PRETERM LABOR What is premature/preterm labor? Preterm labor means having contractions before the 37th week of pregnancy. Preterm labor, if not attended to can cause your baby to be born too soon. How can premature birth affect my baby? Preterm birth causes the most problems for newborns today. Babies born premature can have problems eating, breathing, or keeping their body temperature. They may have damage to their brains, or problems with organs of the body (like the heart) because of incomplete development. What causes preterm labor? The cause is not known for sure. Factors that may cause it or increase your risk include: Carrying more than one baby (twins, etc.) History of preterm delivery or preterm labor Weakened cervix Kidney and urinary tract infections or bacterial vaginosis Vaginal bleeding Cervical dilation Too much or too little amniotic fluid (fluid that surrounds the baby in the uterus) Abnormal shaped uterus Too much physical or mental stress Two (2) or more abortions or miscarriages Trauma to your abdomen or uterus No prenatal checkup with caregiver If you smoke, drink, or use street drugs during pregnancy What are the signs and symptoms of preterm labor? Knowing the risk factors, as well as signs and symptoms of preterm labor may prevent you from having your baby too soon. Watch for these signs of preterm labor even if you are on bedrest or taking medicine: Contractions - You may feel tightening and relaxing of the uterus every 15 minutes or more often. You may or may not feel pain. Cramping - This may feel like menstrual cramps that come and go, or they may be constant. Dull Backache or Low Back Pain - This backache is in the lower back and may move to your sides or front. This pain may also be constant or may come and go. Pressure - You may feel heaviness in your pelvic area. Vaginal discharge that becomes watery, mucousy, brown-tinged, or pink. Vaginal spotting or bleeding. What should I do if I have signs and symptoms? If you have any of the above signs or symptoms, check yourself for contractions and time them.
1. Lie down on your left side. Do not lie flat on your back. 2. Place your fingers on your uterus. 3. Anytime your uterus feels tight (firm/hard) you are having a contraction. 4. If you feel contractions every 15 minutes or more often, call your doctor and go to the hospital. Remember, preterm labor must be treated so that your baby is able to continue growing inside the uterus. How is preterm labor treated? At the hospital, a nurse will place a monitor on your abdomen to check for contractions and your baby's heartbeat. The doctor may check your cervix to see if it has started to open. If you have few or no contractions and your cervix is closed, you may be sent home to rest. Bedrest may be recommended by your doctor. Follow your doctor s advice as to when you can increase your activity level. Medications For Preterm Labor Terbutaline: A shot or pill that may stop contractions. Side Effects: Feeling warm, shaky, and nervous. You may also have headaches, nausea, or vomiting. If you feel chest pain or have trouble breathing, see your doctor right away. Magnesium Sulfate: Given in a vein. Side Effects: Feeling warm, nausea, constipation, drowsiness, and tiredness. If you are given this medicine, your blood will be drawn every 6 hours. Antibiotics: May be given to help treat or prevent an infection. Watch for signs of preterm labor even while you are on bed rest or taking medicine. Remember, preventing or stopping preterm labor helps to give your baby a chance to be born healthy. LA LABOR DE PARTO PREMATURO Qué es labor del parto prematuro? La labor de parto prematuro ocurre cuando las contracciones se presentan antes de las 37 semanas de gestación. Si la labor de parto prematuro no se trata, puede causar el nacimiento prematuro de su bebé. Cómo afecta a mi bebé el nacimiento prematuro? El nacimiento prematuro es el problema más importante que afecta a los recién nacidos hoy. Los bebés que nacen prematuros pueden tener problemas para alimentarse, respirar o mantener la temperatura corporal. Pueden sufrir daño cerebral o problemas en órganos de su cuerpo (por ejemplo el corazón) debido al desarrollo incompleto de los mismos. Cuáles son las causas de la labor de parto prematuro? No se sabe con seguridad cual es la causa. Los factores que pueden causarlo o aumentar su riesgo incluyen: Embarazo múltiple (mellizos, etc.) Antecedentes de partos prematuros o labor de parto prematuro El cuello del útero debilitado Infecciones renales y del tracto urinario o vaginosis bacteriana Sangrado Vaginal Dilatación cervical Demasiado o escaso líquido amniótico (el líquido que envuelve al bebé dentro del útero) Forma anormal del útero Demasiado estrés físico o emocional Dos (2) o más abortos electivos o espontáneos Trauma o lesión del abdomen o útero Falta de atención médica prenatal con un profesional de la salud Si fuma, bebe o usa drogas ilegales durante el embarazo Cuáles son las señales y los síntomas de la labor del parto prematuro? Conocer los factores de riesgo, como así también las señales de la labor de parto prematuro puede ayudarle a prevenir que su bebé nazca prematuro. Manténgase atenta a lo siguiente: Contracciones Puede sentir endurecimiento y relajación frecuentes del útero cada 15 minutos o con más frecuencia. Puede sentir o no dolor. Calambres Estos pueden sentirse como los calambres menstruales, intermitentes o pueden ser constantes. Dolor difuso de espalda Dolor de la parte inferior de la espalda - Este dolor de espalda se localiza en la parte inferior de la misma y puede irradiarse hacia los lados o hacia el frente. Este dolor puede ser también constante o intermitente. Presión Puede tener la sensación de peso en el área pélvica.
Desecho o secreción vaginal acuoso, mucoso, color café o rosado. Sangrado vaginal esporádico o hemorragia Qué debo hacer si tengo estas señales o síntomas? Si tiene alguna de las señales o síntomas anteriormente descriptos, deberá controlarse las contracciones y el tiempo que le duran cuando las siente. Para auto monitorizarse: 1. Acuéstese sobre su lado izquierdo. No se acueste horizontalmente sobre la espalda. 2. Coloque sus dedos sobre su útero. 3. En el momento que sienta endurecimiento del útero (tenso/ rígido) esta teniendo una contracción. 4. Si siente contracciones cada 15 minutos o con más frecuencia, llame a su doctor y vaya al hospital. Recuerde, aunque puede sentirse cansada de estar embarazada, la labor de parto pre-término debe tratarse para que su bebé pueda continuar el crecimiento dentro del útero. Cómo se trata la labor de parto prematuro? Una vez que esté en el hospital, la enfermera le colocará un monitor sobre el abdomen para controlar si hay contracciones y los latidos del corazón de su bebé. Puede que el doctor le examine el cuello del útero para detectar si ha comenzado a dilatarse. Si tiene pocas o ninguna contracción y el cuello del útero está cerrado, puede que la envíen a su casa a descansar. Es posible que su médico le recomiende reposo en cama. Siga las recomendaciones de su médico para saber cuando puede aumentar la actividad física. Medicinas para la labor de parto prematuro 1. Terbutaline: Una inyección o píldora que puede detener las contracciones. Efectos secundarios: Incluyen sentir calor, temblores y sentirse nerviosa. También puede tener cefalea, náusea o vómito. Si siente dolor en el pecho o tiene dificultad para respirar, vea a su doctor de inmediato. 2. Sulfato de Magnesio: Se suministra por vía endovenosa (IV). Efectos secundarios: Incluyen sentirse caliente, estreñida, somnolienta y cansada. (Si le dan esta medicina, le extraerán sangre cada 6 horas). 3. Antibióticos: Puede administrarse para prevenir o para tratar una infección. Manténgase alerta a las señales de la labor de parto prematuro, aún mientras está en reposo o tomando medicinas. Recuerde, prevenir o detener la labor del parto prematura ayuda la posibilidad del nacimiento saludable de su bebé.
