4 Parent/Guardian Consent: I consent to my child(ren) being registered in St. Ladislas Catholic Church in the Catechesis of the Good Shepherd. I commit to taking an active role in his/her faith development. I understand that as the primary catechist for my child, I assume responsibility for his/her attendance, behavior, and meeting the guidelines for this Ministry. For non-parishioners, I also understand that registration will not be complete until the reg. fee is paid. I give permission to use photographs taken of my child during the CGS sessions, for the Bulletin, Webpage or any other activities within the Parish. Firma del Padre/guardian Fecha de hoy General Sacrament Information: In addition to a Catholic Baptism, two full years of formal Religious Education is required by the diocese parish program prior to receiving First Reconciliation and First Communion, regardless of age. Child MUST be in 2nd grade to be able to receive First Holy Communion.
