1 Cirugía de Glaucoma: Dispositivos de Implante Algunos de los implantes utilizados son el Ahmed y el Baerveldt. El nombre les viene de sus inventores. Cirugía de glaucoma La cirugía es utilizada en el tratamiento del glaucoma para disminuir la presión en el ojo. Se realiza cuando usted está tomando la dosis más alta de sus medicamentos y la presión en sus ojos aún está demasiado alta. Usted tiene riesgo de sufrir daño en el nervio óptico. El objetivo de la cirugía es el de disminuir la presión en el ojo a través de la creación de un nuevo canal para drenar el fluido dentro del ojo. La propia respuesta de curación del cuerpo es la de formar una cicatriz, la cual puede cerrar con el tiempo las nuevas áreas de drenaje. Con frecuencia, se usa un implante en pacientes que han tenido cirugía (trabeculectomía) sin éxito, o en pacientes con un tipo complejo de glaucoma. Qué es un implante y cómo funciona? El implante se parece a una placa de plástico unida a un tubo. El tubo se coloca en la parte frontal del ojo, y el fluido puede fluir a través del tubo hasta la placa, la cual está colocada detrás del ojo. La placa mantiene un espacio entre los tejidos de la superficie del ojo, donde el fluido puede acumularse y ser absorbido por el cuerpo. Cirugía de implante Antes de la cirugía, le colocarán una línea intravenosa (aguja en la vena del brazo). Le administrarán medicamentos para ayudarle a relajarse a través de la línea intravenosa. Le pondrán gotas en el ojo que van a operar. El ojo y el área alrededor del mismo estarán adormecidos de forma que no sienta dolor.

2 Le limpiarán el ojo con un limpiador líquido de color amarillo. Le colocarán una sábana de protección sobre la cara. El cirujano realizará una incisión en la conjuntiva (la cubierta delgada exterior del ojo). El dispositivo de implante de la placa será colocado sobre la esclerótica (parte blanca del ojo) aproximadamente en el medio de la órbita del ojo y se asegurará con puntos de sutura. Parte del dispositivo puede pasar por debajo de algunos de los músculos del ojo. Se realizará un pequeño agujero donde la córnea clara (parte frontal del ojo) se une a la esclerótica. La punta de la sonda de implante se introduce en este agujero y permanece en la parte frontal del ojo. Un parche de tejido cubrirá la sonda donde se asienta sobre la parte blanca del ojo. En algunos casos, es posible que la punta de la sonda sea sujetada durante los primeros días o semanas después de ser colocada. El doctor decidirá cuándo abrir la sonda. Esto dependerá de cómo cicatrice el ojo. El agujero en la conjuntiva se cerrará con puntos de sutura. La cirugía de implante dura aproximadamente 2 horas. Se colocará un parche sobre el ojo un escudo de metal sobre el parche. Una vez que le hayan quitado la línea intravenosa del brazo, y cuando se sienta lo suficientemente bien, podrá volver a casa. En la mayoría de los casos, la cirugía con implante tiene éxito. Aproximadamente el 60% de los implantes funcionan tres años después, aunque la mayoría de los pacientes también necesitan tomar medicamentos. La respuesta de formación de tejido cicatricial del cuerpo puede aún causar problemas incluso con un implante. Se puede formar un quiste grueso alrededor de la placa, lo cual puede evitar que drene el fluido del ojo. A veces, se pueden usar medicamentos para evitar que se forme tejido cicatricial. El doctor decidirá si esto es necesario. Dependerá de su edad, si ha tenido o no antes cirugía en el ojo, y de la severidad de su glaucoma.

3 Cuáles son los riesgos? El riesgo más común es que la nueva sonda de drenaje funcione de forma insuficiente o de forma excesiva. Si la sonda de drenaje funciona de forma insuficiente, la presión en el ojo seguirá siendo demasiado alta y necesitará tratamiento adicional para reducir la presión. Usted verá a los doctores con frecuencia para ajustar sus medicamentos, aplicar presión sobre el ojo, o remover la ligadura que puede cerrar la sonda del implante durante un tiempo tras la cirugía. Si la nueva sonda de drenaje funciona de forma excesiva, es posible que el fluido se acumule detrás de la retina (revestimiento interno del ojo) y puede causar pérdida de la visión durante un corto período de tiempo. Es posible que necesite tener una segunda cirugía para volver a llenar el ojo y corregir el problema. Con un implante, la sonda que va de la parte frontal del ojo hasta la placa y la placa en sí están cubiertos con tejido ocular durante la operación. Con el tiempo, es posible que la sonda o la placa se desgaste debido al tejido. Si esto ocurre, es posible que necesite ser reparado. No se trata de un problema común, pero debería llamar a su doctor si nota algún cambio en la apariencia de su ojo. También es posible que el tejido que rodea la placa se vuelva grueso y se cubra con cicatriz. Si esto ocurre, el fluido no podrá ser absorbido y, por lo tanto, puede subir la presión en el ojo. Es posible que necesite recibir tratamiento adicional para corregir este problema. El implante puede ser colocado debajo de varios músculos oculares. Si los tejidos se vuelven más gruesos y se forma tejido cicatricial alrededor del disco, o si el dispositivo en sí es demasiado grueso, esto puede desplazar los músculos fuera de lugar. Si esto ocurre, es posible que experimente visión doble. Es posible que necesite lentes especiales o una segunda cirugía para corregir su visión. Esto se produce en raras ocasiones.

4 Es posible que su visión no sea tan aguda después de la cirugía. Esto sólo debería durar un período corto de tiempo. En raras ocasiones (menos del 5% de los casos), puede producirse una reducción de la visión de forma permanente. Una complicación que se produce muy raramente es que podría quedarse ciego como resultado de la cirugía, o incluso más raro es que fallezca durante la cirugía. Antes de la cirugía Tendrá un examen físico antes del día de la cirugía para asegurar que pueda tener la operación de forma segura. Este examen puede incluir pruebas de sangre, un EKG (evaluación del corazón), y una radiografía del pecho. Debe dejar de tomar anticoagulantes, aspirina, ibuprofeno (Motrin, Advil, Nuprin, Medipren ), naprosyn (Aleve ), medicamentos antiinflamatorios para la artritis, o pastillas para el catarro con estos medicamentos una semana antes de la cirugía ya que puede producir sangrado. Debería hablar con su doctor de cabecera para que le aconseje sobre la suspensión de estos medicamentos. Puede tomar acetaminofén (Tylenol ) si tiene un dolor de cabeza o dolor en general. Una enfermera le llamará el día antes de la cirugía para indicarle a qué hora debe llegar y le dará instrucciones sobre el consumo de comidas y bebidas. Después de la cirugía Deje colocados el parche y el escudo sobre el ojo durante el primer día y noche. Verá al doctor al día siguiente y de forma regular después de eso. Le proporcionará un medicamento para ayudar a que el ojo cicatrice y para prevenir inflamación e infección. Debe dejar de aplicar en el ojo operado los medicamentos para el glaucoma que usaba antes de la cirugía. Le indicarán cuándo puede volver a usarlos, si es necesario. Siga aplicando en el otro ojo los medicamentos para el glaucoma que haya estado usando. Debe usar lentes o el escudo del ojo en todo momento durante las primeras semanas. Para proteger el ojo, debe usar el escudo por la noche. No realice ninguna actividad física que requiera mucho esfuerzo ni aguante la respiración. No levante más de 15 a 20 libras. No se incline hacia delante doblando la cintura; doble las rodillas para agarrar objetos colocados a una baja altura. Puede volver a tener relaciones sexuales cuando se sienta cómodo. Tenga cuidado durante las primeras semanas.

6 Glaucoma Surgery: Implant Devices Some of the implant devices used are the Ahmed and Baerveldt. They are named after their inventors. Glaucoma Surgery Surgery is used in the treatment of glaucoma to lower the pressure in the eye. It is done when you are taking the highest dose of your medicines and the pressure in your eye is still too high. You are at risk for damage to your optic nerve. The goal of surgery is to lower the pressure in the eye by making a new channel to drain the fluid from inside the eye. The body s own healing response is to form a scar, which can close the new drainage sites over time. Often an implant is used in a patient who has had surgery (trabeculectomy) that has failed, or in a patient who has a difficult form of glaucoma. What Is an Implant Device and How Does It Work? The implant device looks like a plastic plate attached to a tube. The tube is placed in the front part of the eye and fluid can flow from the eye through the tube to the plate which is placed behind the eye. The plate holds a space between the tissues on the surface of the eye where the fluid can collect and be absorbed by the body. Implant Surgery Before surgery, you are given an IV (needle in the vein of the arm). Drugs to help you relax are given through the IV. Drops are put in the eye that will have surgery. The eye and the area around it are numbed so that you feel no pain in your eye. The eye will be cleaned with a yellow cleaning liquid. A protective sheet will be draped over your face. The surgeon will make a cut in the conjunctiva (the thin outer cover of the eye). The implant device plate is placed on the sclera (white part of the eye) about halfway back on the eyeball and is stitched down. Part of the device may pass under some of the muscles of the eye. A small hole is made where the clear cornea (front of the eye) meets the sclera. The tip of the implant tube is tucked into this hole and remains in the front part of the eye. A patch of tissue will

7 cover the tube where it lays on the white of the eye. In some cases, the tube s tip may be tied off for the first few days to weeks after it is put in. The doctor will decide when to open the tube. This depends on how your eye heals. The hole in the conjunctiva is sewn shut. When the tube is opened, the fluid inside the eye will flow through the tube to the space made by the plate between the conjunctiva and sclera. The body will absorb this fluid. Implant surgery takes about 2 hours. A patch is placed over the eye, and a metal shield is placed over the patch. Once the IV line has been removed from your arm and you feel well enough, you can return home. Surgery using an implant device is most often a success. Three years later, about 60% of implants are working, though most patients need medicines as well. The body s scarring response can still cause problems even with an implant device. A thick cyst can form around the plate, which can prevent drainage of the eye fluid. Sometimes drugs can be used to prevent scar tissue from forming. The doctor will decide if this is needed. This will depend your age, if you have had eye surgery before, and how severe your glaucoma is. What Are the Risks? The most common risk is that the new drain works too little or too well. If the drain works too little, the pressure in the eye remains too high and further treatment is needed to lower the pressure. You and the doctors will see each other often to adjust your medicines, push on the eye, or remove the tie that may close the implant tube for a while after surgery. If the new drain works too well, fluid may build up behind the retina (inner lining of the eye) and may cause a loss of vision for a short time. You may need to have a second surgery to refill the eye and correct the problem. With an implant device, the tube which leads from the front of the eye to the plate and the plate itself are covered with eye tissue during the operation. Over time, the tube or plate may wear through the tissue. If this happens, you may need to have it repaired. This is not a common problem, but you should call your doctor if you notice any change in the way your eye looks.

8 The tissue around the plate may also get thick and scarred. If this happens, the fluid cannot absorb, so the eye pressure may go up. You may need further treatment to correct the problem. The implant device may be placed under some of the muscles in your eye. If the tissues thicken and scar around the plate or if the device itself is too thick, the muscles can be pushed out of place. You may notice double vision if this occurs. You may need special glasses or a second surgery to correct your vision. The tube is carefully placed in the front of the eye during the surgery. You may be able to see its tip if you look very closely, though it should not be noticed by others. If the tip of the tube touches the cornea (front surface of the eye) it may cause damage. If the tip touches the lens of the eye, it may form a cataract. It is rare for this to happen. Your vision might not be as sharp after your surgery. This should last only a short time. Rarely (less than 5% of cases), vision may be reduced permanently. A very rare complication is that you could go blind as a result of surgery, or much rarer still, to die during surgery. Before the Surgery You will have a physical before the day of surgery to make sure that it is safe for you to have an operation. This exam may include blood tests, an EKG (heart tracing), and a chest x-ray. You should stop taking blood thinners, aspirin, ibuprofen (Motrin, Advil, Nuprin, Medipren ), naprosyn (Aleve ), anti-inflammatory arthritis medicines, or cold pills with these drugs one week before the surgery since they can cause bleeding. You should check with your own doctor for advice on stopping these drugs. Acetaminophen (Tylenol ) is fine to take for headache or pain. A nurse will call you the day before surgery to tell you what time to arrive and give you eating and drinking instructions.

9 After the Surgery Leave the patch and shield on the eye for the first day and night. You will see the doctor the next day and on a regular basis after that time. You will be given medicine to help the eye heal and to prevent swelling and infection. You should stop the glaucoma medicine you were taking in the eye that had surgery. You will be told when to restart them, if needed. Keep using the glaucoma medicines you may have been taking in the other eye. Glasses or the eye shield should be worn at all times for the first few weeks. The eye shield should be worn at night to protect the eye. Do not do any physical activity which makes you strain or hold your breath. Do not lift over 15 to 20 pounds. Do not bend over from the waist; bend at the knees to reach low objects. You may resume sexual activities when you are comfortable. Be careful for the first few weeks.
