1 Navigate plan What you need to know about the UnitedHealthcare Navigate plan When you enroll in the Navigate plan, you get a health plan that connects you with a primary care physician to help guide you toward the right treatment, or the right specialist, at the right time. This brochure will help answer many questions for you, or you can also find plan information online at welcometouhc.com*. Why do I need to select a primary care physician (PCP)? When you enroll in the Navigate plan, you are required to select a PCP from our Navigate network to help you manage your health and get the care you need. Your PCP will be your first point of contact when you need care and will oversee any treatment you may need. In addition, you must get an electronic referral from your PCP before you see another network PCP or specialist. Depending on your plan, without a referral, your costs may be a lot higher or they may not be covered at all. Check your benefit plan documents for more information on referrals. How do I select a PCP? You will need to select a network PCP when you enroll in the Navigate plan. Your PCP must be located in a town or city near where you, as the employee/ subscriber, lives. If you do not select a PCP or you have chosen a medical practice (not an individual PCP), we will assign one to you. You can find one today, before your effective date, by using our physician search tool, available in both English and Spanish, on welcometouhc.com*, or you can call Customer Care for assistance. For English, please call: For Spanish, please call: Can I select any type of physician as my PCP? Your PCP must be a general practice physician, family practice physician, pediatrician or internal medicine physician. The PCP s name and telephone number will be listed on each family member s health plan ID card. If I am covering a family member and/or dependent under my plan but they live out of state, can they select a PCP near where they live? Unfortunately, no. They must choose a PCP in a town or city near where you, as the employee/ subscriber, lives or works not where the family member and/or dependent lives or works. Again, each family member and/or dependent can select his or her own PCP. If I don t select a PCP at enrollment, how will one be assigned to me? We will assign your PCP based on where you live. If you wish to change your assigned physician, you will have the option to select a new PCP after you enroll, but the new PCP must be in the area where you, as the employee/subscriber, live or work. Can each covered family member have his or her own PCP? Yes. You can select one physician for your entire family, or each covered family member may select his or her own physician. Please check with your physician s office before enrolling to confirm that he or she is accepting new patients and if there are any patient age restrictions. * The welcometouhc.com website provides content in English only, but you can access the provider directory in English or Spanish.

2 What you need to know about the UnitedHealthcare Navigate plan After I enroll, can I change my PCP? Yes. Once you are a member, you will be able to change your PCP on myuhc.com or by calling Customer Care for assistance. For English, please call: For Spanish, please call: If you change your PCP before the 15th of the month, the change will go into effect on the 1st of the next month. Otherwise, it will not take effect until the 1st of the following month. For example, a change request submitted on June 16 will not be effective until August 1. How often can I change my PCP? Once I have a PCP, can I choose to see other PCPs without a referral? No. It s important that you get a referral from your PCP before you can see any other primary care or specialist physician. An electronic referral from your PCP is necessary to receive coverage or the highest level of benefit possible. What is a referral? A referral is an approval from your PCP for you to receive care from another Navigate network physician or health care professional. The electronic referral must be submitted before you see a physician other than your PCP. Your electronic referral can be viewed on myuhc.com under the Physicians & Facilities tab. A paper referral is not a valid referral. Do I need to complete any paperwork for referrals? No. All referrals within your Navigate plan are electronic. When you receive a referral through your PCP, he or she will handle the process for you. Can I view and track my electronic referrals? After you enroll, you will be able to see all of your referrals online at myuhc.com under the Physicians & Facilities tab, or you can call Customer Care for assistance. For English, please call: For Spanish, please call: Do I need a referral before seeing a specialist? Yes. It s important that you get an electronic referral from your PCP before you see another network physician, including specialists. A referral is necessary to receive coverage or the highest level of benefit possible. There are some physicians and specialists who do not require a referral. (See the following question). Are there providers I can see without a referral? Yes. Referrals are not needed to see the following providers as long as they are in the Navigate network: Obstetricians/gynecologists (OB/GYNs) Behavioral health or substance abuse disorder clinicians Convenience care clinics Urgent care clinics Emergency room care (as appropriate) What s the difference between referral and prior authorization? Referral: If your PCP thinks you need treatment that is best provided by a network specialist, he or she will issue a referral for you to see another network physician or a specialist before your appointment. Prior authorization: Prior authorization is the process in which UnitedHealthcare reviews certain health care services to determine medical necessity and eligibility for coverage before the service is received. Prior authorization is required for certain covered health services, as noted in your benefit plan documents. If you do not get prior authorization before receiving one of these services, your benefit coverage may be reduced. You also may have no coverage if it s determined that the service is not medically necessary. For information on which services require prior authorization, see your benefit plan documents. Where can I find information after I enroll? Once you enroll and register on myuhc.com, you can log in to take advantage of the available tools and resources. 3 Find your PCP using the provider directory, available in English and Spanish, at welcometouhc.com*. Remember, you need to select a PCP when you enroll in the Navigate plan. If you don t select a physician, or if the physician you selected is not participating in the UnitedHealthcare Navigate network, a new physician will be assigned to you. Step 1: Go to welcometouhc.com* and under Health Plans, click on Navigate Plan, then select the Navigate plan available to you. 4 Step 3: Once you decide on the physician you want, print or write down all of his or her information, including the physician s address, telephone number and 13-digit ID number (circled). When completing the form, include the first and last name for all physicians selected by you and any covered family members and/or dependents. Be sure to include the entire 13 digit physician identification (ID) number for each physician listed on the form. Para ciertos servicios de salud cubiertos se requiere autorización previa, tal como se indica en los documentos de su plan de beneficios. Si no obtiene una autorización previa antes de recibir alguno de estos servicios, su cobertura de beneficios podría reducirse. Además, es posible que no tenga ninguna cobertura si se determina que el servicio no es médicamente necesario.
