Lea este folleto para enterarse más sobre el procedimiento, cómo prepararse para éste, qué esperar durante y después del procedimiento y cómo obtener sus resultados. Qué es una biopsia guiada por ultrasonido? La ecografía utiliza ondas sonoras para tomar imágenes de los órganos y tejidos de su cuerpo. La biopsia es la extracción de un pedazo pequeño de tejido de su cuerpo. La manera más fácil de extraer este tejido de manera segura es con una aguja. Luego se envía la muestra de tejido al laboratorio para que se la examine. Para hacer una biopsia con aguja, el radiólogo insertará una aguja a través de la piel y dentro del tejido. Usted puede someterse a una biopsia del tejido de un órgano, tal como el hígado o los riñones. O, puede someterse a una biopsia de otros tejidos, tal como el tejido muscular o nódulos linfáticos, o tejido de otra parte de su cuerpo. Se puede utilizar una jeringa o una aguja automatizada para tomar la muestra de tejido. Su médico utilizará las imágenes de ultrasonido para guiar la aguja de biopsia exactamente hacia el lugar correcto en su cuerpo. Cómo funciona el procedimiento? Antes del procedimiento, la enfermera de Radiología insertará una vía intravenosa (IV) en una vena en su mano o su brazo. Es posible que usted reciba líquidos o medicamentos que le ayuden a relajarse, a través de esta IV. Luego de aplicarle gel en la piel, el ecografista (técnico en ultrasonido) hace presión con un transductor contra su piel. El transductor es un dispositivo que se sostiene con la mano, que envía y recibe ondas de sonido. A medida que las ondas de sonido se reflejan nuevamente desde los líquidos y tejidos de su cuerpo, se crea una imagen de sus tejidos en un monitor. 2 Durante la biopsia guiada por ultrasonido, usted se recostará sobre una mesa. Usted tendrá que mantenerse bastante quieto durante hasta una hora o más. Cómo debería prepararme para la ecografía? Se le pedirá que se ponga una bata de hospital antes del examen. Todos sus efectos personales permanecerán con usted durante su visita. Es posible que se le pida que se quite las joyas, anteojos o cualquier otra cosa que esté en el área de la biopsia. No coma ni beba nada durante al menos 6 horas antes de su ecografía. Si usted tiene diabetes y toma insulina, hable con el médico que controla su diabetes. Es posible que tenga que ajustar su insulina, ya que no podrá comer ni beber nada durante 6 horas antes de la ecografía. Tome sus otros medicamentos regulares como los recetó su médico, a menos que su médico o el personal de Radiología le hayan indicado de otra manera. Si usted normalmente toma aspirina u otros medicamentos anticoagulantes (adelgazadores de la sangre), siga las instrucciones para no tomarlos que le dio el personal de programación de Servicios de Imágenes. Ellos han consultado con nuestros médicos clínicos acerca de si usted debería dejar de tomar este medicamento antes de la biopsia. Infórmele al radiólogo o al ecografista si existe alguna posibilidad de que esté embarazada. Asegúrese de traer a alguien que le lleve a casa después de la biopsia. Usted no puede conducir hasta casa ni tomar autobús ni taxi. Cómo se realiza una biopsia guiada por ultrasonido? Primeramente, la enfermera de Radiología insertará una vía intravenosa (IV) en una vena en su mano o su brazo. Es posible que, a través de esta IV, usted reciba líquidos o medicamentos que le ayuden a relajarse durante el procedimiento. También es posible que se le administre un sedante, a través de la vía intravenosa, antes de la biopsia. El ecografista le ayudará a acomodarse en la mesa de ultrasonido. Se utilizará almohadas para ayudarle a mantener la posición correcta durante la ecografía. Las primeras ecografías confirmarán el área de la biopsia y la manera más segura de llegar a esa área. Una vez que se confirme la ubicación del área de la biopsia: -- Se marcará en su piel el sitio de inserción de la aguja. -- Se frotará y desinfectará el área en torno al sitio de inserción y se colocará sobre este una gasa estéril. 3 -- Se inyectará un anestésico local. Esto adormecerá la trayectoria de la aguja. -- Se le hará una pequeña incisión en la piel. Se insertará la aguja de biopsia a través de esta insición. Se le pedirá que no se mueva ni tosa durante el procedimiento. También se le pedirá que sostenga la respiración en diferentes momentos. Es muy importante que usted trate de sostener la respiración cada vez que se le diga. Esto asegurará que la aguja esté obteniendo el tejido del área correcta. Usando como guía las imágenes de ultrasonido, su médico dirigirá la aguja hacia el lugar correcto y extraerá una pequeña cantidad de tejido. Es posible que se tome varias muestras de tejido, pero normalmente se necesita solamente 1 punción de la piel. Luego de tomar todas las muestras, se retirará la aguja. Luego de que se haya detenido todo sangrado en el sitio de la incisión, se colocará una venda sobre la incisión. El sangrado será menor y no necesitará puntos de sutura. Luego de la biopsia, usted irá al área de permanencia limitada. Ahí se le controlará en cuanto a cualquier problema. A la mayoría de los pacientes se les controla durante 4 horas o más antes de que salgan del hospital. Qué sentiré durante el procedimiento? Cuando usted reciba el anestésico local para adormecer la piel, sentirá un leve pinchazo de la aguja. Podría sentir una sensación de ardor a medida que se inyecta y hace efecto el medicamento. El área se adormecerá en un corto tiempo. Es posible que sienta la presión de las manos del médico o de la aguja de biopsia en sí. Qué debería esperar después del procedimiento? Normalmente, se les dice a los pacientes que pueden quitarse la venda al día siguiente del procedimiento. Usted puede bañarse o ducharse normalmente al día siguiente del procedimiento. No haga ningún ejercicio físico intenso, como levantar objetos pesados, subir muchas escaleras ni actividades deportivas la noche de la biopsia y durante todo 1 día después. Puede volver a sus actividades normales 2 días después de la biopsia, si se siente bien para hacerlo. Hable con su radiólogo si tiene planeado viajar por aire en el transcurso de 24 horas después de la biopsia. El sitio de la biopsia podría estar dolorido a medida que pase la anestesia local. 4 Cuándo llamar para pedir ayuda Es raro que ocurra sangrado severo de una biopsia. Ocurre en menos del 3% de los pacientes (menos de 3 de cada 100 pacientes). Algunos síntomas que podrían ser signos de sangrado son: Dolor en el lugar que se tomó la biopsia Pulso (ritmo cardíaco) rápido Debilidad general Piel pálida Si usted tiene alguno de estos síntomas, vaya a la sala de emergencia más cercana o llame al Llame a su médico tan pronto como pueda después de que haya recibido tratamiento de emergencia. Quién interpreta los resultados y cómo los obtengo? El radiólogo enviará un informe detallado a su médico que le refirió para la biopsia. Podría tomar algunos días a una semana, o más, para que su médico reciba el informe de la patología de su biopsia. Este informe proporciona los detalles sobre lo que se encontró cuando se examinó el tejido de la biopsia en el laboratorio. Su médico o enfermera referente hablarán con usted sobre estos resultados. 5 UW MEDICINE PATIENT EDUCATION Ultrasound-Guided Biopsy About your procedure This handout explains an ultrasound-guided biopsy. Read this handout to learn more about the procedure, how to prepare for it, what to expect during and after your procedure, and how to get your results. What is an ultrasound-guided biopsy? An ultrasound scan uses sound waves to take pictures of your body s organs and tissues. A biopsy is the removal of a small piece of tissue from your body. A needle is the easiest way to remove this tissue safely. The tissue sample is then sent to the lab to be examined. To do a needle biopsy, the radiologist will insert a needle through your skin and into your tissue. You may have a biopsy of organ tissue, such as the liver or kidneys. Or, you may have a biopsy of other tissues, such as muscle or lymph node tissue, or tissue from another part of your body. A syringe or an automated needle may be used to take the tissue sample. Your doctor will use the ultrasound images to guide the biopsy needle to exactly the right place in your body. How does the procedure work? After gel is applied to your skin, the sonographer (ultrasound technologist) presses a transducer against your skin. A transducer is a hand-held device that sends and receives sound waves. Before your procedure, the Radiology nurse will insert an intravenous (IV) line into a vein in your hand or arm. You may receive fluids or medicine to help you relax through this IV. As the sound waves reflect back from your body s fluids and tissues, a picture of your tissues is created on a monitor. During an ultrasound-guided biopsy, you will lie on a table. 6 How should I prepare for the scan? You will be asked to change into a hospital gown before your exam. All of your belongings will stay with you during your visit. You may be asked to remove jewelry, eyeglasses, or anything else that is in the area to be biopsied. Do not eat or drink anything for at least 6 hours before your scan. If you have diabetes and take insulin, talk with your doctor who manages your diabetes. You may need to adjust your insulin, since you will not be able to eat or drink anything for 6 hours before your scan. Take your other regular medicines as prescribed by your doctor, unless your doctor or the Radiology staff has told you otherwise. If you normally take aspirin or another anticoagulant medicine (blood thinners), follow the instructions for not taking it that the Imaging Services scheduling staff gave you. They have checked with our clinic doctors about whether you should stop taking this medicine before your biopsy. Tell your radiologist or the sonographer if there is any chance you may be pregnant. Make sure you bring someone to drive you home after your biopsy. You may not drive yourself home or take a bus or taxi. How is the ultrasound-guided biopsy done? First, the Radiology nurse will insert an intravenous (IV) line into a vein in your hand or arm. You may receive fluids or medicine through this IV to help you relax during your procedure. You may also be given a mild sedative through your IV before your biopsy. The sonographer will help place you on the ultrasound table. Pillows will be used to help you hold the correct position during your scan. The first few scans will confirm the area to be biopsied and the safest way to reach this area. Once the location of the area to be biopsied is confirmed: The needle insertion site will be marked on your skin. The skin around the insertion site will be scrubbed and disinfected, and a sterile drape will be put over it. A local anesthetic will be injected. This will numb the path of the needle. A small incision will be made in your skin. The biopsy needle will be inserted through this incision. Page 2 of 4 Ultrasound-Guided Biopsy UWMC Imaging Services Box

7 You will be asked not to move or cough during your procedure. You will also be asked to hold your breath at different times. It is very important that you try to hold your breath each time you are asked to. It will ensure that the needle is getting tissue from the right area. Using the ultrasound image for guidance, your doctor will direct the needle to the right place and remove a small amount of tissue. Several tissue samples may be taken, but only 1 skin puncture is usually needed. After all the samples are taken, the needle will be removed. After any bleeding at the incision site has stopped, a bandage will be placed over your incision. Bleeding will be minor and you will not need stitches. After your biopsy, you will go to the limited-stay area. There, you will be monitored for any problems. Most patients are monitored for 4 or more hours before they leave the hospital. What will I feel during the procedure? When you receive the local anesthetic to numb your skin, you will feel a slight pin prick from the needle. You may feel a burning sensation as the medicine is injected and takes effect. The area will become numb in a short time. You may feel pressure from the doctor s hands or from the biopsy needle itself. What should I expect after the procedure? Usually, patients are told they may remove their bandage the day after their procedure. You may bathe or shower as normal the day after your biopsy. Do not do heavy physical exercise such as heavy lifting, a lot of stair climbing, or sports activities the night of your biopsy and for 1 full day afterward. You may return to your normal activities 2 days after your biopsy, if you feel up to it. Talk with your radiologist if you plan to travel by air within 24 hours after your biopsy. Your biopsy site may be sore as the local anesthesia wears off. It should start to feel better 12 to 48 hours after your procedure. Page 3 of 4 Ultrasound-Guided Biopsy UWMC Imaging Services Box

8 When to Call for Help Severe bleeding from a biopsy is rare. It occurs in less than 3% of patients (less than 3 out of 100 patients). Some symptoms that might be signs of bleeding are: Pain where the biopsy was taken Rapid pulse (heart rate) Overall weakness Pale skin If you have any of these symptoms, go to the nearest emergency room or call Call your doctor as soon as you can after you have received emergency treatment. Who interprets the results and how do I get them? The radiologist will send a detailed report to your doctor who referred you for your biopsy. It may take a few days to a week, or more, for your doctor to get the pathology report from your biopsy. This report gives the details about what was found when the biopsy tissue was examined in the lab. Your referring doctor or nurse will talk with you about these results. What else do I need to know? The radiologist or physician assistant who does the procedure will explain this procedure to you in detail before your biopsy begins. They will describe the way it is done and possible problems, and will give you instructions and self-care tips for after your procedure. Be sure to ask all the questions you may have. You will need to sign a consent form that says you understand what you talked about and that you agree to have this procedure. Questions? Your questions are important.
