It s easy to participate Fill out application form if you re not already a Yolo FCU member. 2. Bring completed form to Save at School Day on Monday, September 12 and turn in to the bankers on the playground (room D-6 if it s raining) during recess between 10:00 and 10:30 a.m. 3. Keep saving your money and banking every Monday to earn awesome prizes! There is no minimum deposit required, so every penny saved counts!

3 Welcome, Montgomery Students, to the CU at School Savings Program! Thank you for your interest in Yolo Federal Credit Union s CU at School savings program. This packet of information has everything you need to sign your child up for this exciting and rewarding program! To open your youth savings account at Yolo Federal Credit Union, find the attached Membership Application and Agreement form and follow the following simple steps: 1. Completely fill out Primary Member Information section using your child s information. 2. Completely fill out Joint Owner 1/Trustee Information/Custodian section with your information. **Please note that on a minor account a parent or adult guardian must be a joint member on the account** 3. The UTMA/UGMA/Pay-On-Death/Trust Account and VISA Check Card/Access Line and PC Home Banking sections do not need to be filled out. Your child s account will automatically be enrolled for online banking and the telephone access line. 4. Read and understand the Membership Agreement and Important IRS Information sections. 5. The Trust Account and Uniform Gift/Transfer to Minor Account Agreement section does not need to be filled out. 6. Read and understand the Signatures section. The child does not need to sign this form; however, the parent or adult guardian member must sign the Joint Owner #1 signature line. To review a copy of the Agreements and Disclosures related to the account, ask your PTA representative. Please be sure to include a copy of a valid picture ID (Driver s License, State photo ID) for the adult signer on the account. If a picture ID is not included during account opening, the parent signer will need to present their picture ID to a Member Service Representative at any Yolo FCU branch by September 30, 2011 to complete the opening of the account. CU at School youth accounts are free of monthly service fees and initial membership deposit, so please disregard the minimum membership deposit information. If your child currently has their savings account at another financial institution and wishes to transfer that money to their new account at Yolo FCU, complete the final form in this packet and return it with your child s completed application. This form will serve as your request to close their current account and transfer the funds to their new Yolo FCU account without you having to do any additional work! To complete the process, have your child bring the completed application and a copy of the adult signer s photo ID to the first Save at School Day on Monday, September 12, 2010 on the playground during recess between 10:00 and 10:30 a.m. Regular Save at School Days will occur every Monday at the same times in the same place and we will continue to accept applications on those days. If your child is currently a member of Yolo FCU, the application form does not need to be filled out and they can begin their Save at School deposits on September 12. La cuenta de su hijo/a automáticamente será registrada para servicios bancarios electrónicos (online banking) y acceso telefónico. 4. Por favor lea y entienda el Contrato de Afiliación e información importante del IRS (Membership Agreement and Important IRS Information). 5. Lea y entienda la sección de Firma (Signature). El menor no tiene que firmar esta solicitud; sin embargo, el padre o guardián del menor debe firmar la línea del Copropietario #1 (Joint Owner #1). Para obtener una copia del Contrato de Afiliación, consulte a su representante del PTA. Por favor, asegúrese de incluir una copia de su licencia de conducir o identificación del estado, del padre o guardián que firme la solicitud. Si una copia de su ID no está incluida durante la apertura de la cuenta, el padre o guardián tendrá que presentar su identificación a un Representante de Servicios de Miembros en cualquier sucursal de Yolo FCU. Si su hijo actualmente tiene su cuenta de ahorros en otra institución financiera y desea transferir ese dinero a su cuenta nueva en Yolo FCU, complete la solicitud en la última página de este paquete y entrégalo con la Solicitud de Afiliación de su hijo. Este formulario servirá como su solicitud para cerrar su cuenta de ahorros y transferir los fondos a su cuenta nueva de Yolo FCU sin tener que realizar ningún trabajo adicional! Para completar el proceso, mande la solicitud completa y fotocopia de identificación del adulto con su hijo/a el día Lunes, 12 de Septiembre del 2011 en el campo de juegos de 10:00am a 10:30am. Los días de Ahorra en la Escuela serán todos los Lunes de 10:00am a 10:30am en la misma locación y continuaremos aceptar aplicaciones en esos días. Si su hijo es actualmente miembro de Yolo FCU, no necesita completar la solicitud y puede comenzar con sus depósitos en Ahorra en la Escuela el 12 de Septiembre.
