2 OUR CHILDREN CANNOT GROW SPIRITUALLY WITHOUT OUR WITNESS OF FAITH Registration Information & Criteria for Religious Education Registration and Fees Classes begin: the week of September 27th, Space is limited, so please sign up Early! The cost of registration: $60 for the first child and $10 for each additional child per family until August 12th 4 PM. Late Fee: August 16th - October 14th, 4 PM, an additional $20per family will be added; this is only if space is available. There will be no further registration after October 14th. Minimum Age: Each child must be at least in the first grade in order to be able to register. First Holy Communion: Two consecutive years of Religious Education are required. please initial First Year: Faith Formation; This is the first year of a two-year program. Students must have completed Mass Agreements and Class requirements in year One in order to enroll into Year 2 please initial Second Year: 1st Reconciliation during Lent, First Holy Communion in May (provided that Mass and Class requirements are completed). please initial Pre-Confirmation: This is a program for 3rd - 5th and 6th grade students. Students must have previously received First Holy Communion. Confirmation: Two consecutive years of Confirmation classes are required please initial Confirmation 1: This is year 1 of a 2-year please initial program. Students must have previously received First Holy Communion and be at least in the 7th grade by the beginning of the Religious Education School Year. Confirmation 2: (2nd year) Students must have completed Mass Agreements and Class requirements for Confirmation 1 to be able to enroll into Confirmation 2. please initial Returning Students: Must re-register for the new school year, including the Confirmation-2 students. CRITERIA NEEDED FOR REGISTRATION 1. A current Religious Education Registration Form completed. 2. A copy of the child s Birth Certificate 3. A copy of the child s Baptism Certificate 4. If entering into Confirmation-1: A copy of your child s First Communion Certificate is required. Note: if your child received First Communion at Assumption Parish, please call the office to make sure a record of it is on file. 5. If entering Pre-Confirmation: A copy of your child s First Communion Certificate is required. If entering into Confirmation-2: a Confirmation Candidate Form must be completed. 7. Safety Policy and this form signed by parent. 8. Fees must be paid at time of registration. 9. If transferring from another Parish: a letter is required from that parish stating that your child has received religious education there and for how long; if this will be your child s sacramental year, the letter should state that your child has been preparing to receive the sacrament and was in good standing. Additional Information: Class Absences from class must be justified in person or in writing by the parents. Up to 5 absences please initial are allowed in one year - any more than that should be discussed between parent, teacher and director; it will be up to the teacher if he/she feels the child knows the lessons enough to pass him/her at the end of the year. Mass Attendance: A minimum of 20 please initial Masses must be completed by May 1, Religious Education News - Parents will be notified through the Sunday Bulletin regarding class events, holidays, parent/teacher meetings, as well as any other pertinent information relating to your child s classes. 3 Our Lady of the Lake Church April Safety Policy With safety being one of our main concerns for the students who attend the Religious Education Classes we cannot allow any unregistered child into the class. The list of Registered Children will be posted on the classroom doors. Also for the safety of our students, we need to make every effort possible to have the children picked up in a timely manner. We are not in the position to depend on our volunteer teachers who have given freely of their time to stay after class and wait for a parent or guardian to pick up their child. During the winter months it gets dark and the weather changes quickly, so having the children picked up promptly needs to be the norm. Another concern that we have is for the children who walk to their Religious Education Classes and return for home alone or with other children on foot without an adult present. For the safety of your child we request that an adult be with your child while walking to and from the Religious Education Classes. Please also note that students are not allowed to play in the unsupervised parking area/street and would appreciate it if you could talk to your children about this ahead of time. As a reminder, on a public school Snow Day we will not have Religious Education Classes. On the days that the weather turns bad, Father / the teachers will make the decision to cancel the classes by 2:00PM and you will be notified of the decision as soon as possible. As it has been our policy, if the road conditions are dangerous and you do not feel safe driving please do not attempt to drive. The parish s policy in the event of a missing child is: Once a parish staff member is notified that a child is missing that parish staff member will proceed to search the area, call the child s home and within 15 minutes call 911 for a dispatch of an officer to initiate a formal search for the missing child. To avoid any misunderstandings of your child s whereabouts it is very important that parents communicate any transporting changes with all who are in charge of the child s care and continue to promptly drop off and pick up the children. If there are any special instructions for any particular child or particular circumstance it is important that the teacher and the parish staff be notified of such in writing. Si es para la Confirmación-1: Se requiere una copia del Certificado de la Primera Comunión. Nota: Si su niño recibió La Primera Comunión en esta Parroquia, llame por favor a la oficina para asegurarse de que está en el archivo. 5. Si es para la clase de Pre-Confirmación: Se requiere el Certificado de la Primera Comunión. A Un niño le está permitido faltar a clase no más de 5 veces avor inicial en un año cuando se pasen de 5 veces debe ser discutido entre los padres, el maestro y el coordinador; depende del maestro tomar la decisión si es que el niño sabe las lecciones suficientemente como para pasar el año. Una lista de ninos que estan matriculados será en las puertas de las clases. Tambien, los niños no deben llegar más de 5-10 minutos antes de que empieze la clase y los papas deben caminar con sus niños a los cuartos de clase el primer día para estar seguro que hay un maestro(a) en la clase! Si estas en el horario para enseñar o ayudar y no podras estar, usted es responsable para encontrar alguien para reemplazar la nunca asumen que otro padre o maestro va estar. Los ninos no pueden jugar sin supervisión en la aparacamiento o calle. Por favor llega en tiempo para sus niños. Los padres/maestros tambien tienen familias y quieren llegar con sus familias, muchos ya han trabajado todo el día antes de la clase. Estos reglas son para la seguridad de su niño!
