). What to take to registration Please bring originals and copies of: - Two proofs of residence (deed, lease, bank statement, and/or most recent utility bill) - The child s state-certified birth certificate, immunization records, most recent report card, and Social Security number - Photo ID for the parent/guardian (A parent or guardian must accompany any student under the age of 18) Parents enrolling children in prekindergarten and qualifying on the basis of income must bring documentation of all sources of income for all members of the household. Acceptable forms of documentation include payroll check stubs for the most recent three months or the federal income tax return for Where to register Prekindergarten through eighth grade students register at the school in the attendance zone where their home is located. High school students register at the Alief ISD Enrollment Office, located in the Alief Support Facility, Bellaire Boulevard (between Eldridge and Howell Sugar Land Road). Exception: High school students who have left the district and are returning to re-enroll should go to the registrar s office at their former high school campus. Eligibility Kindergarten through 12th grade: Children who reside in Alief ISD and as of September 1, 2009 are at least 5 years old and under 21 years of age, may attend Alief schools. High school students at least 21 years of age and under 26 years of age may be admitted by the district to complete the requirements for a high school diploma for the year. Prekindergarten Enrollment: To enroll in Alief ISD s prekindergarten program, a child must meet the following requirements: - be 4 years old by September 1, reside in Alief ISD - have the inability to speak or comprehend the English language (testing will take place at registration) or the household income is at or is below the subsistence level as defined by the State Board of Education. (Documentation of all sources of income to all members of the household must be submitted at the time of registration. Acceptable documentation includes payroll check stubs for the most recent three months or the federal income tax return for 2008). - present proof of residence, original state-certified birth certificate, immunization records, Social Security number, and photo ID for parent or guardian. Bilingual program registration Parents interested in enrolling their children in the district s Spanish-English Bilingual Program, available for qualifying students in prekindergarten through sixth grade, must first register at their home school (the school in their attendance zone) before going to the assigned bilingual campus. High school assignments Students receive their assignments to Elsik, Hastings, or Taylor High School by means of a computerized random-draw system. Following the draw assignment, those who are interested in attending Kerr High School may apply for admission. Kerr is a smaller school with approximately 700 students who pursue personalized academic plans and work independently or in small groups with teacher support. For more information about Kerr, please call Texas Minimum State Vaccine Requirements for Students Grades K - 12 This chart summarizes the vaccine requirements incorporated in Title 25 Health Services, of the Texas Administrative Code (TAC). This chart is not intended as a substitute for consulting the TAC, which has other provisions and details. The Department of State Health Services is granted authority to set immunization requirements by the Education Code, Chapter 38, Health & Safety, Subchapter A, General Provisions. IMMUNIZATION REQUIREMENTS A student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas. Vaccines Kindergarten 7th Grade 8th - 12th Grade Diphtheria, Tetanus, 5 doses or 4 doses 3 doses 3 doses and Pertussis 1 Tdap/Td booster 1 Tdap/Td booster (DTaP/DTP/DT/Td/Tdap) within last 10 years Polio 4 doses or 3 doses 4 doses or 3 doses 4 doses or 3 doses Measles, Mumps, 2 doses 2 doses 2 doses and Rubella (MMR) Hepatitis B 3 doses 3 doses 3 doses Varicella 2 doses 2 doses 1 dose Meningococcal Hepatitis A 2 doses 1 dose Changes in Texas Immunization Requirements On March 5, 2009, the Executive Commissioner on Health & Human Services approved several revisions to the Immunization Requirements for Children and Students in Texas Public and Private Schools [Title 25. Health Services, Chapter 97, Texas Administrative Code (TAC) and ]. The changes to the requirements were made in order to update the Texas elementary and secondary school immunization requirements so that they adhere more closely to the recommendations of the Centers for Disease Control and Prevention (CDC) and the Advisory Committee on Immunization Practices (ACIP). These changes include: Hepatitis A Vaccine: Beginning August 1, 2009, all students entering kindergarten, statewide, will be required to have two doses of Hepatitis A vaccine. Measles, Mumps, Rubella (MMR) Vaccine: Beginning August 1, 2009, all students entering kindergarten will be required to have two doses of measles, mumps, and rubella (MMR) vaccine. Varicella Vaccine: Beginning August 1, 2009, all students entering kindergarten and 7th grade will be required to have two doses of varicella vaccine. A written statement from a parent (or legal guardian or managing conservator), school nurse, or physician attesting to a child s positive history of varicella disease (chickenpox), or of varicella immunity, is acceptable in lieu of either dose of varicella vaccine. Tetanus, Diphtheria and acellular pertussis-containing vaccine (Tdap): Beginning August 1, 2009, all students entering the seventh grade will be required to have one dose of Tdap vaccine. Students in the seventh grade will be required to have a booster dose of Tdap only if it has been five years since their last dose of a tetanuscontaining vaccine. Students in grades 8-12 are required to have a booster dose of Tdap if it has been ten years since their previous dose of a tetanus-containing vaccine. Td is acceptable in lieu of Tdap if a contraindication to pertussis exists. Meningococcal Vaccine: Beginning August 1, 2009, all students entering 7th grade will be required to have one dose of meningococcal vaccine. 3

4 4 School Meals Program Students purchasing breakfast and lunch in Alief school cafeterias receive a nutritionally balanced meal planned by a registered dietitian. Meals include a meat or a meat alternative, a vegetable and/or fruit, bread, and a choice of reduced-fat, white or chocolate milk. A vegetarian entree is also available, and students may purchase traditional and reduced-fat a la carte and snack items. Breakfast items and lunches are affordably priced. Please note: Nutrition Services does not accept checks. Breakfast Lunch Students (paid) $0.50 $0.75 Students (reduced) $0.20 $0.20 Staff $1.00 $2.15 Visitors $1.50 $2.75 General Texas Department of Agriculture Food Guidelines The Alief ISD Nutrition Services Department is regulated by the Texas Department of Agriculture (TDA) and the City/County Health Departments. In order to comply with these government agencies, the District has the following procedure in place in regards to food being provided to students: * Parents may not bring food or drinks to school for anyone other than their own children. However, TDA does make an exception to this rule for birthdays. * Parents may bring birthday cake/cupcakes for the class to celebrate with the birthday child. However, the Health Department requires that these cakes must come from an approved source a bakery or grocery store that is inspected by the Health Department. To insure this safety issue, the cake must be brought in the original box in which purchased. Cupcakes may also be purchased from the school cafeteria with advance notice. * Cakes prepared in a private home cannot be brought to share with the students. * Approved birthday cakes will be served after the celebrating class has finished lunch. The full Texas Public School Nutrition Policy and more healthy food information is available at National School Breakfast and Lunch Programs Alief ISD participates in the National School Breakfast and Lunch Programs. Application forms for free and reduced-priced meals are available at each campus. Additionally, each student enrolled in elementary school will receive an application to take home on the first day of school. Note, please complete only one (1) application per family. Applications: 1. Applications that are incomplete or filled out incorrectly cannot be processed. 2. Use black ink only. 3. Submit only one (1) application per family. Submitting more than one application delays the approval process. 4. You may take the completed application to the cafeteria of a school that one of your children attends OR to the Alief ISD Nutrition Meals Application office located at High Star. OR, you may mail it to Nutrition Services, PO Box 68, Alief, Texas Once the completed application is received in the meals application office, processing the application takes up to10 working days. Until a notice of approval is received, parents of new students must plan to send food or money for meals to school with their children. Prior Qualification 1. Parents whose children qualified for free or reduced-price meals in Alief ISD for the school year MUST reapply. 2. Eligibility from the previous year will be honored (or carried over) for the first 30 days of the new school year while applications are being processed. 3. An accurately completed renewal application must be received by Nutrition Services by 4:00 p.m. on September 23 to allow time for processing before the end of the 30-day period. If notice of approval of renewed benefits for has not been received by October 5, parents will need to send either food or money for meals to school with their children until notice of approval is received. 4. Eligibility for the first 30 days of the new school year does not carry over for students who received temporary approval for May New applications for these students must be processed and approved before a student can receive meal benefits for For more information regarding the National School Breakfast and Lunch Programs, please call Special Diets Per USDA regulations, per physician s order, the Alief Nutrition Services Department will make food substitutions or modifications for students with disabilities. -As defined under ADA (Americans with Disabilities Act), a person with a disability means any person with physical or mental impairment that substantially limits one or more major life activities. -Substitutions will be made for children whose disabilities restrict their diets as supported by a statement signed by a licensed physician. The physician s statement is required and must include: 1. child s disability 2. an explanation of why the disability restricts the child s diet 3. the major life activity affected by the disability 4. the food or foods to be omitted from the diet and the food that must be substituted Procedure 1. The student and parent are to make an appointment with the school s nurse and bring a physician s statement. Also, bring all special dietary information provided by the dietitian. 2. The nurse will provide a copy of the school s menu to the parent. The parent and student will choose foods from the menu that the student can eat. 3. Parents will be responsible to teach their child about their diet. 4. The student must be responsible for choosing correct food items. 5. Please encourage your child not to eat food from another student s tray. These foods may not be allowed on your child s diet. Review of Student Code of Conduct Handbook During the first week of school, each high school and middle school student will receive a handbook that includes the District Code of Student Conduct. A copy of the code is also inside the handbook furnished to parents and all elementary and intermediate students. To ensure that instruction takes place in a safe and positive environment, Alief ISD requests the support of students, staff, and parents in upholding the code. Parents are asked to review and discuss code provisions with their children, then sign and return the acknowledgment sheet promptly to the school principal. Failure to do so does not exempt students from complying with the Code of Conduct. What Are the Educational Rights of Children and Youth Experiencing Homelessness? Children and youth experiencing homelessness have the right to: Go to school, no matter where they live or how long they have lived there. Stay in the school they were attending when they became homeless. Receive transportation, if feasible, to the school they were attending when they became homeless. Enroll in school without giving a permanent address. Receive the same special programs and services, if needed, as provided to all other children in these programs. 5 Career and Technical Education opportunities offered in Alief ISD With more than 65 individual courses offered in the Career and Technical Education areas, middle school and high school students can determine a plan of study to include a wide array of courses from within the 13 clusters offered in Alief ISD. The six main areas of study include Science and Technical (Agriculture, Food & Natural Resources, Architecture & Construction, Health Science, Information Technology, Science Technology, Engineering & Mathematics), Arts and Communications (Arts, A/V Technology & Communications, Information Technology, Science, Technology, Engineering & Mathematics), Sales and Management (Business, Management, & Administration, Hospitality & Tourism, Marketing, Sales & Services), Social and Personal Services (Education & Training, Health Science, Hospitality & Tourism, Human Services), Business Operations (Business Management & Administration, Hospitality & Tourism, Marketing, Sales & Service), Outdoor and Mechanical (Agriculture, Food & Natural Resources, Architecture & Construction, Manufacturing, Transportation, Distribution & Logistics, Science, Technology, Engineering & Mathematics). Many of the high school courses qualify for articulated college credit, and students have the opportunity to participate in co-op programs, internships and lab programs. Motorists should know that the law goes into effect on Tuesday, September 1, and Alief ISD police officers will be enforcing it. A person will be ticketed and fined for using a cell phone (talking or texting) while driving a motor vehicle in a school zone. It is permissible for a driver to use a cell phone if the vehicle is stopped, or the phone is being used with a hands-free attachment including a speakerphone, Bluetooth, or other piece of equipment allowing the driver to have both hands on the steering wheel. The law also prohibits the use of these devices by school bus drivers unless they are used in an emergency or official capacity. Mensaje del Superintendente El comienzo de un nuevo ciclo escolar siempre es emocionante para nuestros maestros, padres de familia y, especialmente, para nuestros estudiantes. Mientras el verano se acerca a su final, el Distrito Escolar Independiente de Alief espera con ansias la llegada del ciclo escolar Nuestro enfoque sigue siendo preparar a nuestros estudiantes para el mañana mientras cuidamos de ellos hoy. El próximo año traerá consigo nuevos retos, incluyendo el comienzo de la implementación del nuevo sistema de rendición de cuentas del estado, el cual es resultado de la LXXXI Sesión Legislativa de Texas y pretende ser más fácil de entender para el público. Además, comenzaremos el año celebrando la continuación de las mejoras en el aprovechamiento académico de nuestros estudiantes, logradas durante los últimos años. A pesar de que las clasificaciones finales del año pasado de nuestro plantel no serán dadas a conocer oficialmente sino hasta finales de agosto, los informes preliminares predicen que siete de nuestros planteles serán clasificados como ejemplares y 17 escuelas como reconocidas. Desafortunadamente, a pesar de que Hastings, Elsik y Taylor alcanzaron logros significativos y cumplieron con los requisitos académicos para ser planteles reconocidos, nuestros índices de terminación no cumplieron con los requisitos. Los índices de terminación de estudios o deserción escolar siguen siendo una preocupación y el centro de atención de la mayoría de los distritos del estado. Alief ha visto mejoras en estas cifras durante los dos últimos años ya que nos hemos acercado a quienes han abandonado sus estudios y logramos reinscribir a cientos de estudiantes en programas nuevos e innovadores que satisfacen mejor sus necesidades personales y académicas. Nuestra Preparatoria Nocturna, que permite que los estudiantes que trabajan de tiempo completo regresen a la escuela, y nuestro programa S.O.A.R., el cual permite que los estudiantes rezagados se pongan al corriente incrementando el número de clases por estudiante durante un semestre, han sido bien recibidos y apreciados por los estudiantes de preparatoria. Asimismo, creemos que la nueva Preparatoria de Formación Temprana para la Universidad de este año será otro programa innovador que satisfará mejor las necesidades de nuestros estudiantes para que estén listos para sus estudios profesionales o universitarios. Esta Preparatoria, la cual es una sociedad con el Houston Community College, brindará a los estudiantes de primer año la oportunidad de obtener créditos universitarios mientras cumplen con los requisitos de graduación de la preparatoria, pero la mejor parte de su programa es que es gratuito. Otro programa nuevo y emocionante es nuestro programa piloto de Inmersión en Dos Idiomas, el cual comenzará este otoño en la Escuela Primaria Holmquist. Estudiantes anglo e hispanohablantes estarán en el mismo salón de clases, recibiendo la mitad de su educación en español y la otra mitad, en inglés. Una de las metas es que nuestros estudiantes sean completamente bilingües. Estos programas son adiciones importantes, pero todos sabemos que el factor más importante para determinar nuestro éxito es el aprendizaje que se lleva a cabo en todos y cada uno de los salones de clases de cada plantel y en todos los niveles, junto con la relación de afecto y atención que tienen los maestros para con sus estudiantes. Somos afortunados de contar con algunos de los mejores maestros del estado. Estoy seguro de que sus esfuerzos, aunado al arduo trabajo de sus estudiantes y una relación continua con nuestra comunidad, harán del otro ciclo escolar excepcional en Alief. Servicios de Salud, Capítulo 97, Artículos y a del Código Administrativo de Texas (Texas Administrative Code o TAC)]. Elegibilidad De kinder al 12º grado: Los niños que viven en el ISD de Alief y que tienen de 5 a 21 años de edad cumplidos al 1º de septiembre de 2009, deberán asistir a las escuelas de Alief. Los estudiantes de preparatoria que tengan de 21 a 26 años, podrán ser admitidos por el distrito para completar los requisitos del año para preparatoria. Inscripción a pre-kinder: Para su inscripción al programa de pre-kinder del ISD de Alief, un menor debe reunir los siguientes requisitos: - tener 4 años cumplidos al 1º de septiembre de vivir en el ISD de Alief - no saber hablar ni poder entender el idioma inglés (se realizará una prueba durante la inscripción) o tener un ingreso familiar por debajo del nivel de subsistencia definido por el Consejo de Educación del estado. (Deberá presentar comprobantes de todos los ingresos de todos los miembros de la familia al momento de la inscripción. Los documentos que se aceptarán incluyen los recibos de pago de los últimos tres meses o las declaraciones de impuestos federales de 2008). - presentar un comprobante de su domicilio actual, acta de nacimiento original certificada por el estado, cartilla de vacunación, número de seguridad social e identificación con fotografía del padre o tutor. Inscripción al programa bilingüe Los padres que estén interesados en inscribir a sus hijos al Programa Bilingüe Español-Inglés del distrito, disponible para los estudiantes de pre-kinder al sexto grado que reúnan los requisitos, deberán inscribirse primero en la escuela cercana a su hogar (la escuela en su zona de asistencia) antes de asistir al plantel bilingüe designado. Asignación de preparatoria Los estudiantes son asignados a las Preparatorias Elsik, Hastings o Taylor por medio de un sistema computarizado de sorteo al azar. Después de la asignación por sorteo, aquéllos que estén interesados en asistir a la Preparatoria Kerr High podrán solicitar su admisión. Kerr es una escuela más pequeña, con aproximadamente 700 estudiantes que buscan planes académicos personalizados y que trabajan de manera independiente o en grupos pequeños con el apoyo de sus maestros. Para más información acerca de Kerr, llame al Los cambios en los requisitos tuvieron el propósito de actualizar los requisitos de vacunación en las escuelas primarias y secundarias de Texas para que se apeguen más estrictamente a las recomendaciones de los Centros de Control y Prevención de Enfermedades (Centers for Disease Control and Prevention o CDC) y el Comité Asesor sobre Prácticas de Vacunación (Advisory Committee on Immunization Practices o ACIP). Entre estos cambios se incluyen los siguientes: Vacuna contra la Hepatitis A: A partir del 1º de agosto de 2009, todos los estudiantes que ingresen al jardín de niños, en todo el estado, deberán contar con dos dosis de la vacuna contra la Hepatitis A. Vacuna contra el Sarampión, las Paperas y la Rubéola (MMR): A partir del 1º de agosto de 2009, todos los estudiantes que ingresen al jardín de niños deberán contar con dos dosis de la vacuna MMR. Vacuna contra la Varicela: A partir del 1º de agosto de 2009, todos los estudiantes que ingresen al jardín de niños y al 7º grado deberán contar con dos dosis de la vacuna contra la varicela. Se aceptará una declaración escrita de uno de los padres (tutor legal o tutor administrativo), enfermera de la escuela o médico como prueba de que el menor ha padecido varicela o de que es inmune a ella en lugar de recibir la vacuna. Vacuna contra el Tétanos, la Difteria y la Tos ferina (Tdap): A partir del 1º de agosto de 2009, todos los estudiantes que ingresen al 7º grado deberán contar con una dosis de la vacuna Tdap. Los estudiantes que cursen el 7º grado deberán contar con una dosis de refuerzo de Tdap únicamente si han pasado cinco años desde su última vacuna contra el tétanos. Los estudiantes del 8º al 12º grado deberán contar con una dosis de refuerzo de la vacuna Tdap si ya han pasado 10 años desde su última vacuna contra el tétanos. La vacuna Td se aceptará en lugar de la vacuna Tdap si existe alguna contraindicación a la vacuna contra la tos ferina. Vacuna contra el Meningococo: A partir del 1º de agosto de 2009, todos los estudiantes que ingresen al 7º grado deberán contar con una dosis de la vacuna contra el meningococo. 7

8 8 Programa de Alimentos Escolares Los estudiantes que compran el desayuno o almuerzo en las cafeterías escolares de Alief reciben alimentos nutritivos y balanceados, planeados por dietistas profesionales. Los alimentos incluyen carne o una alternativa a la carne, una ración de verdura o fruta, pan y leche a elegir entre natural, con chocolate o baja en grasa. También cuentan con platillos vegetarianos y los estudiantes pueden disfrutar del menú tradicional o bajo en grasa y refrigerios. Los desayunos o almuerzos tienen un precio asequible. Tengan presente que Servicios de Nutrición (Nutrition Services) no acepta cheques. Desayuno Almuerzo Estudiantes (pagado) $0.50 $0.75 Estudiante (reducido) $0.20 $0.20 Personal $1.00 $2.15 Visitantes $1.50 $2.75 Recordatorios del Reglamento del Departamento de Agricultura de Texas Alimentos de mínimo valor nutricional (FMNV, por sus siglas en inglés) y todas las formas de dulces No se permitirá ningún alimento de mínimo valor nutricional (dulces y bebidas gasificadas) a ninguna hora en las instalaciones escolares y a ninguna persona (incluso expositores invitados) sino hasta después de concluir la última clase programada (que incluye el Programa de Refrigerio después de Clases). Los alimentos de la competencia incluyen cualquier alimento o bebida que se vende o se pone a disposición de los estudiantes y que son ajenos a los que proporciona el servicio de alimentos escolares. Todos los alimentos, bebidas y refrigerios deben cumplir con las normas de nutrición y restricciones en el tamaño de la porción de la política de Nutrición del Departamento de Agricultura de Texas (TDA, por sus siglas en inglés). Se permiten algunas excepciones para estudiantes con necesidades especiales. Las fiestas de cumpleaños en el salón de clases deben programarse al finalizar el periodo de alimentos para que la celebración no sustituya un almuerzo nutritivo. Los pasteles deben comprarse en pastelerías o confiterías aprobadas por el Departamento de Salud y entregarse en el empaque original. Para obtener una copia completa de las políticas y sus excepciones, visite Programas Escolares Nacionales de Desayuno y Almuerzo El Distrito Escolar Independiente (ISD, por sus siglas en inglés) de Alief participa en los Programas Escolares Nacionales de Desayuno y Almuerzo. Cada uno de los planteles escolares cuenta con formularios de solicitud disponibles para alimentos gratuitos y de precio reducido. Cada estudiante de primaria recibirá una solicitud para llevarla consigo a casa el primer día de clases. Formularios de solicitud 1. No se podrán procesar formularios incompletos o completos de manera incorrecta. 2. Use tinta negra únicamente. 3. Sólo presente una solicitud por familia. Si presenta más de una solicitud, se retrasa el proceso de aprobación. 4. Entregue la solicitud completa en la cafetería escolar de su hijo(a) o en el Centro de Distribución de Nutrición del ISD de Alief, ubicado en High Star. También podrá enviar por correo la solicitud a la agencia de Servicios de Nutrición (Nutrition Services) a PO Box 68, Alief, Texas Una vez que se recibe la solicitud completa, el proceso de aprobación demora 10 días hábiles. Los padres de estudiantes nuevos deben enviar a sus hijos a la escuela con alimentos o dinero hasta que reciban el aviso de aprobación. Aceptación o Calificación anterior 1. Los padres cuyos hijos contaron con alimentos gratuitos o a precio reducido en el ISD de Alief durante el año escolar deben volver a presentar la solicitud. 2. La elegibilidad del año anterior se respeta durante los primeros 30 días del nuevo año escolar mientras se procesan las solicitudes. 3. Servicios de Nutrición deberá recibir la solicitud de renovación debidamente completada a más tardar a las 4:00 p.m. del 23 de septiembre para su trámite antes de que termine el periodo de 30 días. Si el aviso de aprobación de la renovación de los beneficios para el año escolar no se recibe antes del 5 de octubre, los padres deberán enviar a sus hijos a la escuela con alimentos o dinero hasta que reciban el aviso de aprobación. Para obtener mayor información relacionada con los Programas Escolares Nacionales de Desayuno y Almuerzo, comuníquese al Dietas especiales De acuerdo con el reglamento del Departamento de Agricultura de Estados Unidos (USDA, por sus siglas en inglés), el Departamento de Servicios de Nutrición de Alief realizará sustituciones o modificaciones a los alimentos para los estudiantes con discapacidades. -Según se define en la Ley para Estadounidenses con Discapacidades (Americans with Disabilities Act o ADA, por sus siglas en inglés), una persona con discapacidad significa cualquier persona con una deficiencia física o mental que limita de manera sustancial una o más de sus actividades vitales. -Las substituciones se harán para los niños cuyas discapacidades restrinjan su dieta y estén respaldadas por la declaración firmada de un médico autorizado. La declaración del médico debe incluir: 1. discapacidad del menor; 2. explicación del motivo por el que la discapacidad restringe la dieta del menor; 3. actividad vital afectada por la discapacidad; 4. alimentos que deben omitirse de la dieta y aquéllos que deben sustituirse. Procedimiento 1. El estudiante y el padre de familia programarán una cita con la enfermera escolar y entregarán la declaración del médico, junto con toda información de la dieta especial proporcionada por el dietista. 2. La enfermera les proporcionará una copia con el menú de la escuela. El padre de familia y el alumno elegirán los alimentos del menú que pueda comer el estudiante. 3. Los padres de familia son responsables de enseñar al menor acerca de la dieta. 4. El estudiante debe ser responsable de elegir los alimentos apropiados. 5. Recomiéndele a su hijo(a) que no coma alimentos de la bandeja de otro estudiante: es posible que esos alientos no estén permitidos en la dieta de su hijo(a). De conformidad con las leyes federales y la política del Departamento de Agricultura de Estados Unidos, está prohibido que en esta institución haya discriminación debido a la raza, color, nacionalidad, género, edad o discapacidad. Para presentar una queja por discriminación, escriba al Director de Derechos Civiles del USDA a 1400 Independence Avenue, SW, Washington, D.C , o comuníquese al (800) o al (202) Revisión del manual del código de conducta del estudiante Durante la primera semana de clases, todos los alumnos de preparatoria y secundaria recibirán un manual que incluye el Código distrital de conducta del estudiante. También dentro se encuentra un duplicado del manual que se facilitó a los padres y a todos los estudiantes de educación elemental e intermedia. Para asegurar que la enseñanza tenga lugar en un ambiente seguro y positivo, Alief ISD solicita el apoyo de los estudiantes, del personal y de los padres de familia para mantener el código. Se pide a los padres que revisen y comenten las disposiciones del código con sus hijos y después, firmen y devuelvan a la brevedad posible al director de la escuela el acuse de recibo. No obstante, el no cumplir con lo anterior no libera a los estudiantes de cumplir con el código de conducta. Cuáles son los derechos educativos de los niños y jóvenes que sufren la falta de un lugar para vivir? Los niños y jóvenes que sufren la falta de un lugar dónde vivir tienen derecho a: Asistir a la escuela, sin importar dónde vivan o cuánto tiempo tengan viviendo ahí. A permanecer en la escuela a la que asistían al momento de perder su vivienda. Recibir transporte, si es posible, para llevarlo a la escuela a la que asistía al momento de perder su vivienda. A quedar adscrito a la escuela sin proporcionar una dirección permanente. A recibir los mismos programas y servicios especiales, de ser necesarios, de la misma forma que se proporcionan a otros niños dentro de dichos programas. Caùc maãu taøi lieäu coù theå chaáp nhaän ñöôïc bao goàm caùc cuoáng chi phieáu löông cho ba thaùng gaàn ñaây nhaát hoaëc giaáy khai thueá lôïi töùc cuûa lieân bang cho naêm Nôi ghi danh Caùc hoïc sinh töø lôùp maêng non tôùi lôùp taùm ghi danh taïi tröôøng thuoäc khu vöïc nhaø cuûa caùc em. Caùc hoïc sinh trung hoïc ghi danh taïi Vaên Phoøng Ghi Danh Alief ISD, toaï laïc taïi Cô Sôû Hoã Trôï Alief, Bellaire Boulevard (giöõa Eldridge vaø Howell Sugar Land Road). Ngoaïi leä: Caùc hoïc sinh trung hoïc naøo ñaõ rôøi boû khu hoïc chaùnh vaø trôû laïi ñeå taùi ghi danh caàn ñi ñeán vaên phoøng nhaân vieân ghi danh taïi tröôøng trung hoïc tröôùc kia cuûa caùc em. Khaû naêng hoäi ñuû ñieàu kieän Lôùp maãu giaùo tôùi lôùp 12: Caùc em cö nguï taïi Alief ISD vaø tính tôùi ngaøy 1 Thaùng Chín, 2009 ñöôïc ít nhaát laø 5 tuoåi vaø döôùi 21 tuoåi, coù theå ñi hoïc taïi caùc tröôøng Alief. Caùc phaân boå tröôøng trung hoïc Hoïc sinh ñöôïc phaân boå tôùi taïi caùc tröôøng Trung Hoïc Elsik, Hastings, hoaëc Taylor qua heä thoáng ruùt thaêm ngaãu nhieân baèng maùy ñieän toaùn. Tieáp theo sau söï phaân boå qua vieäc ruùt thaêm, nhöõng em muoán hoïc taïi tröôøng trung hoïc Kerr coù theå laøm ñôn xin nhaäp hoïc. Thuoác Chuûng Beänh Thuûy Ñaäu: Baét ñaàu töø ngaøy 1 thaùng Taùm, 2009, taát caû caùc hoïc sinh vaøo hoïc lôùp maãu giaùo vaø lôùp 7 baét buoäc phaûi ñöôïc tieâm chuûng hai lieàu thuoác chuûng beänh thuûy ñaäu. Moät baûn khai cuûa phuï huynh (hoaëc ngöôøi giaùm hoä hôïp phaùp hoaëc ngöôøi baûo veä), y taù nhaø tröôøng, hoaëc baùc só cho bieát laø ñöùa treû ñaõ coù tieàn söû bò beänh thuûy ñaäu (beänh ñaäu muøa), hoaëc mieãn dòch vôùi beänh thuûy ñaäu, coù theå ñöôïc chaáp nhaän thay cho vieäc phaûi chuûng ngöøa beänh thuûy ñaäu. Thuoác Chuûng Beänh Uoán Vaùn, Baïch Haàu vaø Ho Gaø (Tdap): Baét ñaàu töø ngaøy 1 thaùng Taùm, 2009, taát caû caùc hoïc sinh vaøo hoïc lôùp 7 baét buoäc phaûi ñöôïc tieâm chuûng moät lieàu thuoác chuûng Tdap. Hoïc sinh lôùp 7 seõ baét buoäc phaûi coù theâm 1 lieàu thuoác boài Tdap chæ khi naøo ñaõ ñöôïc 5 naêm keå töø laàn chuûng lieàu sau cuøng thuoác chuûng beänh uoán vaùn. Hoïc sinh caùc lôùp töø 8-12 baét buoäc phaûi coù theâm moät lieàu thuoác boài Tdap chæ khi naøo ñaõ ñöôïc 10 naêm keå töø laàn chuûng tröôùc ñoù thuoác chuûng beänh uoán vaùn. Td ñöôïc chaáp nhaän thay cho Tdap neáu coù phaûn chæ ñònh ñoái vôùi beänh ho gaø. Thuoác Chuûng Beänh Caàu Khuaån Maøng Naõo: Baét ñaàu töø ngaøy 1 thaùng Taùm, 2009, taát caû caùc hoïc sinh vaøo hoïc lôùp 7 baét buoäc phaûi ñöôïc tieâm chuûng moät lieàu thuoác chuûng beänh caàu khuaån maøng naõo. 11

12 Chöông Trình Caùc Böõa AÊn Trong Tröôøng Hoïc sinh mua böõa ñieåm taâm vaø böõa tröa taïi caùc nhaø aên cuûa tröôøng Alief seõ nhaän ñöôïc böõa aên caân baèng veà dinh döôõng maø chuyeân vieân veà aên uoáng coù ñaêng kyù ñaõ hoaïch ñònh. Giaáy tôø cuûa baùc só baét buoäc phaûi coù vaø phaûi bao goàm: 1. söï khuyeát taät cuûa treû 2. moät söï giaûi thích taïi sao khuyeát taät giôùi haïn veà aên uoáng cuûa ñöùa treû 3. hoaït ñoäng chính trong ñôøi soáng chòu aûnh höôûng bôûi khuyeát taät 4. thöùc aên hoaëc caùc thöùc aên phaûi boû qua trong cheá ñoä aên vaø thöïc phaåm phaûi ñöôïc thay theá Thuû tuïc 1. Hoïc sinh vaø phuï huynh phaûi laáy heïn vôùi y taù nhaø tröôøng vaø ñem giaáy baùc só tôùi. Ngoaøi ra, phaûi ñem theo moïi thoâng tin veà cheá ñoä aên uoáng ñaëc bieät maø chuyeân vieân aên uoáng ñaõ cung caáp. 2. Y taù seõ cung caáp cho phuï huynh moät baûn sao thöïc ñôn cuûa nhaø tröôøng. Phuï huynh vaø hoïc sinh seõ choïn thöùc aên töø thöïc ñôn maø hoïc sinh coù theå aên ñöôïc. 3. Phuï huynh seõ chòu traùch nhieäm chæ daãn cho con em hoï veà cheá ñoä aên uoáng cuûa caùc em. 4. Hoïc sinh phaûi chòu traùch nhieäm choïn ñuùng caùc moùn aên. 5. Xin khuyeán khích con em quyù vò ñöøng aên thöùc aên töø khay aên cuûa moät hoïc sinh khaùc. Caùc thöùc aên naøy coù theå khoâng ñöôïc pheùp coù trong cheá ñoä aên uoáng cuûa con em quyù vò. Ñeå phuø hôïp vôùi luaät Lieân Bang vaø chính saùch cuûa Boä Canh Noâng Hoa Kyø, hoïc vieän naøy bò caám khoâng ñöôïc kyø thò döïa treân chuûng toäc, maøu da, nguoàn goác quoác gia, giôùi tính, tuoåi, hoaëc khuyeát taät.
