6 EDITORIAL La directiva de ALFIM gustaría de desear a todos en este año que se inicia mucha paz, salud y prosperidad. Nosotros tenemos muchos sueños que deseamos concretizar más un año en la dirección de ALFIM. Estamos preparando un plan de acción para el 2012 con las prioridades que nos parece fundamentales para el fortalecimiento de la Física Médica en América Latina. Este plan será discutido con todos ustedes lo más pronto posible. Los avances tecnológicos llegan en América Latina rápidamente, pero estamos preparados? Cómo unir esfuerzos para apoyar los países de la región que más necesita ayuda? Son preguntas que tenemos que contestar con acciones concretas e inmediatas. No hay duda con respecto al potencial de los Físicos Médicos de América Latina pero hay que reforzar las Asociaciones de Física Médica, así como la unión entre ellas. Solo así es que podriamos avanzar y resolver problemas críticos que tenemos y que en muchos casos son comunes en todos los países de la región. Nuestra revista tiene como objetivo buscar profesionales expertos para discutir nuevas tecnologías, formación de profesionales de las diferentes áreas y en especial compartir experiencias con vosotros. Estamos estableciendo secciones que deben ser permanentes en todas las ediciones de la revista, como: formación y entrenamiento en la área medica; innovaciones tecnológicas-artículos técnicos en RT, MN y RX; experiencias de los países de América Latina y apoyos recibidos de cooperación internacional, entrevistas con profesionales de reconocimiento internacional; novedades y anuncios de congresos. Para el próximo número me gustaría hacer algo especial, convocar a todos los presidentes de las Asociaciones de Física Médica a escribir un resumen de la situación actual de la Física Médica en su país: avances y necesidades. Esto es de extrema importancia para nuestro diagnostico y adecuación de los planes de acciones específicos para cada país. Lo más importantes es estar unidos, compartiendo experiencias y dedicándonos a fortalecer la Física Médica. Aprovecho para agradecer lo apoyo que estamos recibiendo de importantes instituciones como la OPS, OIEA, AAPM, AFOMP, IOMP entre otros. Para nosotros es fundamental poder contar con la experiencias de los miembros de estas importantes instituciones y todos tenemos un objetivo común de maximizar los beneficios para el paciente de la aplicación de la radiación para diagnostico y terapia. Profesor Claudius REGAUD Dr Alfonso ESGUERRA GOMEZ Dr Gonzalo ESGUERRA GOMEZ (Fotos Tomadas del libro Setenta años del Cancer en Colombia del Dr Efraín Otero Ruiz y del archivo personal de la familia Esguerra Gomez) El Programa de Cooperación Franco-Colombiano en Salud se inicia en 1934 por el Dr. Claudius Regaud y los doctores colombianos Alfonso y Gonzalo Esguerra Gómez en la creación del Instituto de Rádium, hoy llamado Instituto Nacional de Cancerología (INC) (foto 1), el cual fue una réplica del Instituto Curie de París. Las relaciones franco-colombianas continúan con el Plan Nacional de Cáncer, de 1975 a Posteriormente, gracias a las gestiones de madame Andrée Dutreix, Jefe del Servicio de Radiofísica del Instituto Gustave Roussy, ante el gobierno francés, cuatro VSNA (Voluntariados del Servicio Nacional Activo) lograron trabajar en el Instituto de Cancerología de 1987 a 1999, bajo el título de Cooperantes del Gobierno Francés. Ellos fueron Jean Marc Tauziede, Didier Paul, Vincent Pagnol y Jean Luc Codaforno. El programa también permitió intercambios de médicos e e ingenieros entre el INC y los hospitales de la Assitance Publique de París y el Institut Gustave Roussy de Villejuif. Uno de estos cuatro cooperantes franceses, Didier Paul, quien hoy en día es profesor del INSTN de la CEA de Francia y presidente de honor de la Asociación LARD (Laboratorios Asociados de Radiofísica y de Dosimetría), ha sido la persona que más ha participado desde Francia en el desarrollo de la Física Médica en Colombia, no solo con su trabajo profesional durante su pasantía como VSNA en el INC, sino también por su constante apoyo en la creación de la Maestría en Física Médica y del Doctorado en Física con línea de especialidad en Física Médica en la Universidad Nacional de Colombia y por la multitud de actividades que ha desarrollado conjuntamente con la profesora María Cristina Plazas, promotora de la Física Médica en el Instituto Nacional de Cancerología, la Universidad Nacional de Colombia y el Hospital Universitario Fundación Santa Fe de Bogotá. El profesor Didier Paul ha realizado varias actividades de Física Médica en Colombia. Entre ellas podemos mencionar: - Como VSNA en el Instituto Nacional de Cancerología (INC) y en la Embajada de Francia en Bogotá ( ). Su pasantía en Colombia tuvo dos grandes objetivos: desarrollar la Física Médica en el Instituto Nacional de Cancerología, en conjunto con la Universidad Nacional de Colombia, y crear un albergue para acoger a los niños enfermos de cáncer tratados en el INC. - En 1999, la Secretaría Distrital de Salud de Bogotá lo invitó, en su condición de experto de la Comisión de Energía Atómica francesa, a participar como conferencista en el Seminario sobre Desechos Hospitalarios que tuvo lugar en el Hotel Tequendama. Durante este periodo participó también como profesor en un curso de Radioprotección en la Universidad Nacional de Colombia. Didier aprovechó para visitar también el Albergue Luisito y, en conjunto con las damas voluntarias, realizó el balance de las donaciones de la Asociación de Ayuda para los Niños, con sede en Tours, que había logrado conseguir desde su creación el 15 de noviembre de A finales de 2000 fue invitado por el Service de Coopération et d Action Culturelle de la Embajada de Francia en Bogotá con el objetivo de realizar cuatro conferencias como experto del INSTN (Instituto de Ciencias y Tecnologías Nucleares) en el INC y en la Universidad Nacional de Colombia y participar en el seminario sobre Protección radiológica en salud ocupacional y para colaborar en la formación de los físicos médicos en Colombia con la conferencia Simulación Montecarlo en radiaciones ionizantes. - En 2005 participaron, por Colombia, la profesora María Cristina Plazas y, por Francia, los profesores Didier Paul y Libor Makovicka en la convocatoria del gobierno francés ECOS-NORD y de Colciencias en Colombia, con el proyecto Estudio comparativo teórico y experimental sobre varios dosímetros para la puesta en marcha de un programa de control de calidad de los pacientes y de los Enero 2012 pág. 2

8 trabajadores ocupacionalmente expuestos (dosímetros operacionales) sobre la base de los que han sido desarrollados recientemente en Francia. Aprobado por las instituciones organizadoras, este proyecto permitió el intercambio con la Universidad Franche Comté de un alumno de la Universidad Nacional de Colombia, Ubaldo Nerio, y la realización de un trabajo de grado sobre la caracterización de los dosímetros operacionales por parte del alumno de la Universidad Nacional de Colombia Johnatan Osorio. - En el marco del proyecto ECOS-NORD, los profesores María Cristina Plazas y Harold Machado realizaron una visita científica a Francia con el siguiente cronograma de actividades: París 1. Visita al laboratorio primario de calibración de cámaras de ionización del CEA en Saclay. 2. Visita a la Unidad de Radioterapia del Instituto Gustave Roussy e intercambio de experiencias. Montbéliard 3. Visita, en la Universidad de Franche Comté, al laboratorio de Crest, dirigido por el Dr. Libor Makovicka. - Estudio del funcionamiento del código EGSnrc, dirigido y orientado por la Dra. Regina Gschwind, con la asistencia de la estudiante de Ph.D. Edwige Buffar y supervisado por el Dr. Libor Makovicka. 4. Visita a la unidad de Radioterapia del Hospital de Montbéliard. - En 2007, por iniciativa del profesor Didier PAUL, la Universidad Nacional de Colombia y la Universidad Nacional de Ingeniería de Lima (Perú) se presentaron a la convocatoria, generada por la Maison de l Homme, del gobierno francés, del programa PREFALC, que permite que profesores franceses vengan a países de Latinoamérica a reforzar y apoyar las maestrías en Física Médica de Perú y Colombia. Mediante este programa, y con la coordinación de la profesora María Cristina Plazas, por parte de la Universidad Nacional de Colombia, y de los profesores César Picón, Eduardo Medina y Sandra Guzmán, de la Universidad Nacional de Ingeniería de Lima, contamos con la presencia de los siguientes profesores: - Libor Makovicka y Didier Paul (foto 2), quienes realizaron el siguiente plan de trabajo: ARTÍCULO PROYECTO PREFALC: GOBIERNO FRANCÉS UNI- VERSIDAD NACIONAL DE COLOMBIA (BOGOTÁ) UNIVERSIDAD NACIONAL DE INGENIERÍA (LIMA, PERÚ) Febrero de 2008 I. Durante este congreso, el profesor Paul tuvo la oportunidad, junto con la profesora María Cristina Plazas, de conocer a la nueva presidenta de la ALFIM, Simone Kodlulovich, con quien se desea desarrollar nuevas colaboraciones latinoamericanas entre Colombia y Francia en Física Médica. Actualmente, el profesor Didier Paul continúa apoyándonos desde Francia, con el profesor Akli Hammadi, desde el INSTN de la CEA, con el proyecto DOSEO, mediante el cual estamos tratando de que estudiantes de doctorado hagan intercambio con los laboratorios de Dosimetría, Radioprotección y Radiobiología. Otras actividades que hemos realizado gracias al apoyo de Francia han sido con el profesor Alejandro Ondo, quien, con el apoyo del programa ECOS-NORD, realizó su doctorado en Cotutelle, o bilateral, con el profesor Thierry Pourcher, entre la Universidad de Niza y la Universidad Nacional de Colombia, en Radiobiología, y tras vincularse a su regreso con la Universidad del Enero 2012 pág. 4

10 ARTÍCULO Rosario, hemos establecido una red de investigación entre el Grupo de Física Médica de la UNAL y el Grupo BIO-BIO de Radiobiología de la Universidad del Rosario, a la cual pertenece el profesor Ondo, vinculando a estudiantes de las dos universidades y realizando tesis de maestría en conjunto. Así mismo, la vinculación a la Universidad Nacional de los profesores Héctor Castro y Luis Agulles ha reforzado la formación en Física Médica con las relaciones que el profesor Castro tiene con Francia a través del profesor Christian Morel, de la Universidad de la Mediterranée, con la cual, gracias a las gestiones iniciales del profesor Didier Paul, se está tramitando un acuerdo entre esta universidad y la Universidad Nacional de Colombia para desarrollar conjuntamente doctorados bilaterales o en Cotutelle. En la foto 4 se observa el grupo de estudiantes que participaron en el taller en Física Médica realizado en mayo de 2010 con los profesores franceses Didier Paul y Christian Morel. También con el CERN, la Universidad de la Mediterranée está llevando a cabo el proyecto CERIMED, en cabeza del profesor Paul Lecoq, del cual somos participes y que también les permitirá a nuestros estudiantes mantener nuestras excelentes relaciones con Francia, país que tanto ha apoyado a la Física Médica en Colombia. Los países Latinoamericanos que deseen informarse sobre los diferentes programas que apoya el Gobierno Francés, pueden hacerlo directamente en las sedes de las embajadas en cada país. Professional Matters and Educational Issues at the World Congress, Beijing, May Howell Round Chair Professional Development Committee, Asia-Oceania Federation of Organizations for Medical Physics University of Waikato, Hamilton, New Zealand Presentations at the world Congress in Beijing ( wc2012.org/index.htm) in 2012 are arranged into 20 themes with several tracks in each. For those who are interested in professional matters and education issues for medical physicists, Theme 18: Professional and Educational Topics, will be of interest. There are five tracks of interest on these topics. Education & Training in Medical Engineering and Physics Career Development in Medical Engineering and Physics Accreditation and Certification Technology Enhanced Education Distance Training and Accreditation Education and training is a major topic for all medical physics professional organizations (MPPOs). There is a need to ensure that clinical medical physicists are properly trained and educated, especially for patient safety. Well-defined training programs exist in many developed countries, with the programs leading to physicists being certified as medical physicists qualified to practice independently in a particular specialty. Programs are being developed in less developed countries, such as in Asia, with the aid of the IAEA. As a result the requirements for certification of medical physicists are reasonably well agreed internationally, with the International Medical Physics Certification Board taking a leading role in defining standards. MPPOs that have a well defined education, training and accreditation system then need to have in place an effective continuing professional development (CPD) system to ensure that medical physicists are kept up to date with the latest technology and techniques and to ensure that Enero 2012 pág. 5

11 they keep gathering the skills that they need are their careers progress. Continuing professional development systems for medical physicists are now in operation in 26 countries. Usually complying with the requirements of the CPD system is necessary if a physicist s certification is to be renewed. The systems commonly require physicists to be credited with a certain number of points over a specified period (typically 5 years), with points being awarded for different activities and with limits on the number of points that can be obtained for each activity. This limit ensures that physicists do not undertake a narrow range of activities. The activities can be many different things. Some CPD systems only allow such activities as attending approved conferences and courses to be eligible to gain points. Others are much broader and allow points to be gained for publishing research, producing teaching materials, supervising or examining postgraduate student research, mentoring residents, gaining higher degrees and providing professional services such as serving on committees and working groups. While the requirements for training a medical physicists are relatively similar in most countries, the standards for CPD requirements are not so. As has already been pointed out, some systems allow only learning activities to be counted, while others have a much broader view. Also, while the amount of required activity is generally equivalent to one or two weeks per year, in one country the requirements can ARTÍCULO be met with only a few hours activity each year! In another, it appears that merely being employed full time as a physicist seems to be adequate to fulfill the CPD system s requirements. A broad international discussion is required to define an international standard for continuing professional development systems for medical physicists. Beyond that, the efficient provision of educational materials for CPD needs to be addressed. The need for physicists in all countries for CPD educational materials is very much the same. Physicists in most countries produce various materials in the form of books, videos, workshops etc. there seems to be a lot of unnecessary duplication of effort. The international medical physics community needs to cooperate together more effectively to produce, advertise, distribute and share that materials produced, for the benefit of the whole community. Certainly online materials can be easily shared. For example consider the materials that are available on the Biomedical Imaging and Intervention Journal website: and There will be an opportunity to discuss these issues within Theme 18 at the Congress, so please get there if you can. I. INTRODUCTION The project European Medical Imaging Technology e- Encyclopaedia for Lifelong Learning (EMITEL) developed the first e-encyclopaedia in the profession. This is a powerful reference and educational tool, free for use by a wide spectrum of specialists in Medical Physics and Engineering. The tool has proved very useful for many colleagues and currently its web site attracts more than 7000 users per month. The initial project partners included the core of the partners from our previous EU projects (EMERALD and EMIT [2]) - King s College London (Contractor and Coordinator) and King s College Hospital, University of Lund and Lund University Hospital, University of Florence, AM Studio Plovdiv and the International Organization for Medical Physics (IOMP). Later the number of project contributors grew significantly and now has more than 300 specialists from 36 countries. The colleagues form a large international Network of more than 250 specialists from 35 countries, coordinated by King s College London. In order to facilitate the international use of the EMITEL e-encyclopaedia (with articles in English), a Multilingual Translator (Dictionary) has been added to the web site, translating its 3400 terms (article titles) into 29 languages. II. EMITEL DEVELOPMENT The idea for EMITEL originated as a result of our 3 International Conferences (Budapest , ICTP, Trieste , ICTP Trieste ). These were the first such events dedicated to Education and Training in Medical Physics and Engineering. The senior specialists at these events discussed first the need of a professional Multilingual Translator (Dictionary). As a result our project EMIT ( developed in a CD-based dictionary including 7 European languages (English, French, German, Swedish, Italian, Spanish and Polish). Although the main objective of EMIT was the develo- Enero 2012 pág. 6

12 pment of e-learning materials in Magnetic Resonance and Ultrasound Imaging, its Dictionary was immediately very popular and attracted new languages (Thai, Portuguese and Hungarian). The success of EMIT (which received the inaugural EU Prize for Education the Leonardo da Vinci Award Maastricht, 2004) led to the new project EMITEL, which developed an explanatory article for each term and expanded the number of languages to 21. The new project started in 2006 and was completed by the end of The initial Medical Imaging areas of the EMITEL were X- ray Diagnostic Radiology, Nuclear Medicine; Magnetic Resonance Imaging and Ultrasound Imaging. However new areas were added later Radiotherapy, Radiation Protection and some General terms linked to Medical Physics. Special care was taken for covering the aspects of Medical Engineering related to Imaging and Radiotherapy. III. EMITEL DICTIONARY The first phases of EMITEL developed a Medical Physics Thesaurus with database of specific terms (4000+) covering the main areas of the e-encyclopaedia. These terms (of 1 to 3 or more words) were translated into many languages. The translation resulted in forming new Working Groups and soon became an inde-pendent part of the projects, working in parallel with all other Encyclopaedic Working Groups. The Dictionary is developed with synchronized tables of terms, what allows cross-translation of terms between each two of the languages. This is included in the web software of a Dictionary database served by a dedicated Search Engine. The Dictionary and the Encyclopaedia are coordinated by S Tabakov. The project partner AM Studio, Plovdiv developed the whole web database, the search engines, the web interface and the CMS. The inclusion of the Content Management System allowed the number of languages to in-crease to 29. Thus the Dictionary at present includes: English, Swedish, Italian, French, German, Portuguese, Spanish, Bulgarian, Chinese, Croatian, Czech, Estonian, Finnish, Greek, Hungarian, Japanese, Latvian, Lithuanian, Polish, Romanian, Russian, Slovenian, Bengal, Chinese, Iranian, Arabic, Malaysian, Thai and Turkish. IV. EMITEL ENCYCLOPAEDIA The Encyclopaedia is formed by explaining each term from the Dictionary with an explanatory article in English. The articles are aimed at MSc-level and above. Their volume varies from approx. 50 to 500 words. The model of the Encyclopaedia was built around a larger number of specific entries, rather than small number of multi-page articles. This model allows an easy and ARTÍCULO effective search. A total of 3400 articles have been developed with an overall volume of 2100 A4 pages (font 10, single spacing). Some synonyms (~ 5%) are covered with hyperlink to the main article. The majority of the articles are not internally hyperlinked to improve web site functionality and to allow easy future updates. However most of them include list of related articles and References and web links for further information. Some 2000 images, graphs, and diagrams are included in the articles to enhance the educational value of the reference material. The articles are grouped in 7 categories Physics of: X-ray Diagnostic Radiology, Nuclear Medicine; Radio-therapy; Magnetic Resonance Imaging; Ultrasound Imaging; Radiation Protection; General terms. Each article con-tent includes contribution from three specialists author, referee and group coordinator. The EMITEL web site combines the Dictionary and the Encyclopaedia the latter being served by two Search Engines the Multilingual one searching the Database of terms (in all languages) and another one searching inside the text of the articles (in English only). The latter allows significant increase of the potential of the e-encyclopaedia, including search for related terms, acronyms and synonyms. V. EMITEL CONTENT MANAGEMENT SYSTEM During 2010 the use of the EMITEL web site increased significantly (currently users per month). The number of contributors, forming a Network also increased and a need for updates was obvious. To allow this an additional web site was developed to handle the updates. This web Content Management System (CMS, also developed by AM Studio) underpins the main user web site and allows not only on-line editing of the materials, but also adding new terms/entries and including new languages. The CMS has several levels of access (all password-protected) with different rights. EMITEL CMS primary role covers: data entry, data update, data management, refereeing tools, interface control, terms and articles control tools, automated templating, editable content, preview function, scalable features sets, web standards management. The Administrative tools of the CMS include terms management, indexing, visibility control, authoring information, documents handling. 13 Enero 2012 pág. 8 ARTÍCULO Absolute calibration of high-dose rate sources of 192 Ir used in brachytherapy. High Dose Rate brachytherapy using 192 Ir is well accepted as an important treatment option thus it requires an accurate dosimetry standard. A dosimetry standard for the direct measurement of absolute dose to water for this particular type of source is currently not available. The AAPM TG-43 Report 1,2, constitutes the accepted protocol for dose to water determination based on an air kerma strength (S k,air ) measurement. The dose to water conversion is done via dose rate constant (Λ) and several relative correction factors accounting for scatter, attenuation, and anisotropy of the dose distribution among other effects 2. The Bhabha Atomic Research Center (BARC, India s primary standards laboratory) has produced a cavity ion chamber-based primary standard for 192 Ir HDR sources 3. More recently, the National Physics Laboratory (NPL, Britain s primary standards laboratory) established a primary standard based on a spherical graphite-walled cavity ionization chamber for direct measurements of the Reference Air Kerma Rate (RAKR) of HDR 192 Ir sources 4. However, much of the rest of the world still use various interpolation techniques. The interpolation techniques 5,6,7,8, were proposed by Goetsch et al. 9, Marechal et al. 11,11, and Ferreira et al. 12, the latter of whom was recommended by the IAEA/WHO Network of Secondary Standards Dosimetry Laboratories (SSDL network) 13. Those methods of generalizing the A wall values measured in one geometry to other geometries is incorrect by up to 2%, and cause a systematic errors of only 0.3% in 192 Ir calibration coefficients 14. In clinical practice, where the measurement of the absorbed dose to water is required, two potentially useful attempts have been reported. The first, using a water-based calorimeter, developed in reach uncertainties of 2.5% k=1 due to the source self-heating, which affects the reading. More recently papers 16,17 show an improvement of the uncertainty to 1.9% k=1. The second, based on Fricke dosimetry 18 get uncertainties of 3.43% k=1 due to the small dimensions of the irradiating device and also the experimental uncertainties involved. Recently an important improvement to the procedures was reported for Fricke dosimetry 19, which include: a newly designed irradiation vessel, a new reading device, rigorous temperature control Carlos E. de Almeida during irradiation and reading, and calculation of the correction factors in the same geometry, the G value measured with modern ionometric methods and formalisms 20 and the use of appropriate values for the molar extinction coefficient of the Fricke solution 21. As result, a significant reduction of the overall uncertainties of % was achieved However more work still needs to be done to reduce the uncertainties associated with the water calorimeter and the Fricke methods. After that an inter comparison involving the Fricke and the calorimeter is desirable. The results of both attempts have yet to be improved to allow their use as metrological references. Mota, PhD, DABR ML Ferreira, MSc, DABR CMC-NE, Concord, North Carolina USA A Radiocirurgia produz morte celular e colapso vascular para destruir ou eliminar tumores Como sabemos, a radioterapia convencional aplica altas doses de radiação fracionada durante várias semanas. Um grande volume do corpo que inclui o tecidos tumoral e tecidos normal é irradiado. Em muitos casos o tratamento é feito sem saber a localização exata e a extensão do tumor. Pontos de referência anatômicos são geralmente usados para demarcar a área/volume de planejamento e tratamento. Marcas na pele ou tatuagens são usadas para posicionar o paciente para tratamento. O fracionamento visa produzir mais danos ao tumor do que às células/estruturas normais. É a baixa tolerância destas estruturas à radiação que muitas vezes limita a dose de tratamento a um nível abaixo da dose terapêutica ótima. Se a dose podesse ser confinada mais precisamente ao volume de tratamento, a probabilidade sucesso de controle tumoral aumentaria ao mesmo tempo que o risco de dano nas células normais / estruturas normais é minimizado. O advento da IGRT (radioterapia guiada imagem) e da IMRT (radioterapia de intensidade modulada) trouxe um novo paradigma para a radioterapia. Utilizando estas técnicas, a dose tumor pode ser escalada para doses mais altas, poupando os tecidos normais com doses mais baixas. O radioterapeuta necessita agora identificar melhor o tumor. Varias modalidades de imagem estão disponíveis para planejamento e para guia dos feixes de radioterapia. Tecnicas de IGRT tem sido muito bem sucedidas em corrigir erros devido a descolamento inter-fração do tumor e de setup. No entanto, muitos tumores se movem durante o tratamento. Isso tem sido resolvido em muitos centros usando grandes margens para considerar estes movimentos. Mais uma vez, isso dificulta a redução da dose ao tecido normal e o escalonamento da dose tumor. A radiocirurgia estereotáxica usa 1 a 5 frações e uma técnica de guia por imagem muito precisa. O termo estereotaxia significa a capacidade de atingir o tumor com alta exatidão geométrica. A radiocirurgia estereotáxica tradicional tem sido utilizada para tratar tumores no cerebro por mais de 30 anos. Se utiliza altas doses de radiação precisamente dirigidas ao local do tumor, normalmente administradas em uma sessão de tratamento. Essa radiocirurgia tem como base uma estrutura metálica rígida (frame) que é fixada ao crânio do paciente, imobilizando a cabeça para o planejamento e tratamento. A dose tumor é administrada utilizando vários feixes ou arcos de modo a minimizar os danos ao tecido saudável ao redor do tumor. Em contraste com a radiocirurgia convencional com aceleradores lineares, a radiocirurgia robótica combina a tecnologia de guia por imagem e a robótica controlada por computador que permitem que ao sistema administrar altas doses de radiação sem o uso do frame metálico, mantendo a exatidão sub-milimétrica. Um dos sistemas de radiocirurgia estereotáxica mais conhecidos é a Leksell Gamma Knife. Ela continua sendo o método padrão-ouro de radiocirurgia cerebral. Cerca de pacientes são submetidos anualmente a cirurgia com Gamma Knife. O dispositivo de irradiação utiliza 201 fontes de Cobalto-60 localizadas em um anel em torno de um ponto central de tratamento e é capaz de exatidão submilimétrica. Tamanhos feixe de 4, 8, 14 e 18 mm são definidos por capacetes de tungstênio cambiáveis. Entreatnato, a técnica exige que o frame de cabeça seja fixado no crânio do paciente, mas não necessita sistema de guia por imagem em tempo real uma vez que o frame não permite o movimento durante o tratamento. Um novo sistema GammaKnife chamado Leksell Perfexion utiliza 192 fontes de Co-60 e 3 tamanhos de feixe (4, 8 e 16 mm) sem o uso de capacetes colimadores. 16 ARTÍCULO Até esta data, mais de pacientes foram tratados com Cyber Knife e mais de 226 sistemas instalados em todo o mundo. Veja abaixo uma lista de alguns dos tumores intracranianos (cabeça e cérebro) e de distúrbios cerebrais que podem ser tratadas com Cyber Knife (e com Gamma Knife). A capacidade de tratar qualquer local do corpo distingue a CyberKnife da Gamma Knife. TomoTherapy, Trilogy, Novalis e Elekta body frame são outros sistemas que partilham essa capacidade. A radiocirurgia extracraniana tem um enorme potencial como uma opção de tratamento padrão. Para lesões extracranianas, verdadeiro desafio de hoje é o tracking de tumores em movimento. A única solução real disponível hoje é chamada CyberKnife. O sistema CyberKnife é projetado para rastrear continuamente, detectar e corrigir o posicionamento dos feixes devido ao movimento do tumor e do paciente durante o tratamento. Seu braço robótico pode manobrar o linac e emitir feixes de radiação que podem entrar no corpo por até 1200 ângulos diferentes. Como sistema de radiocirurgia, o objetivo é maximizar a exposição do tumor, atingindo o mesmo com uma grande quantidade de feixes com doses pequenas. A focalização de tumores em movimento é feita usando sementes fiduciais de ouro implantadas perto ou no tumor, ou, no caso de algumas lesões pulmonares, usando a imagem do próprio tumor (xsight-lung tracking). Um modelo matemático que relaciona a posição do tumor com a expansão externa do tórax é usado para prever a posição do tumor em qualquer momento. O robô é capaz de administrar a dose em sincronia com o movimento do tumor. Não há necessidade de aumentar o volume de planejamento ou de sistemas de controle da respiração. Tamanhos de feixe de 5; 7,5; 10; 12,5; 15; 20; 25; 30; 35; 40; 50 e 60 mm são disponíveis usando colimadores cambiáveis fixos ou o sistema de colimação iris. A radiocirurgia robotica é uma alternativa não-invasiva à cirurgia para o tratamento de tumores cancerosos e não cancerosos em qualquer parte do corpo, incluindo cabeça, coluna, pulmão, próstata, fígado e pâncreas. O tratamento oferece uma nova esperança a muitos pacientes em todo o mundo, especialmente de lesões extra-cranianas. 17 ARTÍCULO Computed Tomography: Evolution and Trends over the World Madan M Rehani, PhD International Atomic Energy Agency, Vienna, Austria M.Rehani@iaea,org; madan.rehani@gmail.com Computed tomography (CT) has been rightly considered the greatest inventions for radiology. The global installed base of CT Scanners is forecast to cross 60 thousand units by 2015 according to Global Industry Analysts Inc. Reduction of scan times has been the major goal of all developments since the beginning of CT. However, the radiation dose became a consideration in early part of the last decade and has continued to occupy centre stage. Initial actions on radiation dose were based on observation on collective dose that indicated CT with 5% frequency was contributing 34% to collective dose from all radiological imaging on a global scale and in some industrialised countries the contribution was 60 to 70% [1, 2]. Subsequently it was realized that the dose per examination is itself increasing many fold [3]. This happened as the diagnostic information itself was increasing besides shorter scanning time making the technique more patient and physician friendly. In the last decade, further impetus came from introduction of hybrid imaging systems (PET/CT, SPECT/CT) into clinical domain. One of the important developments in recent times has been cardiac CT that has led the CT generation to dose war with mission to achieve sub-msv dose. From the era of 5 to 20 msv per CT examination of head, chest or abdomen, we are now in era of sub msv to 5 msv for cardiac CT [4]. There are indications that increasing trend in CT utilization will continue as CT angiography and hybrid imaging are likely to increase further. CT machine has been one of the very stable imaging equipments when it comes to performance over a period of time. Professionals involved in periodic QC testing can vouch for the fact that there have been minimal situations when any particular parameter has drifted to outside the tolerance limit. The main problem had been typical of any digital imaging system that of higher exposures not getting detected as images with low noise appear better resulting in most centres using higher image quality than necessary and thus higher patient dose. The exposure to patient to a level to lead to radiation injury were not observed or even foreseen. Cases of over exposures in brain perfusion studies in the last 3 years resulting in more than 350 patients having skin injuries is a total new development in CT imaging [5]. The accidental over exposure of a small child in Mad River stands apart a case requiring introduction of safeguards in CT machines and regulatory actions [5]. Besides the deterministic injuries, the carcinogenic risks are becoming more prominent with observations in recent years that many patients undergo repeated CT examinations sometimes more than 5 times in few years resulting in individual patient dose of more than 100 msv [6]. The IAEA surveys in large number of developing countries indicate that CT frequency is high in large part of the world and is increasing [7, 8]. The frequency of paediatric CT examinations in centres participating in IAEA survey in Africa, Asia and Eastern Europe was 20, 16 and 5 % respectively of all CT examinations. Participating CT facilities in 14 out of 26 countries showed >10% increase in number of CT examination in children from 2007 to There are number of occasions where patient doses in developing countries are higher than the diagnostic reference levels (DRLs). Non availability of imaging protocols and appropriateness guidelines have been observed resulting in unjustified examinations [9]. Probably there was never a time in the history after discovery of x-rays, when individual patient exposure ran into about 100 msv of effective dose or higher. Not all of these examinations are medically justified. There have been reports of 20-70% of the examinations lacking in use of appropriateness criteria. A number of reasons have been cited for this, such as defensive medicine, patient s wishes, financial interest, self-referral, health system factors, industry, use of media and lack of awareness. There has been lack of understanding of responsibility. While radiation safety regulations have considered deliverer of radiation (e.g. radiologist) to be responsible for ascertaining appropriateness, in actual practice typically a radiologist spends not even 5% of his time on ascertaining appropriateness and examination is mostly performed on prescription of a referring physician. While assigning responsibility in regulations and in publications of professional societies has been in effect, in actual practice recognition of responsibility has not taken its place. The dose per examination has been decreasing in low dose examination such as radiography, but has not been so in higher dose examinations such as CT, individual patient dose in some patients undergoing repeated CT examinations is reaching a three digit figure of effective dose (>100 msv), deterministic injuries to patients in CT have started recently and are continuing to occur in interventional procedures, there is gross lack of reporting of events of unintended exposures to patient, the stochastic risks to children are becoming significant in particular to breast and thyroid. Where we are going? With a number of reports having been published of patients having undergone more than 10 CT scans in few years, one can expect epidemiological studies to appear in coming years demonstrating excess cancer incidence among patients and correlation with dose. Tracking of patient exposure and cumulative assessment of patient dose over lifetime is going to become a reality New situations demand new actions and tools. The radiation exposure history of a patient is an important action in this respect. There are two aspects to it, namely information about previous radiological examinations and the radiation dose. No one can deny the value and need for information from previous imaging or non-imaging examinations and its contribution in strengthening the process of justification. Although it has been recommended in the past that patients provide information on previous examinations and that physicians consider this information, the emphasis had been on clinical aspects and this has not been incorporated into radiation safety standards and requirements. It is this incorporation into radiation regulations and requirements where the concept of patient exposure tracking has become important. Every problem needs to be tackled from multiple angles and radiation safety issues created by high numbers of examinations that lack appropriateness require action from a radiation safety angle [10]. At its most basic level, tracking takes the form of a record by the patient on a card somewhat similar to vaccination card. This approach has been accepted and is recommended by the FDA. While this is very simple, feasible across different hospitals and specialties, particularly in countries with lower socio-economic level, there are many limitations. Healthcare providers whenever they want or to bodies and authorities responsible for quality control and dosimetry have no access to the information, the method depends fully upon compliance and maintenance by the patient and may not have an impact on quality of care in terms of radiation dose reduction, of course without affecting diagnostic efficacy. A more comprehensive approach was initiated by the IAEA through its Smart Card project launched in Although the scope of the Smart Card project was comprehensive, it tended to give a narrow impression that the card will have patient dose data and, thus, the new name Smart Card/SmartRadTrack was adopted that lays emphasis on tracking. The purpose of Smart Card/SmartRadTrack was to achieve radiation exposure tracking of an individual patient. Until then, the mechanism to track all the exposures a patient has undergone could be performed only through a manual search of physical or electronic records in a hospital. If the patient had undergone examinations in different hospitals, there was no system based approach to track such examinations and it relied primarily on information provided by the patient. One of the important purposes of the Smart Card project was to emphasize that there needs to be a system in place. The adoption of electronic health records (EHR) in a number of countries in Europe has reached a level of >80% coverage for selected areas such as primary health care and geriatric care. An extension of EHR to include radiology and an extension of PACS to cover radiology equipment nationwide is happening in many countries. An international code of practice on dosimetry in diagnostic radiology is available. Dose information from important imaging modalities such as CT and to some extent in angiography is already provided by all vendors. Structured dose report implementation is about to reach a significant number of vendors and systems. Communication of structured dose reports between different vendors of PACS is already a reality. Communication of structured dose reports across hospital information systems (HIS) is technically possible, but still requires further involvement of IT domain expertise. All major vendors are now providing the facility to display all radiological examinations of an individual patient at the click of a button. The major problem of course lies in countries where there are no permanent identification numbers for its nationals. The future of radiation protection of patient in CT lies in continued technological developments towards the vision of sub-msv CT, making it almost impossible for operator to deliver higher doses to patients than reference levels through technology and using the strength of regulations, bringing the optimization to implementation at individual patient level, tracking of radiation exposure history of patient and achieving justification at individual patient level through use of decision support system for physicians. Lidia Vasconcellos de Sá Una variedad de procedimientos y los beneficios de los diagnósticos y terapéuticos, así como su uso como una herramienta para decidir el tratamiento más adecuado y su seguimiento, ha llevado a esta especialidad un gran reconocimiento en todo el mundo En las últimas dos décadas, los grandes avances en diagnóstico son la combinación de la biología molecular y la ciencia de imagen, dando lugar a nuevos campos de estudio. Estos incluyen todas las modalidades de imagen consagradas en el mundo o en el desarrollo como las exploraciones por emisión de fotón simples (Single Photon Emission Tomography- SPECT), la tomografía por emisión de positrones (Positron Emission Tomography-PET), la resonancia magnética funcional y dinámica (Magnetic Image Resonance -MRI), tomografía computarizada (Dynamic Computed Tomography-TAC), entre otros. En contraste con otras modalidades de imágenes como la resonancia magnética y TAC que principalmente ofrecen información sobre la estructura anatómica, las técnicas de imagen funcional de la medicina nuclear, como el PET, se puede obtener la imagen anatómica y también cuantificar las funciones bioquímicas y fisiológicas. Esta información es importante porque permite la detección de los cambios funcionales causados por la enfermedad antes que las anomalías estructurales sean evidentes. 20 capacidad de detectar las células cancerígenas basada en procesos moleculares y biológicos en el tejido tumoral, que es distinto del proceso en el tejido sano, lo que permite la evaluación de cualquier cambio. Estas técnicas normalmente no proporcionan imágenes con la misma resolución que la imagen de anatomía por resonancia magnética (MRI) o tomografía computarizada (TAC), pero mucho progreso se ha observado en el uso de técnicas en conjunto, es decir, la fusión de imágenes de TC y RM en combinación con SPECT y PET ofrecen, además de la localización anatómica precisa, los datos funcionales. La tomografía por emisión de positrones se puede decir que se originó en la década de 1950, cuando un tipo particular de sustancias radiactivas primero fue utilizado como un marcador. Desde entonces se supo que los fotones de alta energía producida por la aniquilación de los isótopos emisores de positrones se podrían utilizar para describir la distribución fisiológica del cuerpo humano en tres dimensiones. La técnica ha sido desarrollada a gran escala en los llamados países del primer mundo desde Se sabe que los Estados Unidos y en Alemania alrededor de 300 unidades ya están disponibles para una variedad de pruebas clínicas. El desarrollo y comercialización de radioisótopos que emiten positrones también fueron factores determinantes en la mejora de esta técnica. Las imágenes de PET se basaron inicialmente en el uso de oxígeno (15O), marcada en las moléculas de oxígeno (O2), monóxido de carbono (CO) y dióxido de carbono (CO2), por la producción limitada en ciclotrones disponibles en el momento. Sólo a mediados de 1970 otras máquinas se desarrollaron y en curso tiempo vinieron los isótopos conocido y utilizado en la investigación médica en la actualidad, como carbono (11C), nitrógeno (13N), oxígeno (15O), flúor (18F). La mayoría de los ensayos se realizan con Flúordeoxiglucosa (18FDG) en oncología (80%), especialmente en la detección y controle de tumores malignos, en la detección de la recurrencia y seguimiento de la respuesta a las terapias de tratamiento. En cardiología, se utiliza para comprobar el metabolismo del corazón en pacientes con enfermedad coronaria crónica y ayuda en la selección de candidatos a revascularización coronaria (bypass o angioplastia), y en el trasplante cardíaco. En neurología, se utiliza para clasificar los tumores cerebrales, los gliomas en la diferenciación de grado alto o bajo, y para distinguir entre los tumores recurrentes y los efectos del tratamiento, por ejemplo, necrosis por radiación. También se utiliza para la localización de las crisis epilépticas en personas con epilepsia intratable. ARTÍCULO Debido a la corta vida media de los emisores de positrones, cerca de dos horas a unos pocos segundos, los hospitales han tratado de dotarse de ciclotrones de baja energía, típicamente de 10 a 18MeV, para la fabricación, uso y distribución de estos radiofármacos. En la América Latina, la tecnología PET surgió a principios de 2000, en Argentina, México y Brasil. El uso de equipos SPECT asociados a sistemas de coincidencia, fue el inicio. Hoy tenemos más de 60 unidades PET/ CT instaladas en toda la región, pero sigue siendo un factor limitante para la expansión, los altos costos necesarios para adquirir un ciclotrón con lo sistema de marcación en la radiofarmacia y, además, lo propio equipo de imagen. Otros países, como Costa Rica, Cuba, Panamá y Guatemala tienen sus proyectos de implementación de ciclotrones y equipos PET / CT. Ecuador ya ha comenzado la producción de FDG e introdujo la técnica en la práctica clínica, Uruguay ha estado produciendo, además de 18F, 11C, y otros radiofármacos Sin embargo, algunos problemas se añaden a la utilización de esta tecnología. Debido a la utilización la energía más alta, 511keV, las dosis efectivas observadas para los trabajadores ha aumentado, el uso de blindajes en las áreas de manejo y en las salas de examen también requieren un cuidado especial. Además, la combinación de dos técnicas de imagen, TAC y PET, han aumentado la exposición del paciente, especialmente cuando se lleva a cabo controles periódicos para monitorear una enfermedad. La no introducción de niveles de referencia para los países o regiones también pode conducir a dosis mayores. El uso de imágenes PET y CT en la planificación de la radioterapia, donde la limitación en la resolución del equipo y, por lo tanto, los errores asociados con la planificación de tumores de pequeño tamaño, debe ser estimado. Los controles mínimos de calidad en PET/CT y la evaluación individual de los coeficientes de recuperación para corrección del valor SUV, también deben ser estudiados. En la última década, el PET ha sido la técnica de imagen funcional que más creció convirtiéndose en una herramienta de investigación y de práctica clínica. Nuevas geometrías han sido estudiadas, especialmente de las imágenes de órganos específicos, con el desarrollo de nuevos materiales para la detección de centelleo, las nuevas técnicas de reconstrucción de la imagen y nuevos radiofármacos más específicos. Los desafíos siguen presentes y deben ser vistos como oportunidades de mejora en la región, que se presenta como grandes áreas de actividades para los físicos médicos. Enero 2012 pág. 15
