Diabetic kidney disease is a serious kidney disorder that may lead to kidney failure. Kidneys are needed to maintain good health. Their job is to filter waste products from the blood. In healthy kidneys, blood flows through millions of tiny blood vessels. The waste products squeeze through very small holes in the vessels and become part of the urine. Protein and red blood cells are too big to pass through the holes in the vessels and stay in the blood.
What is Diabetic Kidney Disease? What happens when your diabetes or blood pressure is uncontrolled? Uncontrolled diabetes can damage the waste filtering system. High blood sugar makes the kidneys work too hard. With all this extra work, the filtering system eventually becomes damaged. This causes the useful protein to be lost in the urine. Having small amounts of protein in the urine is a condition called microalbuminuria. This is the early stage of kidney disease. Doctors call this stage nephropathy (nef FROP uh THEE). When kidney disease goes untreated, it can eventually lead to kidney failure. Once that happens, the only options are kidney dialysis (filtering by a machine) or kidney transplant. The Good News... When kidney disease is diagnosed early, treatment can be very effective. Even better, kidney disease can be delayed or prevented by good control of blood sugar and blood pressure. There are several things that you and your doctor can do to keep kidney disease from getting worse. 1 Control your blood sugar. Diabetic kidney disease can be prevented by keeping blood sugar as close to normal as possible. Tight blood sugar control significantly reduces your risk of developing microalbuminuria by one third. Good glucose control also reduces by one half the risk that kidney disease will become serious. 2 Control your blood pressure. High blood pressure (even slightly above normal) can affect how fast kidney disease worsens. The American Diabetes Association recommends that, for people with diabetes, blood pressure should be kept at less than 130/80 mm Hg. What About Medication? If you are unable to get your blood pressure under control, your doctor may prescribe medications such as ACE inhibitors or angiotensin receptor blockers (ARB). A low-protein diet also can reduce protein loss in the urine and increase protein levels in the blood. However, you never should begin a low-protein diet without talking to your healthcare team. Your doctor will check your blood pressure, urine, blood, and other organs for complications of diabetes. Not everyone with diabetes develops kidney disease.
