1 Enrollment Information Due to the closure of the district s centralized Enrollment Center, each individual school site will handle enrollment for students who reside in their attendance area. TK-K will be processed every Wednesday beginning February 7, 2018 through March 14, Elementary schools will begin processing 1 st 6 th grade enrollments and will continue to process TK k enrollments on April 2, Middle and high schools will begin accepting enrollments on April 3, Parents and/or guardians who do not know their student s school of residence, should check our School Site Locator at enter your street address and learn the elementary, middle or high school your child will attend. Enrollment packets and information will continue to be available on the district website. 2 Documents Required for Student Enrollment Photo I.D: The parent/legal guardian must present a Government issued photo identification in order to enroll a child. If you are not the parent/legal guardian but you are enrolling a minor child who lives in your home, you must complete a Caregiver s Authorization Affidavit. Birth Certificate: Child s legal birth certificate, Passport or Hospital record of birth. If the birth document is not available, you must complete an Affidavit of Proof of Age for Student Registration (D-87). Immunizations: Child s current immunization record (If you have multiple immunization records, please bring them all.) With the passage of Senate Bill 277 effective, January 1, 2016, we no longer accept personal beliefs or religious waivers. Address Verification: One item of address verification such as a recent gas, trash, water, electric bill, lease agreement, mortgage statement, or pay stub. If none of these items are available you will need to provide two pieces of mail with same name and address and complete a Residential Affidavit (D-118). The Residential Affidavit is subject to a home visit or verification of residency. Living Situation: If you and your child are living with someone, please bring that person with you. They will need to provide their current utility bill and photo identification. You will need to complete a Declaration of Parent and Child Residency form (D-86). Child Custody: Copies of legal custody agreement or court orders, if applicable. Oral Health Assessment: Education Code now requires your child to have a dental check-up by May 13 th in Transitional Kindergarten, Kindergarten, or 1 st grade whichever is your student's first year in public school. First Grade Physical: Copy of Physical Examination only for children entering 1 st grade is required. Program Participation If your child receives services for English Learners, Special Education, 504 Plan, Gifted and Talented Education (GATE) or has any special medical conditions please provide the CELDT testing document, IEP, doctor s note, or any other documentation that supports program participation. Previous School: Copy of any student data or student profile from previous school. Middle & High School: Transcript & Withdrawal/Checkout Grades Please fill out all documents completely using black ink.

3 Información sobre inscripciones para el ciclo escolar Debido al cierre del Centro de inscripciones de nuestro distrito, cada escuela procesará las inscripciones de nuevos alumnos que residan en su área de servicio. 4 Documentos necesarios para inscribir al alumno Identificación gubernamental con foto: El padre/tutor tiene que presentarla para poder inscribir al alumno. Si usted no es el padre/tutor legal pero va a inscribir al menor por que este vive con usted, tiene que llenar una afirmación de autorización de cuidado. Acta de nacimiento: acta de nacimiento- Si no tiene el acta de nacimiento, tendrá que llenar una afirmación de edad del alumno para inscripción. Vacunas: Cartilla de vacunación actual del alumno (Si tiene más de 1 registro tráigalos todos.) Debido a la aprobación de la Ley 277 del Senado, a partir del 1º de enero, 2016 ya no podremos aceptar exenciones por motivo de creencias personales o religiosas. Prueba de domicilio: cuenta reciente de gas, basura, agua, luz, contrato de renta, recibo de hipoteca o talón de cheque. Si no tiene lo anterior tendrá que mostrar 2 piezas de correo con su dirección y llenar una verificación domiciliaria (D-118); la verificación está sujeta a visita domiciliaria o verificación. Situación de vivienda: Si usted y su hijo viven con otra familia, traiga por favor a esa persona con usted. La persona deberá traer una cuenta reciente de los servicios de la casa y su ID con foto. Tendrán en ese caso que llenar la Declaración del domicilio de los padres (D-86). Patria potestad / custodia del alumno: Si es el caso debe presentar copia del dictamen pertinente. Examen dental: El Código educativo exige que el alumno sea revisado por un dentista a más tardar para el 13 de mayo del primer año que el alumno asista a una escuela pública: kínder de transición, kínder regular o 1 er grado. Examen físico de 1 er grado: Se requiere copia del examen físico para inscribir a alumnos de 1 er. grado. Evidencia de participación en algún programa: Si su hijo recibe servicios por ser aprendiz de inglés, educación especial, Plan 504, Educación para alumnos innatamente talentosos o GATE, o tiene algún problema médico presente usted los documentos pertinentes de acuerdo a su caso: información CELDT, IEP, justificante médico o cualquier evidencia pertinente. Escuela anterior: Copia de datos sobre el alumno o perfil del alumno de la escuela anterior. Alumnos de secundaria y preparatoria: constancia de estudios o transcript y calificaciones al darse de baja. Al llenar el papeleo, sea tan amable de llenar por completo y firmar los formularios utilizando tinta negra.
