Este documento explica lo que puede haber en el cuarto y para qué se usa. Cuando su niño ya está en la Unidad de Terapia Intensiva Cardíaca (CICU, siglas en inglés) después de la cirugía, puede haber mucho equipo médico, máquinas y ruidos. La situación puede ser abrumadora. Todos los equipos cumplen un rol en la recuperación de su niño que está médicamente delicado en este momento. Esta información explica qué equipos se usan y para qué sirven. Si tiene alguna pregunta no dude en hablar con la enfermera. Respirador/tubo endotraqueal Un respirador mecánico es una máquina para respirar que se puede usar para ayudar a su niño a respirar después de la cirugía. Con el respirador, el oxígeno entra en los pulmones y a la corriente sanguínea además de eliminar el dióxido de carbono del cuerpo. Si su niño necesita un respirador mecánico, estará intubado. La intubación consiste en un tubo que se coloca en la tráquea, ya sea por la nariz o la boca, para ayudarle a respirar. 2 Equipos de uso común después de una cirugía del corazón Tubos torácicos Se trata de un tubo flexible de drenaje para evitar la acumulación de sangre o líquidos corporales alrededor del corazón y los pulmones. El tubo se encuentra en la pared torácica, alrededor del corazón y los pulmones, y sale del cuerpo a través de la piel. Se mantiene en su lugar con puntadas. El tubo torácico está conectado a un dispositivo colector y el grupo médico de su niño lleva un control de la cantidad. En el sistema colector del tubo torácico se puede ver sangre y otros líquidos corporales. Sistema colector del tubo torácico Marcapasos A veces el ritmo cardíaco natural cambia después de una cirugía de corazón debido a la inflamación o a una lesión. El marcapasos es un dispositivo que funciona con baterías en el exterior del cuerpo. Está conectado al corazón de su niño con cables pequeños y aislados y estimula eléctricamente el músculo cardíaco para que regrese a su ritmo normal. Marcapasos Bombas/medicamentos Después de la cirugía puede que su niño tenga varias bombas intravenosas alrededor de la cama. Se usan para administrar medicamentos. También se les conoce como infusión o goteo. Estas infusiones nos permiten administrar medicamentos continuamente y por cierto período de tiempo evitando inyecciones cada pocas horas. Su niño tendrá una bomba para cada medicamento que necesite. Estos medicamentos ayudan a que el corazón funcione bien, controlan el dolor y proveen nutrición y líquidos. Bomba intravenosa 2 de 4

3 Equipos de uso común después de una cirugía del corazón Vía intravenosa central La vía intravenosa central es un tubo largo, blando y delgado que se coloca en una vena del cuello o la ingle y se conecta a una vena grande cerca del corazón. Este tipo de vía es especialmente útil porque suministra medicina sin irritar las venas menores y dura más tiempo que una vía intravenosa regular alejada del corazón. Las vías centrales se pueden usar para tomar muestras de sangre sin necesidad de pinchazos. Vía arterial Es un tubo plástico corto y delgado colocado a través de la piel en una arteria del brazo o la pierna. Permite que el grupo médico tome la presión de la sangre de su niño y se usa para muestras de sangre sin necesidad de pinchazos. Sonda de alimentación La sonda de alimentación es un tubo plástico delgado y blando que se coloca por la nariz (NG o nasogástrica) o por la boca (OG u orogástrica) hasta el estómago. Por estas sondas pasan alimentos y medicamentos directamente al estómago hasta que su niño pueda comer por la boca. También sirven para sacar aire y líquidos del estómago. Sonda Foley Es un tipo de sonda uretral, un tubo pequeño y blando para drenar la orina de la vejiga de su niño hacia una bolsa colectora. La saturación de oxígeno es la cantidad de oxígeno en la sangre. Tendrá dos adhesivos: uno en la frente para tomar la oxigenación del cerebro y el otro a un costado del cuerpo, entre la cadera y la última costilla, para medir la oxigenación del riñón. Bolsa colectora Foley Monitor NIRS Monitor Después de la cirugía su niño estará conectado a un monitor para que el grupo médico pueda llevar monitorear sus signos vitales (frecuencia cardíaca, presión arterial, saturación de oxígeno, temperatura y respiración). Los pequeños electrodos en el pecho tienen adhesivos que no causan dolor y son para monitorear el ritmo cardíaco y la respiración. Un oxímetro de pulso es un adhesivo que se pone alrededor de un dedo de la mano o del pie para tomar los niveles del oxígeno sanguíneo de su niño. Una sonda de temperatura es un cable con aislación que se adhiere a la piel con un parche para registrar y mostrar la temperatura. El monitor recibe y muestra toda esta información en un solo lugar. A menos que la presión de su niño se tome directamente a través de una vía arterial, su niño tendrá una manga en el brazo o la pierna para tomar la presión. Se usa cuando sea necesario, no toma la presión de la sangre constantemente. 5 Patient and Family Education Equipment to Expect After Heart Surgery In the Cardiac Intensive Care Unit (CICU) The equipment in your child s room after surgery can be overwhelming. This document explains what you might see and what it is used for. When your child is in the Cardiac Intensive Care Unit (CICU) after surgery, there may be a lot of equipment, machines and noise. This can be overwhelming. All of the equipment plays a role in helping your child during this medically fragile time. This information explains what you might see and what the equipment is used for. If you have any questions, feel free to ask your child s nurse. Ventilator/Endotracheal Tube A mechanical ventilator is a breathing machine that may be used to support your child s breathing after surgery. The ventilator will help to get oxygen into the lungs and blood stream, and help the body get rid of carbon dioxide. If your child needs a mechanical ventilator, they will be intubated. 6 Equipment to Expect After Heart Surgery Chest Tubes A chest tube is a flexible drainage tube that is used to prevent blood and body fluids from collecting around the heart and lungs. The tube lies in the chest wall, around the heart and lungs and exits the body through the skin. It is held in place with stitches. The chest tube is connected to a collection device and the amount of drainage is monitored by your child s health care team. Blood or other bodily fluids can be seen in the chest tube collection system. Chest Tube Collection System Pacemaker Sometimes after heart surgery the heart s natural rhythm is changed because of swelling or injury to the heart. The pacemaker is a battery-powered device that is outside the body. It is connected to your child s heart with small, insulated wires. The pacemaker electrically stimulates the heart muscle to get the heart rhythm back to normal. Pacemaker Pumps/Medication Your child may have several intravenous (IV) pumps near their bed after surgery. They are used for giving IV medicines. These are also known as drips or infusions. Instead of giving your child injections every few hours, IV infusions allow medicines to be given evenly over time. Your child will have a pump for every medicine they need. These medicines help your child s heart work, control your child s pain, and give your child nutrition and fluids. IV Pump 2 of 4

7 Equipment to Expect After Heart Surgery Central Line A central venous line (central line) is a long, soft, thin tube that is placed into a vein in the neck or groin that leads into a large vein near the heart. This type of line has special benefits because it can deliver medicines without irritating the smaller veins and lasts longer than IVs that are far away from the heart. Central lines can also be used to take blood samples without extra needle sticks. Arterial Line An arterial line is a small, short plastic tube that is placed through the skin into an artery of the arm or leg. An arterial line allows the health care team to monitor your child s blood pressure and take blood samples without extra needle sticks. Feeding Tubes A feeding tube is a small, soft, plastic tube placed through the nose (NG or nasogastric) or mouth (OG or orogastric) into the stomach. These tubes provide food and medicine directly to the stomach until your child can eat food by mouth. They also remove air and fluid from the stomach. Foley Catheter The Foley catheter is a type of urinary catheter, which is a small, soft tube placed in the bladder that drains your child s urine into a collection bag. This allows the healthcare team to monitor the amount of urine being produced by the kidneys, which can tell us how well the heart is working. Foley Collection Bag 3 of 4

8 Equipment to Expect After Heart Surgery To Learn More Cardiac Intensive Care Unit Your child s healthcare provider Monitors Near Infrared Spectroscopy (NIRS) Monitor The NIRS monitor measures oxygen saturation levels of your child s brain and kidneys. Oxygen saturation is the amount of oxygen in your child s blood. Your child will have 2 stickers to take these measurements. One will be placed on your child s forehead to measure oxygenation of the brain. The other will be placed in the flank area, between the hip and the last rib, to measure oxygenation of the kidney. Free Interpreter Services In the hospital, ask your child s nurse. From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. NIRS Monitor Monitor After surgery your child will be attached to a monitor so the healthcare team can check your child s vital signs (heart rate, blood pressure, oxygen saturation, temperature and breathing rate). Chest leads are small, painless stickers that will be placed on your child s chest to count your child s heart and breathing rate. A pulse oximeter, which is a sticker that goes around your child s fingers or toes, will be used to display your child s blood oxygen levels. A temperature probe, a coated wire adhered to your child s skin with a patch, can track your child s temperature. The monitor picks up and displays all of this information in one place. Unless your child s blood pressure is being directly monitored through an arterial line, your child will have a blood pressure cuff on their arm or leg. The cuff is used to take their blood pressure as needed by the healthcare team, but doesn t monitor blood pressure all the time.
