If yu are injured at wrk, ntify yur emplyer as sn as pssible. Yur emplyer will prvide yu with a claim frm. When yu ntify yur emplyer r insurer that yu have had a wrk-related injury, yur emplyer r insurer will arrange an initial appintment with a dctr in the MPN. Can I use my current physician? If yu have grup health cverage, yu may pre-designate yur physician prir t injury r cnditin, if: 1) yu have received care with the physician; and 2) the physician agrees t be yur primary treating physician. If yur physician des nt agree t participate in this capacity, yu will be required t seek care with a MPN prvider. Hw d I chse a prvider? After the first visit yu may cntinue t be treated by this dctr r yu may chse anther dctr frm the MPN. Yu may cntinue t chse dctrs within the MPN fr all f yur medical care fr this injury. If apprpriate, yu may chse a specialist r ask yur treating dctr fr a referral t a specialist. If the type f specialist yu need is nt available in the MPN, yu have the right t see a specialist utside f the MPN. If yu disagree with either the diagnsis r treatment prescribed by yur dctr, yu may ask fr a secnd pinin frm a dctr within the MPN. If yu want a secnd pinin, yu must cntact the MPN Liaisn and tell them yu want a secnd pinin. The Liaisn will make sure yu have a reginal area listing f MPN dctrs t chse frm. Then yu may chse a dctr frm the MPN and make an appintment within 60 days. Yu must tell the MPN Liaisn f yur appintment date. If yu d nt make an appintment within 60 days, yu will nt be allwed t have a secnd pinin with regard t this disputed diagnsis r treatment f this treating physician. If the secnd pinin dctr feels that yur injury is utside f the type f injury he r she nrmally treats, the dctr's ffice will ntify yur emplyer r insurer and yu will get a new list f MPN dctrs r specialists s yu can make anther selectin. After yu receive a secnd pinin, if yu still disagree with yur dctr, yu may ask fr a third pinin. If yu want a third pinin, yu must cntact the MPN Liaisn and tell them yu want a third pinin. The MPN will prvide yu with a reginal area listing f MPN prviders. Then yu may chse a dctr frm the list and make an appintment within 60 days. Yu must tell the MPN Liaisn f yur appintment date. If yu d nt make an appintment within 60 days, then yu will nt be allwed t have a third pinin with regard t this disputed diagnsis r treatment f this treating physician. If the third pinin dctr feels that yur injury is utside f the type f injury he r she nrmally treats, the dctr's ffice will ntify yur emplyer r insurer and yu will get a new list f MPN dctrs r specialists s yu can make anther selectin. If, after the third pinin yu still disagree with yur dctr, yu may ask fr an Independent Medical Review (IMR). Yur emplyer r MPN Liaisn will give yu infrmatin n requesting an Independent Medical Review and a frm at the time yu request a third pinin. An IMR will be dne by a physician selected by the Administrative Directr t cnduct an independent assessment f yur dispute. As lng as yur secnd pinin, third pinin r Independent Medical Reviewer agrees with the treating dctr, yu will need t cntinue t receive yur medical treatment with dctrs in the MPN netwrk. If the secnd pinin, third pinin r Independent Medical Reviewer des nt agree with yur treating dctr, yu will be allwed t receive that medical treatment frm a prvider either inside r utside the MPN. If yur current treating dctr is a member f the MPN, then yu may cntinue t treat with this dctr and yur treatment will be under MPN. Yur current dctr may be allwed t becme a member f MPN. If yur current treating dctr is nt r is nt allwed t becme a member f MPN, then yu may be sent t a MPN dctr fr treatment. If this ccurs, yu will be sent a letter and yur dctr will be ntified. Yur emplyer r insurer has a written Cntinuity f Care Plicy that may allw yu t cntinue treatment with yur dctr if yur dctr is n lnger actively participating in MPN. Yur emplyer will make a cpy f this plicy available t yu upn request. If yu are being treated fr a wrk-related injury in the MPN and yur dctr n lnger has a cntract with the MPN, yur dctr may be allwed t cntinue t treat yu if yur injury r illness meets ne f the fllwing cnditins: An acute cnditin.
