These yearly services include: *Clinical Breast Exam *Pelvic Exam Cholesterol Testing Diabetes Screening/New Diabetes Prevention Program **Mammogram ELIGIBILITY FOR PROGRAM: Age Uninsured woman Insured with high deductibles, Or co-pays Medicaid Income Guidelines: Family Size/Income 1. $29, $40, $50, $60, $71, $81,450 If you meet these requirements, you qualify for the program and there will be no charge for any of these services. Here s why Breast Self Exam and mammography can save your life! Average-size lump found by women untrained in BSE.
