El protector solar, gel de silicona y masajes pueden ayudar a que la cicatriz quede lo mejor posible. Cómo cambiará la cicatriz con el tiempo? Todas las cicatrices se ven rosadas y firmes al principio. Con el tiempo se ablandan más y el color se aclara. Este proceso puede llevar de 6 meses a 2 años. En algunos casos, la cicatriz continúa cambiando por más tiempo. Los cambios por los que pasa una cicatriz varían para cada persona. A algunas les queda más marca que a otras. Esto se debe al tipo de piel y a cómo cicatriza. Hay varias cosas que usted puede hacer para que la cicatriz de su niño quede lo mejor posible. Protección del sol Las cicatrices deben protegerse mucho del sol al menos el primer año después de una cirugía o lesión. El sol puede oscurecer la cicatriz permanentemente y que sea más notable. Después de dos semanas de la cicatrización de la herida, se puede comenzar a usar protector solar. Aplique el protector durante todo el año, no sólo durante el verano. Los protectores solares contienen sustancias que bloquean los rayos ultravioleta (UV) que dañan la piel. También puede proteger la cicatriz del sol con la ropa o con curitas. Cómo elijo el protector solar? Los protectores solares vienen en muchas formas: crema, barra, bálsamo para labios y en aerosol. Evite el aerosol porque puede ser peligroso si se inhala o se ingiere. El número SPF (factor de protección solar) aparece en la etiqueta. Elija un protector solar con SPF entre 30 y 50. En la etiqueta también debe decir que es un protector solar de amplio espectro (full spectrum) o que protege contra los rayos UVA y UVB.Prefiera los que tienen Parsol 1789 (avobenzono), dióxido de titanio, óxido de zinc o Meroxyl SX en la lista de ingredientes. Evite los que contienen oxybenzone o vitamina A. Evite los protectores solares con repelente de insectos porque no deben aplicarse tan a menudo como el bloqueador solar. Para bebés y niños con piel sensible use un protector que tenga óxido de zinc o dióxido de titanio. Aplique protector solar de 20 a 30 minutos antes de salir al aire libre. Use una buena cantidad para todo el cuerpo, como si llenara una pelota de golf. 1 de 3

2 Cuidado de la cicatriz Reaplique por lo menos cada 2 horas después de estar en el agua o de sudar mucho. Para actividades acuáticas debe usar el water-resistant (resistente al agua) cada 40 a 80 minutos. Evite la luz directa del sol entre 11:00 a.m. y 3:00 p.m. ya que a esas horas es más fuerte. También es más intenso en altura y en zonas cercanas al ecuador como México y Hawaii. Use protector solar incluso en días nublados. Las nubes ni la neblina bloquean completamente los rayos UVA y UVB. Tenga cuidado cerca de la arena, el agua, la nieve y el cemento pues reflejan los rayos del sol. Gel de silicona El gel de silicona en la cicatriz de su niño puede mejorar la apariencia. Se compra sin receta en almacenes y en Internet. Si quiere usar el gel de silicona comience unas 2 semanas después de la cirugía o cuando la herida haya cicatrizado bien. Es decir, cuando esté seca, sin costras y cuando las tiritas adhesivas y los puntos se hayan salido. El gel de silicona viene en 2 formas: láminas de silicona y gel de silicona líquida. Láminas de silicona Es un gel de silicona espeso y pegajoso que viene en una hoja que puede cortar como para que cubra la cicatriz. Igual que una calcomanía, tiene un lado pegajoso. Quite el papel del lado pegajoso para colocarlo directamente sobre la cicatriz. Las hojas de gel de silicona deben quitarse antes de bañarse o meterse en el agua. Son reusables pero cuando se quitan se deben enjuagar o lavar ligeramente con agua y jabón y dejar que se sequen. Cuando se secan se pone pegajosa y se puede usar de nuevo. Gel de silicona líquida Este tipo de gel es un líquido espeso y pegajoso que puede aplicar con la yema del dedo. Frote un poco sobre la cicatriz y déjelo secar. Queda una capa transparente que se seca en 2 a 3 minutos y más tarde se cae en escamas, solo con el movimiento de su niño. Debe aplicarse 2 veces al día y después de bañarse o meterse en el agua. Qué tipo de gel de silicona es mejor para mi hijo? El mejor tipo (láminas o líquido) depende de las necesidades de su hijo. La hoja de gel de silicona se nota y también es reusable. Pero, si su hijo no se deja la hoja sobre la cicatriz, se la mete en la boca o no le gusta como queda, puede usar el gel de silicona líquida. El gel de silicona líquida también puede ser más fácil de usar con cicatrices muy pequeñas o en lugares del cuerpo donde no es fácil que la lámina se pegue. 2 de 3

Para protegerse del sol, busque láminas que no tengan papel transparente. También se puede cubrir con curitas o cinta. El gel de silicona líquido se puede aplicar sobre el protector solar. Aplique primero el protector solar, frótelo y espere unos minutos hasta que la piel lo absorba. A continuación aplique el gel de silicona. Masajes en la cicatriz Los masajes se aplican frotando los dedos sobre la cicatriz de su niño con regularidad. Puede usar cualquier tipo de crema, loción o aceite (vaselina) que acepte su hijo. Use el pulgar u otro dedo para frotarla en tres direcciones: De una punta a la otra de la cicatriz. De un lado al otro, a lo ancho de la cicatriz. En círculos pequeños encima de la cicatriz. Aplique suficiente presión para que la piel se ponga blanca y la cicatriz se extienda pero sin que duela. Trate de dar los masajes por 3 a 5 minutos, 2 a 3 veces al día. El masaje no debe ser doloroso para su niño. Si le duele pare y vuelva a intentarlo después de una semana o dos. No está claro cuánto ayuda el masaje, si su hijo no lo tolera no es necesario. Cuándo debo llamar al doctor? Llame al especialista de su niño o a su pediatra si la cicatriz: Comienza a doler. Tiene ampollas. Se vuelve a abrir. Aparece una erupción o sarpullido. 5 Patient and Family Education Scar Care Your child s scar will change as it heals. Sun protection, silicone and massage can help your child have the best scar possible. How will the scar change over time? All scars are pink and firm at first. Over time, they become softer and lighter in color. This process can take 6 months to 2 years. For some, a scar will continue to change for even longer. How a scar changes over time is different for everyone. Some people have more scarring than others. This is related to skin type and how the skin heals. There are several things you can do to help your child have the best scar possible. Sun protection Scars should be carefully protected from the sun for at least 1 year after surgery or injury. Sun exposure can darken scars permanently, making them more noticeable. After about 2 weeks of healing, you can start applying sunscreen over your child s scar. Apply sunscreen in every season, not just in the summer. Sunscreens contain substances that stop harmful ultraviolet (UV) light from damaging the skin. You can also use clothing or Band-Aids to protect the scar from the sun. How do I choose a sunscreen? Sunscreens come in many forms, like lotions, creams, sticks, lip balms and sprays. Avoid spray sunsceens becuase they can be dangerous if inhaled or swallowed. Look for an SPF (Sun Protection Factor) number on the label. Choose a sunscreen with an SPF of 30 to 50. The sunscreen label should also say that it is a full spectrum sunscreen, or that it protects against both UVA and UVB rays. Look for sunscreens that have zinc oxide, titanium dioxide, Parsol 1789 (avobenzene), or Meroxyl SX in the ingredient list. Avoid sunscreens with oxybenzone or vitamin A. Avoid sunscreens with insect repellent because insect repellent should not be applied as often as sunscreen. For babies and children with sensitive skin, use a sunscreen containing zinc oxide or titanium dioxide. You can browse sunscreen ratings on the Environmental Working Group website: ewg.org. How do I use the sunscreen? Put sunscreen on 20 to 30 minutes before going outside. Use a generous amount, about the size of a golf ball for the whole body. Reapply it at least every 2 hours, and after being in the water or sweating heavily. Use waterresistant sunscreen for water activities and reapply it every 40 to 80 minutes. 1 of 3

6 Scar Care Avoid direct sun between 11 a.m. and 3 p.m. The sun is strongest during these times. Sun is also more intense at high altitudes and closer to the equator, such as in Mexico and Hawaii. Use sun protection even on cloudy days, as clouds and smog do not fully block UVA and UVB light. Be careful when near sand, water, snow, and cement because they reflect the sun s rays and increase your sun exposure. Silicone gel Putting silicone gel on your child s scar might help the appearance of some scars. You can buy it in stores and online without a prescription from your doctor. If using silicone gel, it should be started about 2 weeks after surgery, or when the incision is well healed. This is when the incision is dry, has no scabs or crusts and all the Steri-Strips and stitches are gone. Silicone gel comes in 2 forms: silicone sheeting and liquid silicone gel. Silicone sheeting This is a thick, sticky silicone gel that comes in a sheet. You can cut it into pieces to fit the size of your child s scar. Like a sticker, it has a sticky side and a protective backing. When the backing is removed, the sticky side is placed directly over the scar. Silicone gel sheets must be taken off before bathing or swimming. They are reusable. When removed, the piece of silicone sheeting should be rinsed or washed gently with soap and water and allowed to dry. When it is dry, it becomes sticky again and can be reused. Liquid silicone gel This type of silicone gel is a thick, sticky liquid. It can be put over a scar by putting a little on your fingertip, rubbing it on the scar and allowing it to dry. It dries to an invisible, clear layer in 2 to 3 minutes. It flakes off on its own as your child moves around. It should be applied 2 times a day and after bathing or swimming. Which type of silicone gel is best for my child? The best type of silicone gel (sheeting or liquid) depends on the needs of your child. Silicone gel sheeting is visible, so you can see that it is in place. It is also reusable. However if your child will not leave the sheeting on the scar, would put it in their mouth, or doesn t like the way it looks, liquid silicone gel can be used instead. Liquid silicone gel may also be easier to use on very small scars or in places on the body where it is hard to get the silicone gel sheeting to stick. Silicone gel does not give any sun protection. For sun protection, look for silicone sheeting that has a backing you cannot see through. You can also 2 of 3

From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. cover it with a Band-Aid or tape. You can apply liquid silicone gel over sunscreen. Apply the sunscreen first, rub it in and let it get absorbed into the skin for a few minutes. Then apply the silicone gel. Scar massage Scar massage is regularly using your fingers to rub your child s scar. Massaging might help make your child s scar: More soft and flat Less sensitive Less itchy When can I start scar massage? You can begin massaging your child s scar about 1 month after surgery, when the area is completely healed with no open areas, stitches or scabs. How do I do scar massage? You can use any kind of cream, lotion, or oil (like Vaseline) that works for your child. Use your thumb or finger to rub it in and massage over the scar in 3 directions: Up and down the length of the scar Across the scar In small circles over the scar Use enough pressure that the skin turns white and the scar stretches, but not so much that it hurts. Try to massage the scar for 3 to 5 minutes at a time. You should do this 2 to 3 times a day. The massage should not be painful to your child. If it is painful, stop and try again after a week or two. It is unclear how much massage can help, so if your child does not tolerate it well, you do not have to do it. When should I call the doctor? Call your child s specialty clinic or their primary care provider, if the scar: Becomes sore Blisters Reopens Develops a rash Seattle Children s offers interpreter services for Deaf, hard of hearing or non-english speaking patients, family members and legal representatives free of charge.
