1 Gestational Diabetes Mellitus Diabetes Mellitus Gestacional Qué es la diabetes? La diabetes se produce cuando el cuerpo no puede usar y almacenar de forma apropiada el azúcar que recibe a través de los alimentos que consume. Cuando esto ocurre, el nivel de azúcar en la sangre es más alto de lo normal. Qué es la insulina? La insulina es una hormona producida en el páncreas. Su función es la de llevar el azúcar del flujo sanguíneo a las células de su cuerpo. Sólo entonces las células de su cuerpo pueden usar el azúcar como energía. Qué es la diabetes mellitus gestacional? La diabetes mellitus gestacional (DMG) es un tipo de diabetes detectada durante el embarazo. Se detecta con mayor frecuencia después de la semana 24 de gestación. Se desconoce la causa exacta. Se piensa que está relacionada con ciertas hormonas que aumentan durante el embarazo. Estas hormonas funcionan contra la acción de la insulina. Cuando esto ocurre, los niveles de azúcar en la sangre llegan a niveles altos. Quién tiene riesgo? Entre los factores de riesgo se incluyen: Tener otros familiares con diabetes Ser de un origen hispano, asiático, afro-americano, indígena americano, o de las islas del Pacífico Ser obeso (tener sobrepeso) Haber tenido diabetes en embarazos anteriores Quedarse embarazado después de los 25 años de edad
2 Cómo se detecta? En algún momento entre la semana 24 a la 28 del embarazo se realizará una prueba de estimulación de la glucosa (PEG). Durante esta prueba, beberá un líquido semejante a una soda/gaseosa que contiene azúcar adicional. Una hora después de beber el líquido, se sacará para comprobar su nivel de azúcar. Cómo se trata? La clave para tratar la diabetes gestacional es mantener los niveles de azúcar en la sangre cerca del nivel normal. En la mayoría de las mujeres, esto se puede hacer con cambios en la dieta y el ejercicio. Un nutricionista hablará con Usted sobre los alimentos y las porciones que son adecuados para usted. Aprenderá a usar un medidor de glucosa en la sangre para que así pueda comprobar sus niveles de azúcar al menos todos los días. Si no se pueden alcanzar niveles normales de azúcar en la sangre de esta manera, quizás también necesite tomar medicamentos. Los medicamentos pueden ser pastillas o inyecciones de insulina. Si las inyecciones son necesarias, se le enseñará cómo dárselas Usted misma. La diabetes gestacional afectará a mi bebé? Si los niveles de azúcar permanecen dentro de los límites normales a través de los cambios en su dieta y el ejercicio, los riesgos para su bebé son muy bajos. Si el nivel de azúcar en la sangre de la madre permanece alto durante demasiado tiempo, el bebé puede tener los problemas indicados abajo: El nivel de azúcar en la sangre del bebé puede ser bajo al nacer, de manera que él o ella quizá necesite recibir azúcar de forma intravenosa para hacer que suba a un nivel normal. Su bebé puede tener problemas adicionales con la ictericia (color amarillo de la piel y los ojos). El bebé tiene más posibilidades de sufrir de sobrepeso o ser obeso como niño o adulto. Usted y su bebé tendrán pocos problemas si mantiene un buen control de los niveles de azúcar en la sangre y tiene visitas regulares con su proveedor de cuidados médicos.
3 Después del nacimiento de su bebé Tendrá una cita con su proveedor de cuidados médicos seis semanas después de dar a luz. En este momento, tendrá una prueba de sangre para evaluar la diabetes. Quizás le tomen el nivel de azúcar en la sangre en ayunas o realicen una prueba de dos horas para evaluar la tolerancia a la glucosa. En la mayoría de las mujeres, la diabetes desaparece después de nacer el bebé. Aunque eso ocurra, Usted debe informar a su proveedor local de cuidados médicos de que tuvo diabetes gestacional. 4 Gestational Diabetes What is diabetes? Diabetes occurs when the body cannot use and store the sugar you get from the food you eat the way it should. When this happens the sugar in your blood is higher than normal. What is insulin? Insulin is a hormone made by the pancreas. Its job is to carry the sugar in your bloodstream to the cells in your body. Only then can the cells in your body use the sugar for energy. What is gestational diabetes? Gestational diabetes is a type of diabetes found during pregnancy. It is most often seen after the 24 th week of gestation. The exact cause is unknown. It is thought that it has to do with certain hormones that increase during pregnancy. These hormones work against insulin action. When this happens, blood sugars become high. Who is at risk? Risk factors include: Having other family members with diabetes Having an ethnic background of Hispanic, Asian, African American, Native American or Pacific Islander Being obese (overweight) If you had diabetes when you were pregnant before Being pregnant after age 25
5 How is it found? At some time between the 24 th and 28 th week of pregnancy, a glucose challenge test (GCT) will be done. During this test you will drink a soda-like liquid which has extra sugar in it. One hour after drinking the liquid your blood will be drawn to check your blood sugar level. How is it treated? The key to treating gestational diabetes is to keep blood sugar levels close to normal. In most women this can be achieved by making changes in their diet and exercise. A nutritionist will talk with you about foods and portions that are right for you. You will learn to use a blood glucose meter so you can check your blood sugar levels at least daily. If normal blood sugar levels cannot be achieved in this way you may need to have medicine, too. The medicine may be pills or insulin shots. If insulin is needed, you will be taught how to give yourself a shot. Will gestational diabetes harm my baby? If blood sugars stay within the normal range by making changes in diet and exercise, the risks to the baby are very low. If a mother s blood sugar does remain high for too long, the baby may have the problems listed below. The baby s blood sugar may be low at birth so he or she may need intravenous (IV) sugar to make the blood sugar rise to normal. The baby may grow too large making a vaginal birth very hard. A cesarean birth may be needed. The baby may have extra problems with jaundice (yellowing of the skin or eyes). The baby has a greater chance of being overweight or obese as a child or an adult. With good control of your blood sugar levels and regular visits with your health care provider, you and your baby should have few problems.
6 After your baby is born Six weeks after your baby is born you will have a visit with your health care provider. At this time, expect to have your blood tested for diabetes. You may have a fasting blood sugar or a two-hour glucose tolerance test done. In most women, diabetes goes away after the baby is born. Even so you will want to let your local health care provider know you had gestational diabetes. You will be at a greater risk for getting type 2 diabetes as you age. Your health care provider will want to monitor you for this. Your risk for getting diabetes in the future can be decreased if you are able to lose any extra weight you gained while pregnant. Try to maintain good eating and exercise habits.
