Gracias por contactar a Familias Primero de Mecklenburg (Mecklenburg FIF) El cual es un capitulo local de Familias Primero de Carolina del Norte, que proporciona ayuda a individuos con discapacidades de desarrollo (DD) o Lesión Cerebral Traumática (TBI) y ayuda a sus familias también. Nuestra misión es de ayudarle, creer en sus sueños, lograr sus objetivos y devolver a otros que lo necesiten. Aquí adjunto una aplicación para completar y regresar a nuestra oficina. Sea por favor tan específico como sea posible con respecto a su necesidad. Una vez que nosotros recibimos su aplicación, nuestra Coordinadora de Recurso le contactará para solicitarle si es necesaria más información como documentación de IEP de la escuela o una evaluación medica. Ella también puede necesitar hablar con Usted personalmente o con la persona con la discapacidad. Para ser elegible a nuestros servicios Usted necesita: Tener unos ingresos de la casa que no exceda $65,000.00 al año libre de impuestos Que un miembro de la familia tenga una discapacidad de desarrollo y/o Lesión Cerebral Traumática. Vivir en el Condado de Mecklenburg. Mecklenburg FIF utiliza NC 122-C-3(12a) estatuto para definir discapacidad de desarrollo. Una copia de este estatuto puede ser suministrada para Usted si desea. Una vez que elegibilidad es determinada la Coordinadora de Recurso trabajará con usted para identificar claramente su necesidad y encontrar las fuentes para la ayuda. El objetivo del Personal de Mecklenburg FIF/Familias Primero es de ayudarlo a encontrar lo que Usted necesita dentro de la comunidad y le relaciona a Usted y al miembro de su familia a esos recursos. Esto crea una asociación que implica Usted, Mecklenburg FIF/Familias Primero y la comunidad. Appointment Reminders: May disclose health information to provide you with appointment reminders (voicemail messages, postcards or letters). PATIENT RIGHTS Access: You have the right to look at or get copies of your health information, with limited exceptions. You may request that we provide copies in a format other than photocopies. We will use the format you request unless we cannot practicably do so. You must make this request in writing to obtain access to your health information. You may obtain a form to request access from your care provider. We may charge you a reasonable cost-based fee for expenses such as copies and staff time.
