3 In-Network Premier Plan (Culinary Network Dentist) Deductible: None Maximum Yearly Benefit: No Maximum Co-pays: Depending on the service Dental services must be from a Culinary Network dentist in the Nevada Dental Benefits (NDB) group. There are no co-pays for examinations, basic cleanings, x-rays and silver fillings. Orthodontics (braces) are covered for you and your eligible dependents. Dual Culinary Coverage: If you and your spouse both have Culinary dental benefits, you and your eligible dependents will not have any co-pays. Anesthesia for some extractions are covered. Orthodontics (braces): In-Network (Culinary Network Dentist) Only Benefit eligible dependents (under 19): For regular 24-month treatments*, the co-pay is $850 ($300 at the beginning of treatment and then $50 for 11 months). Benefit for you and your eligible dependents (age 19 and over): The co-pay is $3,100 ($900 at the beginning of treatment and then $50 every month for 36 months, plus a maximum of $400 at the end of active treatment). *For eligible dependents (under 19): Dental treatment depends on the diagnosis. If treatment will be longer than 24 months or requires additional appliances (a device that is used to move or position teeth during treatment), you may have to pay additional costs. If you and/or your dependent already have braces and are new to the plan, Culinary will not cover your treatment. You can find this sign hanging in all our In-Network Premier Dentists offices!

4 Out-of-Network Basic Plan (Non-Culinary Network Dentist) Deductible: None Maximum Yearly Benefit: $1,500 each for you and your eligible dependents Co-pay: Depending on the service Amount Payable (out-of-pocket): You are responsible for all charges beyond the Plan Schedule of Allowables. You may go to any licensed dentist of your choice in the United States. Nevada Dental Benefits will pay for charges that are covered up to the total listed in our Schedule of Allowables. You will pay the difference between what NDB pays and what the Non-Culinary Network dentist charges. Dual Culinary Coverage: If you and your Spouse both have Culinary dental benefits, you may still have co-pays and extra costs. Dental Emergencies (applies to both plans) If you have a dental emergency, call your dentist. If your dentist is unable to see you within 24 hours, contact NDB at NOT ALL DENTISTS WHO SAY CULINARY IS ACCEPTED HERE ARE IN OUR NETWORK. TO AVOID EXTRA COSTS, PLEASE CALL NDB AT TO MAKE SURE YOUR DENTIST IS IN OUR NETWORK.

No se cobran co-pagos por revisiones, limpiezas básicas, radiografías y empastes de plata. La ortodoncia (frenos) están cubiertos para usted y para sus dependientes elegibles. Doble Cobertura del Seguro de la Culinaria: Si tanto usted como su cónyuge tienen los beneficios del seguro de la Culinaria, ni usted ni sus dependientes elegibles tendrán que dar un co-pago. 6 Plan Básico Fuera de la Red (Dentistas Fuera de la Red de la Culinaria) Deducible: Ninguno Beneficio Máximo Anual: $1,500 por cada uno de usted y sus dependientes elegibles Co-pago: Depende del servicio Cantidad Pagadera (costos de su bolsillo): Usted se responsabiliza por cualquier costo que se exceda de la Lista de Permisibles del Plan. Puede ir con cualquier dentista con licencia que elija dentro de Estados Unidos. Nevada Dental Benefits pagará por los cargos que estén cubiertos hasta alcanzar el total que aparezca en nuestra Lista de Permisibles. Usted pagará la diferencia entre lo que pague NDB y lo que cobre el Dentista Fuera de la Red.
