3 Roseland Charter School Roseland University Prep and Roseland Accelerated Middle School Athletics Eligibility and Discipline Policies Student athletic eligibility I. Students must have a 2.0 grade point average before joining an athletic team and must maintain a 2.0 grade point average during the season to remain on the team. II. A signed parent/guardian permission slip must be turned in for each athlete prior to first practice. III. The student and their parent or guardian must sign a responsibility and policies contract prior to first practice. IV. A physical form signed by a licensed physician declaring the student in good health to play the particular sport must be received prior to the first practice. V. Students with below a 2.0 grade average but above a 1.7 grade point average may practice if there is an opening on the team. a. This student may not travel to or play in games. b. This student has until the first grading period after joining the team to raise his/her grades to a 2.0 grade point average or above. c. If the grade is not raised by the first grading period then the student must leave the team. VI. No student may join an athletic team after it has played its first league game of the season. VII. No student may be removed from a team by a coach without first consulting the athletic director. VIII. The school administration reserves all right to remove any student athlete from a team at any time. IX. Tutorial may be mandatory. Please see coach or Athletic Director for more information. X. Students may not participate on an athletic team outside of school while on an athletic team at Roseland Charter School. Student athletic discipline policy I. Student athletes receiving a detention will serve the detention on the specified day and time even if this interferes with games or practices. Participation in games will be determined by each school site. II. Discipline committees may make decisions and issue consequences regarding a student athlete s eligibility. III. For each day of suspension a student athlete receives he/she will be suspended for one week from all team activities or possibly removed from the team (up to principal or principal s designee discretion).. I give my student permission to begin practice and participate in (name the sport). 2. I am aware of the risk inherent in interscholastic sports and that the charter school cannot guarantee that my student will not be injured despite the charter school s commitment to provide for students health and safety. 3. I give permission for my child to participate in interscholastic sports. 4. I understand that it is imperative that my child strictly adheres to all safety rules, regulations and instructions as well as rules and guidelines related to sportsmanship and citizenship. 5. I understand that athletics/school activities are not a required part of the high school curriculum. 6. I give my permission for school authorities to authorize medical care should the need arise. 7. I give permission for physician authorized medication to be administered to my child. All necessary forms for this medication have been completed and are on file in the school office. 8. I give permission for my child to be transported by bus or by staff driver. 9. I hereby release all liability from Roseland Charter School, Roseland University Prep, Roseland Accelerated Middle School, Roseland School District, and the State of California and all personnel employed by these agencies for property damage, injury, accident, illness or death occurring by transporting to school sponsored athletics, during school sponsored athletics or by reason of school sponsored athletics. Estudiantes deben tener un promedio de 2.0 antes de anotarse a un equipo atlético y deben mantener un promedio de 2.0 durante la temporada para continuar en el equipo. XII. Una hoja de permiso firmada por el padre/tutor debe ser entregada por cada atleta antes de la primera práctica. XIII. El estudiante y su padre o tutor deben firmar el contrato de responsabilidad y políticas antes de la primera práctica. XIV. Una forma del físico firmado por un medico certificado declarando que el estudiante esta en buena salud para jugar el deporte especificado debe ser recibido antes de la primera practica. XV. Estudiantes que tengan un promedio menor de un 2.0 pero arriba de 1.7 puede participar en las prácticas si hay lugar en el equipo. a. Este estudiante no puede viajar con el equipo ni jugar. b. Este estudiante tiene hasta el primero periodo de calificaciones después de haberse unido al equipo para subir su calificación a un 2.0 de promedio o mayor. c. Si la calificación no ha subido para el primer periodo de calificaciones, entonces el estudiante tendrá que dejar el equipo. XVI. Ningún estudiante puede unirse a un equipo atlético después de que este haya jugado su primer juego de liga de esa temporada. XVII. Ningún estudiante puede ser sacado de un equipo por el entrenador sin antes consultar con el director atlético. XVIII. El administrador escolar reserva todo derecho de sacar cualquier estudiante del equipo en cualquier momento. XIX. Tutoría es obligatorio. Favor de ver al entrenador o Director Atlético para más información. XX. Estudiantes no pueden participar en un equipo atlético fuera de la escuela mientras están en un equipo atlético en la Escuela Charter Roseland. Política Disciplinaria del Estudiante Atleta IV. Estudiantes Atletas que reciban una detención la servirán en el día y horario indicado aun si esta interfiere con partidos y practicas. Participación en los partidos se determinara en cada recinto escolar. V. Comités disciplinarios pueden tomar decisiones e emitir consecuencias en referencia a la elegibilidad del atleta. VI. Por cada día de suspensión de un estudiante atleta que el/ella reciba se le suspenderá una semana de toda actividad del equipo o posiblemente sacar del equipo (a discreción del director o asignado). Estoy consciente del riesgo que hay dentro de los deportes entre escolares y que la escuela charter no me puede garantizar que mi estudiante no se lastimara a pesar del compromiso de la escuela charter de proveer seguridad y saludo para los estudiantes. 12.
