The goal of the Children s Health Initiative (CHI) is to insure every eligible child in Marin County. To start the process of obtaining health insurance for your child please complete the online flyer and fax it back to the CHI office located at 3240 Kerner Blvd. in San Rafael. Our fax number is (415) As soon as we receive your fax we will contact you to set an appointment to meet with you, determine which insurance program your child is eligible for, and complete the necessary forms. 3 FREE AND LOW-COST HEALTH INSURANCE FOR CHILDREN (regardless of immigration status) Medi-Cal provides no-cost comprehensive health, dental and vision coverage for children and pregnant women. Eligibility is determined by family size, children s ages and family income. Healthy Families provides low-cost health, dental and vision coverage for children who are not eligible for no-cost Medi- Cal. Eligibility is determined by family size, children s ages and family income. HealthyKids provides affordable preventive and primary health, dental, vision and behavioral health coverage for eligible children that do not qualify for Medi-Cal or Healthy Families. AIM Access for Infants and Mothers provides low-cost health coverage for pregnant women. Kaiser Permanente Child Health Plan provides low-cost health care coverage for uninsured children who are not eligible for no-cost Medi-Cal or Healthy Families. Eligibility is based on family size and income.
