1 GI Outbreak Survey Instructions The Import Call List will be exported from Infinite Campus. Log into IC and select Ad Hoc from the Index. The HDE Attendance query is located in the following folders. A. Ad Hoc Attendance B. Ad Hoc ES Secretary C. Ad Hoc Registrar D. Ad Hoc Secretary Select the HDE Attendance query from the list. Click Copy. This will create a copy and place it in your Saved Filter list. It will be named Copy of HDE Attendance. Select the query in your list. Click Edit. Select Next at the bottom of the window. Change the attendancedetail.date to the appropriate date. Click in the field with the date and enter the correct date. Click Save. Select the Index tab. Select Data Export under Ad Hoc Reporting. Search for the HDE Attendance query from the appropriate folder. Select the query. From the Pick an Export Format section click in the Delimited values (CSV) radio button. Keep the default Delimiter and Double quote data. Click Export and save the file to your desktop. 1

2 Log into Blackboard Connect Blackboardconnect.com Click the Message Center tab at the top of the window. Select Survey on the Message Type bar. Enter the title GI Outbreak. Click the To button. The Send Message To window will open. Select Import Call List from the Select By drop down list. Click the Select files button and locate the saved file on your desktop. Click Open. 2

3 The file name will appear under the Select files button. Click Done. Click in the Greeting area to activate. Type in the following greeting. Make sure that you enter the name of the person sending the survey message and the name of your school. Hello, This is from the NAME OF SCHOOL. You are receiving this call because your child has been excluded from school due to illness. This is a reminder that your child must not attend school until 72 hours or 3 days after diarrhea or vomiting or both have stopped occurring. We appreciate your helping us control the gastrointestinal outbreak at your child s school by answering this call. We hope your child feels well soon and can return to school. You will receive this call daily during the exclusion period for your child. Please answer the following questions. Thank you. 4 Type the provided question in the question field and enter the provided answer next to a number on the rights. The numbers will correspond with the numerical selection the guardian will push on their phone to select their answer. Click in the Questions area to activate. Type in the following statements. If your child has had any diarrhea or vomiting or both in the last 24 hours please press 1. If your child has not had any diarrhea or vomiting in the last 24 hours press 2. Click the Proceed to Recording button. 4

5 The Record Survey Greeting window will open with the phone number and the survey number. Record each section of the survey. Follow the prompts and record each section of the survey. Please listen to your recording and make corrections if needed. When the recording is complete, click Done. Enter the date by clicking on the calendar icon and selecting the date to send the survey. Select the time of day to send the survey by clicking on the clock icon. When the date and time appear correct in the Select a date field, click Add Selected Date. The Message Review window will open and allow you to review the message information. Check the date and time to make sure they are correct. Click Send. The survey will start preparing to send. GI Outbreak and Text Log into Blackboard Connect. Select the Message Center tab at the top of the window. Select Outreach. 5

6 Enter the title GI Outbreak and Text. Click To. Select Call Import List from the drop down list. Click the Select File button and find the import list that you saved to your desktop. Select the file name. Click Open. The file will load and show how many students are in the file. Select the icon from the Delivery Modes bar. Enter GI Outbreak in the subject field. Enter the following script into the message area. Make sure you enter a name and the name of the school. Hello, This is from NAME OF THE SCHOOL. You are receiving this because your child has been excluded from school due to illness. If you did not participate in the phone survey today, please call your child s school office. Please make sure that you call your child s school today to tell them if your child has had an occurrence of vomiting and/or diarrhea in the past 24 hours. We appreciate your helping us control the gastrointestinal outbreak at your child s school by answering this . We hope your child feels well soon and can return to school. You will receive this message daily until the exclusion period has passed for your child. Thank you. Spanish Version click on the Spanish Icon and enter the following statement. Select a date by clicking on the Calendar Icon and selecting the date to send the message. Select a time by clicking on the Clock Icon and selecting a time to send the message. Click Add Selected Date to finalize the date and time selection. The selected date and time will appear below the Select a date field. Select Next. Review the message data and check the date and time to make sure they are correct. Click Send, the message will begin preparing to send. 7
