Ejemplos cómo los productos de PTW ayudan a prevenir errores: Octavius, SW DIAMOND, David, 1.- TPS Dose calculation error in Varian Eclipse PCB 11.0 (11-02-2013) Description In this example the PBC 11.0 dose calculation algorithm of Varian Eclipse produced an error in the dose calculation when different energies are used within one plan. The resulting error can be in the order of 15 to 30 %. This would surely have been detected by a plan verification measurement. To cite the field safety note from Varian: The issue may be detected during plan QA using a phantom (not Portal Dose Prescription), if the verification plan is created by placing all fields into the same verification plan. The plan will then be calculated in the same way as the treatment plan using the incorrect energies, and the discrepancy should be detected during the QA. If, however, each field is placed into a separate verification plan, the dose calculation would use the energy associated with that individual field since each plan would only contain a single field, It would therefore be unlikely for the plan QA to detect whether the treatment plan was incorrectly calculated unless other information were also used. ó + + VeriSoft DIAMOND SW
2.- Wrong dose calculation when a plan is reloaded (Cyberknife and TomoTherapy MultiPlan TPS) (15-02-2012) Description Under certain conditions, the dose was recalculated to a value which was much too low, when a treatment plan was reloaded into the TPS. Possibly, the user would then increase the dosage to arrive at what he might think was the planned dose and hence treat the patient with an overdose of radiation. 3.- When you manually change MLC-Settings in Brainlab iplan, the dose will not be updated, dose exports will be wrong (2013) Description When you manually change MLC-Settings (e.g. their position), and then export the plan in BrainLAB iplan, the calculated dose is not updated. This happens only under certain conditions and only in version 4.0 and 4.1 of the software. 4.- MLC positions freeze during treatment (Elekta DMLC IV) Description The sensor telling the linac where the gantry currently is located, may malfunction. In can then happen that the gantry continues to rotate, but the MLC does not know this and hence freezes in its position while the beam is still on. This malefunction could happen during plan verification (OCTAVIUS would help) or during treatment (DAVID would help). 5.- Shift of isocenter when using Brainlab ExacTrac (2013) Description Using ExacTrac in combinaiton with a non-brainlab TPS the isocenter during treatment can be shifted due to data that can be misinterpreted. Such a shift can lead to a serious mistreatment of the patient. Such a shift would be detected with DAVID. 6.-A cable resistance problem can lead to wrong collimator positioning in Varian linac Description Due to a cable-resistance which is slightly larger than anticipated, the position-check of the collimator leaves can be misinterpreted by the linac. In result of this, wrong collimator positioning is possible. This can be detected in daily QA by comparing the radiation and light field. For any field size, this can be accomplished using FIELDCHECK with a STARCHECK and for some selected field sizes it can also be performed using the MultiCheck software and an OCTAVIUS array.
