3 Test ERA The ERA assay consists on: El ensayo ERA consiste en: A proprietary microarray that quantifies the expression of 238 genes involved in endometrial receptivity. Un microarray exclusivo que cuantifica la expresión de 238 genes implicados en receptividad endometrial. A molecular tool based on gene expression to determine if the endometrium is receptive. 4 Scientific Support EVIDENCIA CIENTÍFICA The development of this tool was published by Díaz-Gimeno et al., 2011 (Fertil Steril January; 95(1):50-60, 60.e1-15), and the accuracy and reproducibility of the ERA test was proven in Díaz- Gimeno et al., 2012 (Fertil Steril October; doi: /j.fertnstert ). Finally, its clinical applicability in patients with repeated implantation failures was demonstrated in Ruiz-Alonso et al., 2013 (Fertil Steril. 2013; submitted). A prospective, randomized study on the effectiveness of the ERA test in patients who have not received previous assisted reproduction treatments is presently underway. El desarrollo del array customizado ha sido publicado por Díaz- Gimeno et al, 2011 (Fertil Steril January; 95(1):50-60, 60.e1-15). Así mismo, su eficacia y consistencia se ha demostrado en el trabajo Díaz-Gimeno et al, 2012 (Fertil Steril October; doi: /j.fertnstert ). Finalmente su aplicabilidad clínica en pacientes con fallo de implantación se ha constatado en la publicación Ruiz-Alonso et al, 2013 (Fertil Steril. 2013; submitted). Actualmente se encuentra en proceso un estudio randomizado para evaluar su aplicabilidad en pacientes sin tratamientos previos de reproducción asistida. 7 Biopsy BIOPSIA Natural cycle The biopsy must be taken on the 7th day after the LH surge (LH+7). LH surge can be determined in urine or serum once the follicle has reached 15 mm under ultrasound monitoring. Considering the peak of LH as day 0, seven days must be counted from that day. Hormone replacement therapy Biopsy will be taken on day P+5, assuming that the day of administration of Progesterone is day P+0. Ciclo natural La biopsia debe ser tomada en el séptimo día tras el pico de LH (LH+7). El pico de LH se puede determinar en orina o sangre una vez que el folículo ha alcanzado 15mm según se puede monitorizar por ecografía. Considerando el pico de LH como día 0, se contarán siete días a partir de ese momento. Ciclo sustituido (HRT) La biopsia se tomará en el día P+5, asumiendo que el día de la administración de progesterona es día P+0. 8 Biopsy and Storage BIOPSIA Y CONSERVACIÓN Biopsy Prepare and label one of the supplied cryotubes. Inmediately after the biopsy introduce the sample in the cryotube and shake it vigorously for 10 seconds. Biopsia Prepare y etiquete uno de los criotubos suministrados. Inmediatamente tras la toma de la biopsia introduzca la muestra en el criotubo y agite por 10 segundos. Storage Keep the biopsy sample in a fridge (4-8ºC) for at least 4 hours. This preserved sample, in the original cryotube, can then be immediately shipped at room temperature. Alternatively, the samples can be kept in the fridge for one month or frozen at -20ºC (recommended) until the moment of shipment, at room temperature. Conservación Guarde la muestra en nevera (4-8ºC) durante al menos 4 horas. Esta muestra, ya preservada, puede ser enviada a temperatura ambiente inmediatamente. Alternativamente se puede guardar en la nevera hasta 1 mes o congelarla a -20ºC hasta el momento del envío, a temperatura ambiente. 9 Shipment ENVÍO Samples are sent at room temperature with the patient informed consent and a filled request form. Samples shipped at room temperature should reach us in 4 to 5 days. 11 RESULTS RESULTADOS Interpretation of results After the analysis, there are two possible results: Interpretación de los resultados Después del análisis, hay dos posibles resultados: Receptive (R): The gene expression profile is similar to that of a normal receptive endometrium with the diagnosis probability which is shown in the report. You are recommended to proceed with the embryo transfer in the same conditions, type of cycle and day, in which the endometrial biopsy has been performed. Receptivo (R): El perfil de expresión génica es similar al de un endometrio receptivo normal con la probabilidad de diagnóstico indicada en el informe. Se recomienda proceder con la transferencia de embriones en el mismo día y tipo de ciclo en el que se ha tomado esta biopsia de endometrio. Non Receptive (NR): The gene expression profile is similar to an endometrium out of the receptive phase with the probability which is shown in the report. We recommend taking a new endometrial biopsy to validate a displacement of the window of implantation and to guide the embryo transfer. No Receptivo (NR): El perfil de expresión génica es similar al de un endometrio fuera de la fase receptiva con la probabilidad de diagnóstico indicada en el informe. Se recomienda tomar una nueva biopsia endometrial para validar un posible desplazamiento de la ventana de implantación y guiarle en la transferencia embrionaria personalizada. 15 Clinical Outcome in ERA Non Receptive Patients RESULTADOS CLÍNICOS EN PACIENTES ERA NO RECEPTIVOS Dating the WOI of the Non Receptive samples The predictor and clustering analysis indicate that most Non Receptive samples classify as Pre-Receptive or Post-Receptive and therefore personalization of the WOI with a second ERA is recommended. Personalizando la ventana de implantación en muestras No Receptivas Tanto el predictor como el análisis del perfil transcriptómico indican que la mayoría de muestras No Receptivas clasifican como Pre-receptivas o Post-receptivas y por tanto se recomienda la personalización de la WOI con un segundo ERA.
