1 Form 3 - Public Disclosure Form This form shall be submitted by the CAB no less than thirty (30) working days prior to any onsite audit. Any changes to this information shall be submitted to the ASC within five (5) days of the change and not later than 10 days before the planned audit. If later, a new announcement is submitted and another 30 days rule will apply/ Este formulario deberá ser enviado por el Organismo Certificador (OC) en un período no menor a 30 días hábiles previo a cualquier auditoría*.
