1 Resumen del proyecto COMPARACIÓN METROLÓGICA DE TERMÓMETROS CLÍNICOS INFRARROJOS Autor: Directores: Giannetti, Romano Sáenz Nuño, María Ana Entidad colaboradora: ICAI Universidad Pontificia Comillas RESUMEN DEL PROYECTO El objetivo de este proyecto es la construcción un prototipo verificador de termómetros clínicos infrarrojos para desarrollar un procedimiento de verificación rápido, sencillo y económico para usos en hospitales y centros de salud. Es importante hacer hincapié en el hecho que el prototipo no está diseñado para calibrar los termómetros sólo verificar su funcionamiento, proceso más económico dónde si no se desea demasiada exactitud sirve para mantener un control periódico de los instrumentos de medida. La necesidad de construir este aparato verificador surge de la legislación actual en la que no es necesaria la calibración de los instrumentos de uso clínico. Esto abarata costes para los fabricantes pero afecta, en mayor o menor medida, en la precisión de estos. El verificador consta de varias partes. La parte principal con la que va a interactuar el usuario es una placa de aluminio de pequeñas proporciones que, mediante un sistema de calentamiento basado en transistores de potencia, permite que se fije y mantenga la temperatura deseada en el centro de la placa. Esta temperatura se mide con un sensor principal que envía la información de la temperatura de la placa a un sistema de control que es el encargado de mantener el valor fijado por el usuario. Además, la placa de I

2 aluminio presenta cuatro sensores periféricos cuya función es la de marcar el régimen permanente para que el usuario pueda realizar los ensayos. El prototipo cuenta también con un sensor auxiliar, de las mismas prestaciones que el principal, por si el usuario desea comprobar la temperatura de la placa en otro punto. La temperatura de la placa, gracias al sistema de control, es constante e uniforme. Para una correcta utilización del prototipo han de cumplirse una serie de requisitos a la hora de realizar los ensayos de medida. Si alguno de estos pasos no se realizan de manera correcta puede provocar errores en la medida que repercutirán en la curva de verificación del instrumento. El punto de medida del termómetro ha de estar formando un ángulo de 90º con el target de aluminio y enfocado en la parte central que es donde se garantiza que la temperatura es la deseada. El instrumento ha de colocarse a una distancia, el fabricante proporciona un recubrimiento que permite el contacto con la superficie objetivo. Cualquier aumento de esa distancia afecta, de manera negativa, a la lectura del termómetro. Es recomendable mantener el termómetro en la misma posición durante la realización de los ensayos. Si no se posee un modo de fijar el instrumento, cuanto menos se varíe la distancia, el ángulo (tanto horizontal como vertical) del dispositivo con respecto a la placa o el punto en el que se realiza la medida, menor será el efecto provocado en la lectura del instrumento. Una vez tenidos en cuenta estos requisitos que hacen referencia a la colocación del instrumento hay conectar el prototipo a una fuente de alimentación via USB o directamente a la red. Con el prototipo funcionando se fija la temperatura deseada mediante el interfaz y se espera a que un LED se encienda, indicando que se ha alcanzado el régimen permanente en el centro de la placa. Debido a que la temperatura se alcanza más rápidamente calentando que enfriando, se recomienda que las temperaturas que se vayan a fijar sean de manera ascendente. Mientras el prototipo esté II

3 Resumen del proyecto encendido no se debe tocar la placa de aluminio pues, aunque no alcanza temperaturas superiores a 60ºC, un contacto prolongado puede provocar ciertas quemaduras leves. Tras la realización de diferentes ensayos en los que se ha caracterizado varios termómetros clínicos infrarrojos frontales mediante el estudio de su histéresis; el efecto de la distancia en la medida; la variación de la lectura con respecto al ángulo de incidencia; la repetibilidad o el efecto de las variaciones ambientales en la medida de temperatura, se ha observado la presencia de un valor de offset fijado por el fabricante. Este valor representa la diferencia térmica entre la temperatura corporal y la de la frente, que es menor. Hay que constatar que este offset, una vez tenido en cuenta no es un error en sí mismo por lo que su obtención es el primer paso en el proceso de verificación. Otro resultado que se obtiene al observa la curva de verificación de los termómetros es la gran similitud entre estas, lo que hace suponer que, por un lado, la electrónica interna y el sistema de control de los termómetros sean similares entre ellos y por el otro, que la diferencia significativa entre estos sea debida a la calidad del sistema de lentes. Esto explicaría la variación de precios entre un dispositivo y otro. Teniendo en cuenta estos resultados se recomienda que cada termómetro infrarrojo sea sometido al procedimiento de verificación, desarrollado e incluido junto al prototipo, al menos una vez. Una vez obtenida la curva de verificación y si no se desea repetir este proceso periódicamente, se puede hacer una comprobación mucho más rápida y sencilla mediante la realización del ensayo de repetibilidad. Si el resultado obtenido de este ensayo no es el deseado se recomienda la repetición del procedimiento completo. Si pese a todo esto el termómetro no responde dentro de los límites deseados se recomienda una calibración del mismo, en la que no se podrá usar el prototipo. III

It is important to emphasize the fact that the prototype is not designed to calibrate thermometers only verify operation, more economical process to keep a periodical control. The need to build this verificator arises from the current legislation, where is not necessary the calibration of instruments for clinical use. This reduces costs for manufacturers, but it affects, more or less, on the accuracy of these products. The verificator is made up of some parts. The main part to which the user will interact is a small aluminum plate. Through a heating system based on power transistors, allows to set and maintain the desired temperature in the center of the plate. This temperature is measured with a main sensor that sends thermal information to a control system which is in charge of maintaining the value of temperature set by the user. Furthermore, the aluminum plate has four peripherical sensors whose function is to mark the steady state of the plate so that the user can perform the tests. The prototype also has an auxiliary I

6 sensor of the same features as the principal, to check the temperature of the plate at another point. The plate temperature, thanks to the control system, is constant and uniform. For correct use of the prototype must be met a number of requirements when performing the measurement tests. If any of these steps are not done correctly can cause measurement errors that will impact the curve of instrument verification. The measuring point of the thermometer has to be an angle of 90 º with the target and focused in the central part that is where it ensures that the temperature is correct. The instrument is placed at a distance, the manufacturer provides a coating that allows the contact with the target surface. Any increase of distance affects negatively to the thermometer lecture. It is advisable to keep the thermometer in the same position during the course of the tests. If there is not a way of fixing the instrument, the user must try to not modify the distance, the angle (horizontal and vertical) of the device with respect to the plate or the point at which measurement is made, the smaller change can provoked a error on the instrument lecture. To start with the verification procedure, the prototype must be connected to a power source, via USB or similar. While the prototype is working, a temperature can be imposed trough the interface. It is necessary to wait for an LED comes on, indicating that it has reached steady state in the center of the plate. Because the temperature is reached faster heating than cooling the plate, it is recommended that the test temperatures will be set in ascending order. While the prototype is working, do not touch the aluminum plate, it does not reach temperatures above 60ºC but, prolonged contact may cause some minor burns. After performing various tests to characterize several Infrared clinical frontal thermometers by studying the hysteresis; the effect of distance; the variation of the angle of incidence; the repeatability or the effect of environmental variations in the II

7 temperature measurement, it has been observed the presence of an offset value set by the manufacturer. This value represents the temperature difference between body temperature and the forehead, which is smaller. It must conclude that this offset, is not an error in itself so its calculation is the first step in the verification process. Another result obtained by observing the verification curve is the great similarity between the thermometers, which suggests that, on one hand, the internal electronics and control system of the thermometers are similar between them and on the other, that the significant difference between these is due to the quality of the optical system. This would explain the variation in prices between one device and another. It is recommended that each infrared thermometer is subjected to the verification procedure, developed and included with the prototype, at least once. Once the verification curve is traced and if the main process is not needed, a faster and easier test can be done. A repeatability test is recommended in this case. If the result of this test is not the intended, it is recommended the repetition of the entire procedure. If despite all this, the thermometer does not respond within the desired limits, it is recommended the calibration of the instrument. The prototype is not designed to do this procedure. III
