Bogotá, Colombia Presbyopia is a condition developed between the 4th and 5th decade of life that diminishes the ability to focus near objects. For the past years physicians have developed different presbyopia correcting prodecures to improve near vision decreasing spectacle dependence. Central PresbyLASIK procedure uses PresbyMAX software developed by Schiwnd eye-tech-solution. The software reshapes the cornea using an excimer laser, making a multifocal biaspheric ablation profile. It steepens the centermost portion of the cornea, optimizing the central area for near vision and the peripheral area for distance vision, enhancing the depth of focus. It is a less invasive technique ideally for patients over 40 with a spherical equivalent between +5.00 to -5.00D and addition from 1.25 to 2.5D. Purpose: Evaluate results using visual art center PresbyLASIK in hyperopic patients to treat presbyopia. Methods: A retrospective longitudinal study was performed at the Clínica Barraquer, Bogotá - Colombia. Forty hypermetropic patients (80 eyes) underwent central Presbylasik treatment with PresbyMAX software developed by Schwind eye-tech-solutions, and were assessed 10 days (n=80, 100%) and 90 days postoperatively (n=50, 62.5%). The remaining 30 patients did not attend the respective follow-up. Average patient age was 53 5 years and mean preoperative manifest refractive spherical equivalent (MRSE) +1.45 0.97 diopters (D). Mean preop uncorrected distance visual acuity (UDVA) was 0.45 0.28 LogMAR. Preoperative uncorrected near visual acuity (UNVA) was investigated as reading speed and presented in LogRAD. The mean addition corrected was +2.38 0.34 D with a UNVA of >0.48 LogRAD. Data was analyzed with the statistic program IBM SPSS V. 21. Results: Ninety days after the refractive treatment the mean postoperative MRSE was -0.46 0.66 D (p<0.001) with an average UDVA of 0.22 0.16 LogMAR. The mean postoperative UNVA at the 10 days follow-up was 0.09 0.10 LogRAD and 0.16 0.16 LogRAD at the 90 days follow-up (p=0.005). Patients gained on average 1.41 3.82 lines in UDVA at the 10 days follow-up (57.5%), which increased to 2.44 3.78 lines at the 90-day follow-up (66.0%). The loss of lines of UDVA in the first postoperative control was 27.5%, decreasing to 22.0% at the last follow-up. When analyzing the change of corneal aberrations between preoperative and postoperative results, there was a significant change in positive spherical aberration toward negative spherical aberrations. A statistical significance modification in coma was also observed (p <001). There was not a significant change in the cylinder, distance best corrected visual acuity (CDVA) and corrected near visual acuity (NBCVA). Table 1. PresbyMAX is a therapeutic treatment option for correcting presbyopia and hyperopia. Similar results have been reported with statistical significance improving the sphere, spherical equivalent, DUCVA and UCNVA, not showing statistical significance with the cylinder. It enhances distance and near visual acuity, however with time there was a loss of 1 line in UCNVA in our study. The short follow-up was a limitation; in order to determine stability of the procedure long-term follow-up would be necessary.

Presbicia: Una opción de tratamiento quirúrgico Angela María Gutiérrez Marín. Escuela Superior de Oftalmología. Instituto Barraquer de América. Bogotá, Colombia Propósito: Evaluar resultados visuales usando técnica de presbylasik central en pacientes hipermétropes para tratamiento de presbicia. Métodos: Se realizó un estudio retrospectivo longitudinal en la Clínica Barraquer de América (Bogotá- Colombia) en cuarenta pacientes (80 ojos). Fueron tratados con presbylasik central con el software PresbyMAX desarrollado por Schwind eye-tech-solutions, se realizó seguimiento a los diez días (n=80, 100%) y a los noventa días del postoperatorio (n=50, 62.5%). Los 30 pacientes restantes no continuaron el seguimiento. La edad media fue 53±5 años y el equivalente esférico preoperatorio promedio fue (EE) +1.45±0.97 dioptrías (D). La agudeza visual lejana no corregida media decimal (DUCVA) fue 0.45±0.28. La adición media corregida fue 2.38±0.34 dioptrías con agudeza visual cercana no corregida preoperatoria (NUCVA) de >2M. Los datos fueron analizados con el programa estadístico IBM SPSS V.21. Resultados: A los noventa días del tratamiento refractivo el equivalente esférico postoperatorio promedio fue -0.46±0.66 dioptrías (p<0.001) con agudeza visual lejana no corregida media de 0.65±0.24 decimal. El 77.5% de los pacientes presentaron 0.75M de agudeza visual cercana no corregida a los diez días de seguimiento con disminución al 56% a los 90 días de seguimiento (p=0.005). Los pacientes ganaron un promedio de 1.41±3.82 líneas de distancia de agudeza visual lejana no corregida a los diez días de seguimiento lo cual aumentó a 2.44±3.78 líneas a los 90 días de seguimiento. Al analizar las aberraciones corneales entre el preoperatorio y el resultado postoperatorio, se produjo un cambio significativo en la aberración esférica positiva hacia aberraciones esféricas negativas. También se observó una modificación estadísticamente significativa en Coma y RMS (p<0.01). No se observó cambio significativo en el cilindro, agudeza visual lejana mejor corregida y agudeza visual cercana mejor corregida. Conclusiones: En conclusión, la técnica PresbyMax es una opción para la corrección de la presbicia en hipermetropía. La selección de los pacientes es esencial para obtener buenos resultados. Aunque hubo mejoría de la agudeza visual cercana y lejana no corregida, se requieren más estudios y evaluación de resultados a largo plazo.
