Buenos Aires, Argentina Author Disclosure Information: Kantor Pablo: Boston Scientific, Modest, Consultant/Advisory Board Lagioia A, Coria D, Arellano J, Fernandez J, Monacci F, Palacios A: None 1- INTRODUCTION: COURAGE trial showed that as an initial management strategy in selected patients with stable coronary artery disease, PCI did not reduce the risk of death or myocardial infarction when added to optimal medical therapy (OMT). 2- HYPOTHESIS: We assessed the hypothesis that real-world treatment of COURAGE-like patients is far from the statements of that trial. 3- METHODS: From November 2009 to May 2010 we evaluated prospectively and consecutively all patients admitted to the catheterization lab in four hospitals in Buenos Aires with an indication of a coronary angiography because of stable coronary syndrome. After performing the angiography we selected the patients using the same inclusion and exclusion criteria of COURAGE trial. We assessed the medical treatment and contacted their cardiologist in order to let them decide to perform a PCI or continue medical treatment alone. 4- RESULTS: 274 patients with the same inclusion and exclusion criteria of COURAGE trial were included in our analysis. Introducción: El trabajo Courage muestra una estrategia de manejo de un selecto grupo de ptes. Con síndrome coronario estable. Donde la ATC no reduce el riesgo de Muerte, IAM. Con tratamiento Medico Optimo (OMT) Hipótesis: En el mundo real, las premisas del Courage se cumplen? Metodo: Noviembre 2009 a Mayo 2010, evaluamos prospectivamente y consecutivamente, todos los ptes. admitidos en los laboratorios de cateterismo de cuatro centros de Bs.As. Con indicación de coronariografia por Síndrome Coronario Estable. Luego de realizar la coronariografia, seleccionamos los ptes. Con los mismos criterios de inclusión y exclusión COURAGE TRIAL. Controlamos el tratamiento y luego del diagnostico angiografico, nos contactamos telefónicamente con el Médico de Cabecera para que decida ATC o tratamiento médico ACC 2011 J Am Coll,2011.57:1048

Conclusiones: En ptes con enf de múltiples vasos a los que se le realiza ATC con DES, la medición rutinaria de FFR. Comparado a La ATC guiada con angio solamente. Resulta en una significativa reducción de la Mortalidad, Infarto de miocardio a 2 años. Pudiendo diferenciar lesiones isquémicas revascularizadas Y tratamiento medico a las no isquémicas.
Revascularización de Urgencia Conclusión: 1-En pacientes con Angina Crónica Estable con al menos una lesión coronaria con FFR 0,80 El TMO tiene mayor tasa de Revascularización de Urgencia 2- pacientes con Angina Crónica Estable sin evidencia de Isquemia por FFR esta asociado a una muy buena evolución Alguna Revascularización

Optical Coherence Tomography OCT
Evaluación funcional en la sala de hemodinamia 71% de los pacientes llegan a la sala de hemodinamia sin estudio funcional. De los que lo traen. Un 40% no concuerdan con el hallazgo de la anatomía. Cuando enfrentamos un paciente con enfermedad de múltiples vasos, que cumple con uno de las dos premisas precedentes, se presenta la disyuntiva de tratar todas las lesiones con múltiples stent. O guiar el procedimiento con FFR, IVUS y tratar solamente las lesiones significativas e isquémicas.
