1 August 20, 2012 Dear Parent(s)/Guardian(s): Your child may be eligible to receive free tutoring through the Supplemental Educational Services (SES) program for the school year. What is SES? Supplemental Educational Services are additional academic instruction designed to increase students academic achievement in reading/language arts, mathematics, or science. This tutoring takes place outside of the regular school day by state-approved SES providers. How does my child qualify? Students who scored at Level 1 or Level 2 on the 2012 Florida Comprehensive Assessment Test and are enrolled in a Title I school are eligible for SES services. How do I apply? First, choose a provider for your child from the list of state-approved providers included in this SES Enrollment Packet. Then, complete the SES Application Form, also included in the packet, and list three providers in the order of your preference. The District will try to assign your child to your first choice. If your first choice is not available, your child will be assigned to your second or third choice. Students will be prioritized if there are not enough funds to offer services to all eligible students whose parents request the services. The enrollment period is from September 18, 2012, through September 28, You must return the SES Application Form on or before Friday, September 28, 2012, to your child s school or mail/deliver it to: Title I Administration, Attn: SES Enrollment 1450 N.E. 2 Avenue, Room 500 Miami, FL If you send the Application Form by U.S. mail, the SES Application must be postmarked by September 28, Also, you may submit the SES Application Form online by visiting the Parent Portal at: /parents/parents.htm.

Es un programa de Servicios Educaciones Suplementarios que ofrece instrucción académica adicional diseñado con el fin de incrementar el logro académico estudiantil en la lectura/artes del lenguaje, matemáticas y ciencias. Estas clases particulares toman lugar fuera de las horas regulares de clase por proveedores de SES aprobados por el estado. Cómo pudiera ser elegible mi hijo? Los estudiantes que recibieron calificación de Nivel 1 o Nivel 2 en la prueba de Evaluación Integral de la Florida en el 2012 (Florida Comprehensive Assessment Test) y están matriculados en una escuela de Título I son elegibles para recibir los servicios de SES. Cómo puedo hacer la solicitud? Primero, seleccione un proveedor de la lista de los proveedores aprobados por el estado que se incluye con los documentos que componen el SES Enrollment Packet.. Después, llene la solicitud de inscripción de SES (SES Application Form) también incluida y seleccione tres proveedores en el orden de su preferencia.
