1 Welcome to the CU at School Savings Program! Thank you for your interest in Yolo Federal Credit Union s CU at School savings program. This packet of information has everything you need to sign your child up for this exciting and rewarding program! To open your youth savings account at Yolo Federal Credit Union, find the attached Membership Application and Agreement form and follow the following simple steps: 1. Completely fill out Primary Member Information section using your child s information. 2. Completely fill out Joint Owner 1/Trustee Information/Custodian section with your information. **Please note that on a minor account a parent or adult guardian must be a joint member on the account** 3. The UTMA/UGMA/Pay-On-Death/Trust Account and VISA Check Card/Access Line and PC Home Banking sections do not need to be filled out. Your child s account will automatically be enrolled for online banking and the telephone access line. 4. Read and understand the Membership Agreement and Important IRS Information sections. 5. The Trust Account and Uniform Gift/Transfer to Minor Account Agreement section does not need to be filled out. 6. Read and understand the Signatures section. The child does not need to sign this form; however, the parent or adult guardian member must sign the Joint Owner #1 signature line. To review a copy of the Agreements and Disclosures related to the account, ask your PTA representative. Please be sure to include a copy of a valid picture ID (Driver s License, State photo ID) for the adult signer on the account. CU at School youth accounts are free of monthly service fees and initial membership deposit, so please disregard the minimum membership deposit information. If your child currently has their savings account at another financial institution and wishes to transfer that money to their new account at Yolo FCU, complete the Switch Form and return it with your child s completed application. This form will serve as your request to close their current account and transfer the funds to their new Yolo FCU account without you having to do any additional work! To complete the process, have your child bring the completed application and a copy of the adult signer s photo ID to the next Save at School Day at your school. If your child is currently a member of Yolo FCU, the application form does not need to be filled out and they can begin their Save at School deposits. Este paquete de información contiene todo lo que usted necesita para inscribir a su hijo/a a este excitante y gratificante programa! Para abrir su cuenta de ahorros para jóvenes en Yolo Federal Credit Union, busque la Solicitud y Contrato de Afiliación y siga los pasos siguientes: 1. Complete la sección del Afiliado Principal (Primary Member Information) utilizando la información de su hijo/a. 2. Por favor lea y entienda el Contrato de Afiliación e información importante del IRS (Membership Agreement and Important IRS Information). 5. Lea y entienda la sección de Firma (Signature). El menor no tiene que firmar esta solicitud; sin embargo, el padre o guardián del menor debe firmar la línea del Copropietario #1 (Joint Owner #1). Para obtener una copia del Contrato de Afiliación, consulte a su representante del PTA. Por favor, asegúrese de incluir una copia de su licencia de conducir o identificación del estado, del padre o guardián que firme la solicitud. CU al Escuela, las cuentas para jóvenes son libres de cuotas mensuales y el depósito inicial, no ponga atención al depósito mínimo por afiliación. Este formulario servirá como su solicitud para cerrar su cuenta de ahorros y transferir los fondos a su cuenta nueva de Yolo FCU sin tener que realizar ningún trabajo adicional! Para completar el proceso, mande la solicitud completa y fotocopia de identificación del adulto con su hijo/a en Ahorra en la Escuela a su escuela. Si su hijo es actualmente miembro de Yolo FCU, no necesita completar la solicitud y puede comenzar con sus depósitos en Ahorra en la Escuela.
