1 Educación de pacientes Centro de maternidad y bebés Antes de que salga del hospital Mientras esté en el hospital, estará ocupada, conociendo a su bebé y recuperándose de su alumbramiento. Esta información es para ayudarle a preparar lo relativo a varios documentos importantes antes de que le den de alta y vuelva a su casa. Acta de nacimiento de su bebé Mientras esté en el UWMC después del nacimiento de su bebé, se le entregará un formulario a colores denominado "Birth Certificate Information Form" ("Formulario de información de acta de nacimiento"). Deberá llenarlo en ambos lados. Luego, entrégueselo a su enfermera o llévelo a Patient Data Services (Servicios de datos de pacientes) a la habitación BB-304, en el tercer piso (el principal) del centro médico. Si tiene preguntas, haga el favor de llamar al teléfono (206) Es mejor llenar el formulario antes de que salga del hospital. Si lo lleva a su casa, tiene que devolverlo a UWMC Patient Data Services dentro de los cinco días del nacimiento de su bebé. Carta de verificación de nacimiento Una carta de verificación de nacimiento del UWMC es prueba temporal del nacimiento de su bebé. Por ejemplo, el original de la carta de verificación de nacimiento lo aceptan tanto el Departamento de Servicios Sociales y de Salubridad (Department of Social and Health Services (DSHS) como el Seguro Social. Si obtiene asistencia del DSHS, deberá notificarle al Departamento del nacimiento de su bebé. Podrá obtener una Carta de Verificación de Nacimiento desde el momento del nacimiento de su bebé hasta que tenga dos meses de edad. Podrá recogerlo en los Servicios de Datos de Pacientes del (UWMC Patient Data Services), habitación BB-304, de las 9:00 a.m. a 3:45 p.m., de lunes a viernes. Si desea que se le envíe la carta por correo, marque la casilla de la página 2 del Formulario de información de acta de nacimiento. Dice: "Please send a Verification letter" (envíen por favor una carta de verificación). El UWMC puede dar sólo una carta de verificación de nacimiento a cada familia por el nacimiento de su bebé y no podrá reemplazar una carta perdida. Formulario de reconocimiento del padre Es posible que usted sea una madre soltera y desee que aparezca el apellido del padre de su bebé en la partida (el certificado) de nacimiento. Usted y el padre de su bebé tendrán que llenar un formulario especial denominado "Formulario de reconocimiento de paternidad" (Paternity Acknowledgement Form) dentro de los diez días siguientes al nacimiento del bebé. Pueden obtenerse folletos de información sobre este formulario en los Servicios de Datos de Pacientes. Este formulario deberán firmarlo usted y el padre de su bebé en presencia de un notario. Los dos padres deberán mostrarle a dicho notario una identificación con fotografía no caducada como, por ejemplo, una tarjeta (un carnet) de manejar (conducir) o un pasaporte. Para llenar el formulario y hacer una cita para obtener servicios gratuitos de notario, llame a los teléfonos (206) ó (206) Si entrega el formulario a Servicios de Datos de pacientes del UWMC dentro de los cinco días siguientes al nacimiento de su bebé, el hospital lo enviará al mismo tiempo que la partida de

Si llena el formulario más de una semana después del nacimiento de su bebé, deberá encargarse de llevarlo a la Oficina de Estadísticas Vitales (vea la dirección bajo "Partida de nacimiento") y pague 15 dólares. Tarjeta del seguro social Deberá obtener una tarjeta del seguro social para su bebé. Podrá hacer esto inmediatamente, yendo a cualquier oficina del seguro social. Hay una lista de las oficinas locales bajo "Federal Government"(Gobierno Federal), en la parte inicial de las páginas blancas del directorio telefónico. Tome en consideración los riesgos de rapto del bebé que correría si anunciara la llegada de su recién nacido. Si desea poner un anuncio del nacimiento, no debería incluir nunca el domicilio de la familia y limitarse sólo a sus apellidos.

Centro de maternidad y para bebés (206) Fotos y huella del pie del bebé Tomamos fotografías y huellas de los pies de todos los bebés al poco tiempo del nacimiento. Si desea comprar las fotografías de su bebé, hable con su enfermera. Puede ser que usted desee llevar ropas especiales para las fotografías del bebé. Puede obtener la huella del pie de su bebé en un certificado especial, sin costo alguno. Pregúntele también sobre esto a su enfermera. Ese certificado no es la partida oficial de nacimiento de su bebé. Asiento para el bebé en el automóvil Es ley del estado de Washington que su bebé esté siempre asegurado en un asiento aprobado para el automóvil cuando esté viajando en un vehículo. Puede alquilar un asiento de automóvil para bebé en el UWMC con un depósito de 20 dólares. Si lo devuelve en buen estado dentro de seis meses, se le reembolsarán catorce dólares. 5 Patient Education Before You Leave the Hospital You will be busy getting to know your baby as well as recovering from your birth while you are in the hospital. This information is to help you take care of some important papers before you are discharged and go home. Your Baby s Birth Certificate While you are at UWMC after your baby s birth, you will be given a colored form called Birth Certificate Information Form. You need to fill out both sides. Then give it to your nurse or take it to Patient Data Services in room BB-304, on the medical center s third (main) floor. If you have questions, please call (206) It is best to fill out the form before you leave the hospital. If you take it home, you must return it to UWMC s Patient Data Services within five days of your baby s birth. The hospital sends the information to the King County Department of Vital Records. If you do not fill out and return this form, you will have to pay for updates or corrections. Where to Get Copies Certified copies of your baby s birth certificate are available from the Vital Statistics Office, King County Administration Building, Room 214, 500 4th Ave., Seattle, WA Use a special form call Order Form for a Certified Copy of a Birth Certificate. UWMC will give it to you to order copies of your baby s legal birth certificate. Fill out the form and mail it to the Vital Statistics Office with a check for $13 for each copy. It may take two months for the copies to reach you. For more information, call the Vital Statistics Office at (206) When you receive your baby s birth certificate, put it in a safe place. It is an important legal document.

6 Page 2 Before You Leave the Hospital Birth Veritication Letter A Birth Verification Letter from UWMC is temporary proof of your baby s birth. For example, an original Birth Verification Letter is accepted by the Department of Social and Health Services (DSHS) and by Social Security. If you get DSHS assistance, you need to let DSHS know about your baby s birth. You can get a Birth Verification Letter from the time of your baby s birth until he or she is two months old. You may pick it up from UWMC Patient Data Services, Room BB-304, from 9:00 a.m. to 3:45 p.m., Monday through Friday. If you want the letter mailed to you, please check the box on page 2 of the Birth Certificate Information Form. It says, Please send a Verification letter. UWMC can give only one Birth Verification Letter to each family for their baby s birth and cannot replace a lost letter. Father s Acknowledgement Form You may be a single mother and want your baby s father s name on the birth certificate. You and the baby s father will need to fill out a special form called a Paternity Acknowledgement Form within 10 days of the baby s birth. Information booklets about this form are available from Patient Data Services. This form must be signed by both you and the baby s father in front of a notary. Both parents will need to show the notary unexpired legal photo identification, such as a current driver s license or a passport. To fill out the form and make an appointment for free notary services, call (206) or (206) If you turn in the form at UWMC s Patient Data Services within five days of your baby s birth, the hospital will send it, along with the birth certificate, to the Vital Statistics Office. If you fill out the form more than a week after your baby s birth, you must take it to the Vital Statistics Office (see address under Birth Certificate ) and pay $15. Social Security Card You will need to get a Social Security card for your baby. You can do this right away by going to any Social Security office. There is a list of local Social Security offices under Federal Government at the front of the white pages of the telephone book.

7 Page 3 Before You Leave the Hospital Take the Birth Verification Letter, your baby s hospital wristband, and the name card form your baby s hospital crib to the Social Security office. You will be given the number right away. If you want UWMC to get a Social Security number for your baby, please check the box on page 2 of the Birth Certificate Information Form. It says, Request Social Security Number. By asking UWMC to process this request, it will take 6 to 8 weeks before you get your baby s Social Security card. Medical Records UWMC keeps a record of the health care services given to you and your baby. You may ask to see and copy the record and correct the record. We will not show your medical record to others unless you tell us to, or the law tells or makes us. You may get information about how to get your medical record from UWMC Patient Data Services, room BB-340, by calling (206) from 9:00 a.m. to 3:45 p.m., Monday through Friday, or by writing to Patient Data Services, UWMC, Box , 1959 NE Pacific St., Seattle, WA Newspaper Birth Announcements UWMC does not give newspapers information about births. Consider the infant abduction risk you may be taking by publicizing the arrival of your newborn. If you decide to place a birth announcement, it should never include the family s home address and should be limited to last names only. Baby Photos and Footprint We take photos and footprints of every baby soon after birth. You may buy the photos of your baby. Ask your nurse. You may want to bring a special outfit to the hospital for your baby to wear in these photos. You can get a copy of your baby s footprint on a special certificate for no cost. Again, ask your nurse. This certificate is not the baby s official birth certificate.

8 Before You Leave the Hospital Questions? Your questions are important. Call us if you have questions or concerns. Maternity & Infant Center (206) Baby Car Seat It is the law in Washington State that your baby always be secured in an approved car seat when he or she is traveling in a car. You can rent a baby car seat from UWMC for a $20 deposit. If you return the car seat in good condition within six months, you will get $14 back. Your baby is safest in the middle of the back seat of the car. Until your baby weighs about 20 pounds, put the car seat on the seat so your baby faces the back of the car. Never put your baby in a seat where there is an airbag. The baby could be badly hurt when the airbag blows up. Prescriptions If you need medicines to take home, you can fill them at the UWMC outpatient pharmacy. It is near the information desk on the third (main) floor. You can also have them filled at your local pharmacy. Early Days at Home One of our nurses will call you at home to see how you and your baby are doing. She will answer any questions you have. Families whose baby is discharged from the hospital before 48 hours after birth will be offered a home visit by a nurse.
