1 Servicios de atención a domicilio Giving IV Gancyclovir with a BD or Curlin Pump / Spanish Cómo administrar Gancyclovir intravenoso con la bomba BD o Curlin Cuando recibe la fórmula y los materiales Antes de aceptar un pedido asegúrese de ue ha recibido las medicinas y los materiales correctos. Compare las medicinas y los materiales con la factura a nombre de su niño, la receta médica y las cantidades. Si todo está correcto, agregue su firma, la fecha en la factura y acepte el pedido. Es importante guardar la copia amarilla de la factura, puede servirle de referencia en el futuro. Una vez ue los materiales están en su casa no podemos aceptar devoluciones. Inspeccione cuidadosamente ue las jeringas no goteen, ue no tengan grietas y ue no hayan expirado. Lea la etiueta de la farmacia para saber cómo almacenar las medicinas y los materiales. 2 Cómo dar Gancyclovir intravenoso por la bomba BD o Curlin Materiales Área o bandeja Producto de Gel con Guantes no Toallitas de Paños de trabajo limpieza alcohol para esterilizados alcohol las manos O Bomba Curlin Sonda para la bomba Curlin Bomba BD Sonda Ultramicrobore con filtro de 0.22 micrones Contiene: Bata, mascarilla, toalla absorbente y bolsa con cierre para desechos. Euipo para preparación y administración de uimioterapia Euipo en caso de derrames Preparación de la superficie de trabajo y de la bomba 1. Limpie bien con desinfectante el área y deje ue se seue al aire. 2. Limpie bien con desinfectante el exterior de la bomba diariamente y deje secar. 3. Fíjese si la batería de la bomba tiene carga. 4. Lávese bien las manos con agua y jabón. 5. Reúna y coloue los materiales en un lugar cercano al área de trabajo. Preparación de la sonda 1. Lávese las manos o use gel desinfectante. 2. Abra 1 paño de papel tocando solo el borde y extiéndalo sobre el área de trabajo. 3. Coloue los materiales sobre el paño de papel y la bomba cerca de la orilla del paño. 4. Si va a dar más de una medicina, necesitará 3 etiuetas de identificación. 2 de 4

3 Cómo dar Gancyclovir intravenoso por la bomba BD o Curlin 5. Abra el euipo para preparar y administrar la uimioterapia. Abra la bolsa para desechos (con cierre hermético). Colóuelo adentro de la bolsa amarilla de desecho de uimioterapia (doble bolsa). Coloue el paño absorbente parcialmente dentro de la bolsa. 6. Lávese las manos o use gel desinfectante. Póngase el primer par de guantes. 7. Póngase la bata con la abertura hacia atrás y amárresela bien (la máscara es opcional). 8. Póngase el segundo par de guantes y jálelos para ue cubra los puños de las mangas de la bata. Saue la gancyclovir refrigerada. 9. Quítese el segundo par de guantes y póngase un par de guantes nuevos. 10. Bomba BD/sonda: Quite la tapa ue cubre el extremo de la jeringa con gancyclovir y también de la punta de la sonda más alejada del filtro. Deseche las tapas en la bolsa de desecho. Conecte la sonda intravenosa a la jeringa sobre una toalla absorbente, por si gotea el gancyclovir. Manteniendo la punta cubierta, coloue el final de la sonda adentro de la bolsas de desecho. Presione el émbolo para ue la medicina pase por la sonda. Coloue la jeringa en la bomba, manteniendo el final de la sonda adentro de la bolsas para desechos. Bomba Curlin/sonda: Quite la lengüeta amarilla y ovalada de la sonda. Quite la lengüeta de la nueva bolsa y tírela dentro de la bolsa con cierre. Retire de la sonda el portal con la punta. Con la punta, entre a la bolsa girando y presionando hacia adentro. Abra la puerta de la bomba levantado el pestillo. Inserte la lengüeta azul en el orificio marcado con la flecha azul. Coloue la sección blanda de la sonda encima de la bomba. Inserte la pieza amarilla en el orificio marcado con la flecha amarilla y cierre la puerta. Manteniendo la punta tapada, coloue el final de la sonda adentro de la bolsa de desecho. Purgue la sonda (vea el folleto Nutrición intravenosa ([IV] a través de una sonda central ). 11. Quítese los primeros guantes y deséchelos en la bolsa roja y anaranjada para residuos biológicos. 12. Si da más de 1 medicina, coloue las etiuetas de identificación en la sonda, cerca de la bomba, y donde se conecta con el catéter del niño. 13. Colóuese un segundo para de guantes limpios antes de enjuagar la línea. Que los guantes ueden por encima de los puños de la bata. 3 de 4

Llame a Atención en el Hogar para ordenar suministros de recambio, si es necesario. To Learn More Home Care Services (Cuidados en el Hogar) Número gratuito Siga el método SASH/SASASH para administrar la medicina. 15. Quítese los guantes y deséchelos en la bolsa de desecho. Guarde la bata para cuando tenga ue desconectar la bomba. 16. Antes de desconectar: Lávese las manos o use el gel desinfectante. Póngase un par de guantes. Póngase la bata con la abertura para atrás y amarrada arriba y abajo. Colóuese un segundo para de guantes cubriendo los puños de las mangas de la bata. 17. Deseche todos los suministros en una bolsa doble y ciérrela con un alambre de atadura (twist tie). Póngalo en la basura. Derrame de Gancyclovir 1. Si la medicina se derrama no la toue. Si esta en sobre una superficie, use el euipo para derrames siguiendo las instrucciones. 2. Si cae Ganciclovir en la ropa: Quítese la ropa y colóuela en una bolsa plástica. Bañe al niño, especialmente en el área afectada. Lave su ropa aparte y lávela una segunda vez (puede lavarla con otra ropa). 3. Pida al Servicio de atención a domicilio un euipo nuevo para derrames. Servicio gratuito de intérprete Estando en el hospital, solicítelo con la enfermera de su niño. Fuera del hospital utilice la línea gratuita de interpretación Dígale al intérprete la extensión o el nombre de la persona ue necesita. A uién llamar Para emergencias: para CUALQUIER emergencia llame al 911. Notifiue a Atención en el Hogar cómo se encuentra su niño después de ocuparse de la emergencia. Para urgencias fuera del horario de oficina llame a Atención en el Hogar, al o y presione 0 para dejar mensaje por cosas urgentes ue no puedan esperar hasta la mañana siguiente o si no sabe bien ué hacer. Una enfermera de Atención en el Hogar le regresará la llamada tan pronto como sea posible. Suministros: para ordenar suministros llame a Atención en el Hogar de 8 a.m. a 5 p.m. Llame al o Por favor, llame por lo menos 1 semana antes de ue se acaben para ue no se uede sin suministros. Siempre tenga suministros de más, como para 3 a 5 días y 1 dosis de medicina en caso de emergencia. 5 Home Care Services Giving IV Gancyclovir with a BD or Curlin Pump Receiving medicine and supplies When you receive a shipment, make sure you have the correct medicine and supplies. Check the medicine, supplies and invoice for your child s name, prescription and uantity. If all is correct, sign/date the invoice and accept the delivery. It is important to keep the yellow copy of your invoices for future reference. We are unable to take back any supplies once they are in your home. Carefully look at the syringes/iv bags, checking for leaks, cracks or discoloration and that they have not expired. See the pharmacy label for storage of medication and supplies. Flush syringes # saline syringes with ml of saline per syringe. # heparin ( unit/ml) syringes with ml of heparin per syringe. Chemo preparation and administration kit Spill kit Preparing the work surface and pump 1. Clean the work surface well with disinfectant and allow to air dry. 2. Wipe outside of the pump daily with disinfectant and let dry. 3. Check that the pump has working batteries. 4. Wash hands well with soap and water. 5. Gather supplies and place them in a clean spot near the work area. Setting up tubing 1. Wash or gel hands. 2. Open 1 drape, handling the edges only, and place on a cleaned surface. 3. Arrange the supplies on the drape. Place the pump next to the drape edge. 4. If giving more than 1 type of medicine, you will need 3 of therapy s ID labels. 2 of 4

7 Giving IV Gancyclovir via a BD or Curlin Pump 5. Open the chemo preparation and administration kit. Open the disposal bag with zip closure. Place inside the yellow chemo waste bag (double bagging). Place the absorbent pad partially inside the bags. 6. Wash or gel hands. Put on the first pair of gloves. 7. Put on the gown with the opening in the back and tie closed. (The mask is optional.) 8. Put on a second pair of gloves, pulling the gloves over the gown cuffs, and remove gancyclovir from refrigeration. 9. Remove the top gloves and put on another pair of clean gloves. 10. BD pump/tubing: Remove the tip covers from the end of the gancyclovir syringe and end of the tubing farthest from the filter. Discard the tip covers into the disposal bags. Connect the IV tubing to the syringe just above an absorbant towel to catch any drips of gancyclovir. Keeping the tip cover on, place the lower end of the tubing inside the disposal bags. Push the plunger, moving the medication through the tubing. Place the syringe into the pump, leaving the end of the tubing in the disposal bags. Curlin pump/tubing: Remove the oval yellow tab from the tubing. Pull the tab off the new IV bag and discard the tab into the disposal bags. Remove the spike port from the tubing. Twist and push the spike into the new IV bag. Open the door of the pump by lifting the latch. Insert the blue tab into the hole marked with a blue arrow. Lay the soft section of tubing over the top of the pump. Insert the yellow piece into the hole marked with a yellow arrow, and close the door. Keeping the tip cover on, place the lower end of the tubing inside the disposal bags. Prime the tubing (refer to the handout Giving Intravenous Nutrition Through a Central Line ). 11. Remove top gloves. Throw away in the disposal bags. Wash or gel hands. 12. If giving more than 1 medication, place ID labels on the tubing near the pump and where it connects to the child s catheter. 13. Put on a second pair of clean gloves before flushing the line. Pull the gloves over the gown cuffs. 14. Follow the SASH/SASASH method to give the medicine. 3 of 4

8 Giving IV Gancyclovir via a BD or Curlin Pump When in doubt, throw it out. If medicine or tubing is dropped or becomes contaminated, throw it out. Call Home Care Services for replacement supplies if needed. To Learn More Home Care Services or toll-free Free Interpreter Services In the hospital, ask your child s nurse. From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. 15. Remove your gloves and place in disposal bags. Save your gown for when you disconnect the pump. 16. Before disconnecting: Wash or gel hands. Put on 1 pair of gloves. Put on the saved gown with the opening in the back and both ties closed. Put on a second pair of gloves, pulling them over the gown cuffs. 17. Dispose of all supplies in a double bag and secure with a twist tie. Put in the trash. Gancyclovir spill 1. If a spill occurs, do not touch the medicine. If it is on a surface, use the spill kit and follow the directions on the kit. 2. If clothing is involved: Remove the clothing and place in a plastic bag. Shower the child and thoroughly wash the affected area. Wash clothing separately once, then wash again (can be with other items). 3. Order a new spill kit from Home Care Services. Who to call For emergencies: Call 911 for ALL emergencies. Notify Home Care Services about your child s status after the emergency situation is addressed. For urgent issues after business hours Call Home Care Services at or , and press 0 to reach our answering service for urgent issues that cannot wait until the morning or if you are unsure what to do. A Home Care Services nurse will return your call as soon as possible. For supplies: Contact Home Care Services between 8 a.m. and 5 p.m. to order supplies. Call or Please contact us at least 1 week ahead so that you do not run out. Always keep an extra 3 to 5 days of supplies, and 1 dose of medication on hand in case of emergency.
